A checklist was used to note the requirements that I monitored today. During today’s visit I monitored one (1) NC Pre-K classroom and one (1) developmental day classroom designated for 4-and 5-year-old children. The lead teachers in each classroom assisted me during today’s visit. I observed a total of twenty-eight (28) children present between both classrooms. Ratio, group size, supervision, capacity and approved space were in compliance. The children were observed in free choice activities, outdoor play and eating lunch. doing. ** License Status: Currently this center operates with a five-star rated licensed issued October 3, 2024. ** Annual Inspections: The last sanitation inspection was completed April 2, 2026, and received a ‘Superior’ classification with 2 demerits. The last fire inspection was completed on January13, 2026. The last documented fire drill was conducted on March 24, 2026, the last documented lockdown drill was completed on March 5, 2026, and the last documented playground inspection was completed on April 20, 2026. During today’s visit, a full assessment of Child Care Requirements was conducted. I observed all required postings, attendance logs, and NC Pre K requirements. I reviewed a sample of children’s records, staff files and all applicable program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The lesson plan was not current. GS 110-91(12); .0508(a) The violation cited during today’s visit was corrected therefore, a compliance letter is not needed. Prior to today’s visit, the 18-month compliance history for this facility was 100%. Rated License: Currently this center operates with a five-star rated license issued October 3, 2024, earning 6 points in staff education, 7 points in program standards, and 1 quality point by all lead teachers having an AAS or higher in early childhood education or child development. The application for a rated license was completed on 4/23/26. The program chose the classroom and instructional quality pathway. The facility is using enhance reduced ratio, enhanced space and has an NC Pre-K classroom. The facility is applying for a five-star rated license using Creative Curriculum and formative assessment Teaching Strategies Gold. The training on the Curriculum was completed on 11-24-24. In addition to meeting the foundational practices in the Family engagement standards, the administrator chose three (3) additional options from three categories of engagement. All staff Continuous Quality Improvement was observed and complete. The facility Continuous Quality Improvement plan was submitted with the application. Fifty percent of all lead teachers and fifty percent of other educators meet five-star education requirements. Therefore, pending final approval, this facility is eligible to receive a five-star license. Please note that any complaints or administrative actions against your facility may impact on the license. Once in the classroom, I was assisted by the lead teachers, A. Bailey, S. Forsythe and P. Gaskins. LICENSE STATUS: Currently, this child care center operates with a 5- star rated license issued August 22, 2023. A total of forty one (41) children were present during today’s visit. Children were observed in circle time activities, routine activities and eating lunch. For lunch the children had fish, fries, cornbread, bananas and milk. During today’s visit, a partial assessment of the Child Care Requirements was conducted. I observed all required postings and attendance logs. I completed a review of all required postings and program records. REQUIRED INSPECTIONS: The last annual compliance visit was completed on 05/17/24. The last documented fire inspection was conducted on 1/9/24. The last documented fire drill was conducted on 8/27/24. Violation Number Comment Rule 115 A summary of the NC Child Care Law was not posted in a prominent place in the center. The NC Summary of Child Care Law was not posted in space 2119. G.S. 110-102 1755 Certified developmental day center staff/child ratios and group sizes were not met. There were sixteen (16), four year old children with two caregivers. .2902 (f) Child care licensing requirements are established to ensure a safe and healthy environment; therefore, it is important for you to be knowledgeable of all the licensing rules and requirements that apply to your program. Please visit the Division of Child Development and Early Education (DCDEE) website periodically for updates and What’s new in Childcare to get updates and current information about child care. Prior to today’s visit you informed me that you had a new playground. During today’s visit, I monitored the new playground for compliance with child care requirements. The new playground has a large composite play structure, but it is not in the fenced area. There is an area that is fenced and I suggested that you block the ramp that leads back to the building since that could produce a supervision issue and take your portable toys to that playground for the children to play. I also visited the old playground today. There are holes on the playground, which could potentially produce a tripping hazard, but other than the holes I didn’t see any other hazards. The allegation was as follows: There is a concern that a provider does not have the required criminal background check and qualifying letter. The allegation was shared with the teachers in all four classrooms on site and Patricia Robinson, Johnston County Public Schools Preschool Program Specialist via telephone, and all were given an opportunity to provide information. Based on the information provided and on observations made, the following was determined: A staff member in classroom 2131 did not have a qualification letter. A. Cosby was hired prior to receiving a letter. The other teacher in classroom 2131 had a current qualification letter and all of the teachers in classrooms 2119, 2125 and 2128 had a current qualification letter. I explained to Ms. Robinson that Ms. Cosby would need to leave and not return to the classroom until she has a current qualification letter. Ms. Robinson stated that she would speak to Dr. Faith Jackson, Preschool Coordinator, Johnston County Public Schools, and the principal at the school regarding the situation and get the problem resolved. Before I left the school, the principal visited the preschool classrooms and explained to Ms. Cosby that she would have to leave until she received her qualification letter. Based on the classroom attendance she was able to take one of the assistants from classroom 2119 and place her in classroom 2131 in order to maintain compliance in all classrooms. Based on the information provided, the allegation pertaining to a provider not having the required criminal background check and qualifying letter was confirmed and a violation was documented.
