I monitored the indoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation placard, evacuation plan, safe arrival and departure procedures, first aid chart, menus, daily schedules, staff/child ratios, emergency medical care plan, emergency telephone numbers and a summary of NC Child Care Law were all posted. The storage of medication and hazardous products was also monitored for compliance. The last outdoor inspection was documented as completed on 11-3-25. An approved fire inspection was completed on 5-6-25. A sanitation inspection was last completed on 5-28-25 with 14 demerits. The children were observed playing. A meal or snack was not observed. Violation Number Comment Rule 318 Children between 12 and 24 months of age were grouped with children 3 years of age or older. There were six children ages one to four grouped together. 10A NCAC 09 .0713(a)(6) 805 Fire drills were not practiced monthly and/or the drill record was incomplete. A fire drill was not documented. .0604(t); .0302(d)(5) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The written statement must be received no later than 12-1-25. The time frame for submission of a compliance letter is established to allow time for you to complete the letter and submit it to me and is not intended as time for correction of the violations. The child care facility Rainbow Child Care and Learning Center located at 107 West Waddell Street Selma is located adjacent to a church that is going to be demolished and it was deemed by the Twon of Selma that it would be unsafe for the children to remain in care at the center during demolition, so you temporarily relocated to the Noble Street location. During the visit I monitored the following: • A working telephone was available. You have cell phones available at all times. You had a copy of the Emergency Medical Care Plan posted. • Fire extinguishers were on site and all of them had current tags where they had been serviced recently. All children are in classrooms with direct exits. • Toilets were available. • Safe drinking water was available. The facility is provided with water from the Town of Selma. • Handwashing accommodations were available and each sink and bathroom was supplied with soap and paper towels. • Outdoor activities- There is an outdoor play area for the children. You stated today that you have mobile toys to take outside • I observed developmentally appropriate toys and materials in each classroom. Activity plans were current and posted and daily schedules were also posted in each classroom. All required items were posted today. Due to the nature of the visit an enrollment worksheet was not completed. Enrollment information was as follows: There was a total of ten (10) children present as follows: 1 child less than one year of age 1 child who was one year of age 4 children who were 4 years of age 1 child who was 5 years of age 3 school age children. Staff files and children’s records are located in the temporary location. All staff members are the same. I reviewed the staff and training worksheet completed during the 3-3-16 annual compliance visit and observed that all required training was current for all staff members including full day CPR/first aid coverage, current criminal background qualification letters, and current ITS-SIDS. Each staff member had up-to-date emergency information and a health questionnaire. You stated that the projected timeframe for the repairs to be completed was August 5-17, however due to Hurricane Debby demolition did not begin on time therefore you will be occupying the temporary location for an additional week, through August 26, 2024. I informed you that you must not go beyond 30 days, or you will be required to obtain statements from the building, fire and sanitation inspectors that the area being used is free from hazards and safe for temporary occupancy. A temporary license will be issued for the temporary location upon receipt of the required documents. Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. There were ten (10) children ages birth to twelve with one caregiver for a brief period of time. 10A NCAC 09 .2818 The temporary location is one large classroom, which you have divided for various ages by using tables and furniture. A staff/child ratio violation was documented today because as I entered the building today, there was one adult in the room with ten children ages birth to twelve years of age. Within a couple of minutes of my arrival, you entered from the kitchen and stated that you had just stepped out to get a lady and her son some food. A rated license assessment was also completed. Upon my arrival, I was greeted by the center director, S. McLaurin. LICENSE STATUS: Currently, this center operates with a three-star rated license issued on 7/26/17. REQUIRED INSPECTIONS: The last documented fire drill was conducted on 06/3/24, the last documented lock down drill was conducted on 05/1/24, and the last documented outdoor playground inspection was conducted on 06/3/24. The last fire inspection was conducted on 5/3/24and the last sanitation inspection was conducted on 3/4/24 and received 11 demerits with a Superior classification. I was accompanied by Ms. McLaurin as I completed a general walk-through of the indoor spaces, the outdoor play space and kitchen. During today’s visit, a total of eleven (11) children were present. Children were observed participating in teacher directed activities and playing in the classrooms. During today's visit, a full assessment of the child care requirements was conducted. I reviewed a sample of children's records and staff files. The facility is in compliance with all required trainings including pediatric CPR and First Aid and SIDS, where applicable. I observed valid CBC qualification letters and verified that staff members are in compliance with the required health and safety trainings. I observed all required postings. Any items not in compliance are listed in the violations section below. Staff Education - Based on the information currently available in the Workforce database, the staff members are eligible for the following point levels: Administrator - 5 Lead Teachers- 7 Teachers- 7 Program Coordinator-7 Group Leader-7 Therefore, you are eligible for 5 points in education. Program Standards - You continue to use your approved policies and procedures and continue to meet enhanced requirements including ratio requirements. This makes you eligible for 2 points in program. Quality Point - You continue to be eligible for one quality point by meeting the programmatic option of providing enhanced policies and an infrastructure of parent involvement. Total points - 5 (education) + 2(program) + 1 (quality) = 8 points and a three-star license.
