A checklist was used to note the requirements I monitored today. This facility currently operates with a Four Star license issued on March 27,2020. Upon my arrival, I was greeted by Ms. L. Gregory, program coordinator. I informed Ms. Gregory the purpose of today’s visit. Ms. Gregory gave me the necessary paperwork I needed to complete today’s visit. I observed and documented the attendance. Forty-seven (47) children were present during today’s visit. They were observed arriving, washing their hands in preparation for snack, eating snack, doing homework, playing outside on the playground and departing. All indoor and outdoor spaces used use by the children was observed today. All required items were posted. All medication and hazardous items were out of the children's reach. A sanitation inspection was conducted on February 13,2024. This facility received a superior rating with six (6) demerits. Staff and children files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. Six (6) violations cited during today’s visit. Violation Number Comment Rule 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. Monthly playground inspections were not completed for the months March-June 2023. .0605(q) 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. The EMC plan was not reviewed with all staff annually. 10A NCAC 09 .0802(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff did not successfully complete certification in First Aid before expiration date. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff did not successfully complete certification in CPR before expiration date. .1102(d) 1825 All staff did not review the center's EPR Plan during orientation and/or on an annual basis with the trained staff. Documentation of the review was not maintained on file. All staff did not review the center's EPR Plan on an annual basis with the trained staff. .0607(f) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Two staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. .1102(g) The above violations must be corrected immediately, and a letter must be emailed to me no later than March 25,2024 stating how each item was corrected. My email address is wytina.blackmon@dhhs.nc.gov If I do not receive your letter by March 25,2024 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Gregory that the staff members need in-service training hours, health safety trainings, CPR and First Aid Trainings by specific dates. Please refer to the staff/training worksheet. You need to email the training certificates once staff has completed the training hours. Consultation: Today I discussed with Ms. Gregory CPR and First aid certification must be completed prior to the expiration of the current certification. Ms. Gregory informed me today the two group leaders were scheduled for CPR and First Aid classes on March 12,2024 . New rules: I also discussed with Ms. Gregory the Rated license assessment process will begin July 1,2024 next year. I informed Ms. Gregory this facility is in Cohort 2. You must let me know if you ready to start the process for rated reassessment. Thank you for your time during today's visit. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
