I completed a general walk-through of the indoor and outdoor areas, which consisted of one (1) large space divided into three (3) classrooms, the kitchen, bathrooms, and the outdoor play environment. The children were observed playing in the classrooms, participating in teacher-directed activities and eating lunch during the visit. For lunch the children had pizza rolls, corn, mixed fruit and juice. **Current License Status Currently this center operates with a three-star rated license issued January 22, 2022. ** Annual Inspections: The last sanitation inspection was completed January 10, 2025, and received a ‘Superior’ classification with 2 demerits. The last documented fire drill was conducted on May 7, 2025, the last documented shelter-in-place drill was completed on March 26, 2025. Prior to today’s visit, I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed as current/active. During today’s visit, a full assessment of Child Care Requirements was conducted. I observed all required postings. I reviewed a sample of children’s records, staff files and all applicable program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 303 Children were not adequately supervised at all times. The teacher for the infant/toddler children walked out of the classroom to answer the door at the front of the building. .1801(a)(1-5) 316 Children under one year of age were not kept separate from children two years and older. While the teacher for the infant/toddler children prepared lunch the five and six year old children were combined with the infants and toddlers. 10A NCAC 09 .0713(a)(5) 318 Children between 12 and 24 months of age were grouped with children 3 years of age or older. I monitored the indoor environment for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation placard, evacuation plan, safe arrival and departure procedures, first aid chart, menus, daily schedule, staff/child ratios, emergency medical care plan, emergency telephone numbers and a summary of NC Child Care Law were all posted at the time of the visit. No new staff have been hired since the 6-12-24 annual compliance visit. The storage of medication and hazardous products was also monitored during the visit. One infant child was present today. He was observed sleeping. A meal or snack was not observed today. The last fire drill was documented at conducted on 12-16-24 at 4:00 pm with an evacuation time of seven seconds. An emergency drill was not documented. A sanitation inspection was last completed on 9-16-24 with three demerits and a superior classification. A three-star license with five points in education, two points in program standards and one quality point was issued to the facility on 1-22-22. Based on observations made during the visit, you continue to meet the requirements previously met to obtain such a license.
