The last annual compliance visit was conducted 9/17/25. Currently this program operates with a GS110 Notice of Compliance issued 8/12/2009. Center director Lori Hogan assisted me today. I conducted an assisted walk-through of the center. The last sanitation inspection was completed 3/10/26 with a ‘Superior’ classification and 13 demerits. The last fire inspection on file was conducted 6/20/25. Children were observed playing outside and routine care activities. A partial assessment of applicable childcare requirements was conducted today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). The letter for L. Hogan expired 3/1/26. The new letter was dated 3/16/26. The letter for H. Tyree expired 3/8/26. G.S. 110-90.2(b) & .2703(n)&(o) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. A drill was not recorded for January-March 2026. .0604(u);.0302(d)(8) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Hogan. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 6/2/26. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All items must be included, or the corrective action letter will be returned for corrections. The last annual compliance visit was conducted 10/11/24. A checklist was used to note the childcare requirements monitored today and a copy was left with you. Currently this program operates with a GS110 Notice of Compliance issued 8/12/2009. Center director Lori Hogan was not present today. Staff member Geneva Cox assisted me today. I conducted an assisted walk-through of the center. The last sanitation inspection was completed 7/7/25 with a ‘Superior’ classification and 14 demerits. The last fire inspection on file was conducted 8/29/24. Children were observed playing outside, eating lunch, and routine care activities. A partial assessment of applicable childcare requirements was conducted today. Program, staff and children’s files were not monitored today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. A piece of plastic edging from an old flower bed was sticking up of the ground on the playground used be school-age children. This posed a tripping hazard. Two infant style portable swings on the playground used by toddlers were not sturdy. One of the swings had tears in the material around the swing. The swings were removed during the visit. .0601(c) 721 All equipment and furnishings were not in good repair. In the old sandbox area on the playground used by school-age children, there were torn balls and broken toys/equipment. G.S. 110-91(6); .0601(b) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. There were small toy cars located in room two. The room is used by one and two-year old children. A small Hot Wheels car was on the playground used by this group of children. This was corrected during the visit. .0604(q) 1867 The depth of the loose surfacing was not based on critical height of the equipment. No mulch was present directly under the swings on the playground for preschool children. Technical assistance was provided. .0605(k)(1-4) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Cox. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 10/1/25. The new PVC pipe structure on the playground used by school-age children must be kept off limits until it has been secured. The frame alone is not stable enough for children to play around. Staff need to monitor any items used on the playgrounds to ensure they are stable and safe for the children in care. Items should not be broken, wobble back and forth, or have torn/rough edges. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children playing outside and eating lunch. No new staff were reported at this program. Storage and administration of medication were monitored. Medication authorization was monitored. Storage hazardous items were monitored today. A playground inspection was recorded on April 18, 2025. A fire drill was recorded on April 24, 2025. A shelter in place drill for the facility was recorded on March 31, 2025. The most recent fire inspection was on August 29, 2025. I observed a sanitation inspection was last conducted on March 25, 2025, earning a Superior classification. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. In Space 3 there was a tube of Benadryl Itch Stopping Cream that had expired December 2024. 10A NCAC 09 .0803(1)(d) 1867 The depth of the loose surfacing was not based on critical height of the equipment. Surfacing under the School Age playground play equipment did not meet the required depth. The structure was over eighteen (18) inches high. .0605(k)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The fall zone, covered with protective surfacing, is an essential area surrounding equipment where a child might fall, helping to absorb the force of the impact. Additional Information/Consultation • You must notify your consultant 30 days prior to changing the ownership status of your facility. •Please fluff the mulch by the swing set where the three and four year old children were playing. Upon my arrival I was greeted by the Administrator, Lori Hogan, she assisted me during today’s visit. I completed a walk-through of the classroom spaces, attendance was documented, and observations were made. The children were observed participating in free play on the outdoor learning environment. The last annual compliance visit was conducted November 3, 2023. The center is operating under a Notice of compliance that was issued August 12, 2009. The last sanitation inspection was conducted February 13, 2024 the center received a Superior rating. Fire drills and playground inspections were monitored. The last fire inspection was conducted August 29, 2024. All the required documents were posted. One new staff file was monitored along with 6 children’s files. The center does not provide transportation. Your compliance history is 90%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. The was a diaper cream that had no time to be given or when to be given. 10A NCAC 09 .0803(4)(6-9) 1320 Children's records that include an application for enrollment, medical and immunization records, and permission to seek emergency medical care was not on file for each child. Two children did not have medical reports on file. Upon my arrival I was greeted by the Administrator, Lori Hogan, she assisted me during today’s visit. I completed a walk-through of the classroom spaces, attendance was documented, and observations were made. The children were observed participating in free play on the outdoor learning environment. The last annual compliance visit was conducted November 3, 2023. The center is operating under a Notice of compliance that was issued August 12, 2009. The last sanitation inspection was conducted February 13, 2024 the center received a Superior rating. Fire drills and playground inspections were monitored. The last fire inspection was conducted August 29, 2024. All the required documents were posted. One new staff file was monitored along with 6 children’s files. The center does not provide transportation. Your compliance history is 90%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. The was a diaper cream that had no time to be given or when to be given. 10A NCAC 09 .0803(4)(6-9) 1320 Children's records that include an application for enrollment, medical and immunization records, and permission to seek emergency medical care was not on file for each child. Two children did not have medical reports on file. GS 110-91(1);.0302(d)(2); .0304(g) 1323 Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. Two children did not have immunization reports on file. 10A NCAC 09 .0302(d)(2) The above violations must be corrected immediately, and a letter must be sent to me no later than October 25, 2024 stating how each item was corrected and a plan to maintain compliance with the requirement. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation and your plan to keep each item in compliance. The facility currently operates with a Notice of Compliance that was issued on August 12, 2009 . The last annual compliance visit was conducted on November 3, 2023. . The Director was present and assisted me during the visit. Children were observed participating in free play outside on their designated playgrounds. The children were served cheese pizza, corn, apple sauce and milk for lunch. The last sanitation inspection was completed February 13, 2024 with a “Superior” classification. The last fire inspection was conducted July 23, 2023. I monitored one new staff file. During today’s visit, a partial assessment of applicable child care requirements was conducted. I observed the following violations. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On the playground for preschool age children, there was a white basket in the area where the blocks are located. The handles on the basket have broken off and this has created sharp ends. 10A NCAC 09 .0601(a) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. There was an Epi-Pen for a child did not have a parental authorization on file. 15A NCAC 18A .2820(d) The violation(s) must be corrected immediately, and a letter must be sent to me no later than May 29, 2024 stating how each item was corrected and how you plan to maintain compliance with the requirement. The last fire inspection was conducted July 28, 2023. Fire drills and playground inspections were monitored. One new staff file was reviewed. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. Three violations were observed during today’s visit. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. There was a Rubbermaid tote holding blocks in the classroom for school age children that was cracked. G.S. 110-91(6); .0601(b) 846 Over-the-counter medicines were not in their original containers or administered as authorized in writing by parent, physician or authorized health professional. There was a cream in a ziploc bag for one of the children. 10A NCAC 09 .0803(4) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. One diaper dream and one sunscreen had expired parental authorizations .0803(12) The above violation must be corrected immediately, and a letter must be sent to me no later than November 13, 2023 stating how each item was corrected and a plan to maintain compliance with the requirement.
