The administrator, Silvia Huffer, assisted me with the visit. The last temporary time period visit was conducted on November 5, 2025. The last sanitation inspection was conducted on July 21, 2025, earning a superior rating. The last fire inspection was conducted on April 1, 2025. The program’s compliance history was ninety-eight (98) percent as of 2/9/2026. I visited each indoor and outdoor space. Children enrolled were observed during teacher directed activities, free play, routines such as diapering, toileting and handwashing, meals and outdoor play. You stated that the Dial 4 screenings have been completed. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year and have been completed for this school year. The first day of school was 8/25/2025. The classroom operates from 7:30am to 2:30 pm. Parent conferences are held three times per year. You stated that you use the Class DoJo app to communicate with parents. In addition to face to face reports when possible, a daily report is sent home to the parents which includes any information about the child’s day and any reminders. Lunch was observed and consisted of hot ham and cheese sandwiches, french fries, peaches and milk. The following violations were cited during the visit: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. An aerosol can of Dust Off for Electronics and two cans of Lysol were stored in unlocked cabinets in space #108 and #109. .2820(b) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. Albuterol prescribed for M. Ramirez was not in it's stored in it's original box. .0803(2)(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. Technical Assistance was offered on creating a system for staff that allows them to periodically check required documentation and materials for medication to ensure that the dates are valid and that all necessary information is included. Silvia Huffer, administrator, assisted me with the visit. This program currently operates with a temporary license effective July 2, 2025, to January 2, 2026, because of a Change of Location. Applicable childcare requirements were discussed with Mrs. Huffer. I observed children in each indoor and outdoor learning environment. Children throughout the facility were participating in free play in the activity areas, personal care routines, outdoor play, eating lunch and transitioning to nap/rest period. The caregivers were interacting and meeting the developmental needs for each of the children. One (1) staff file was reviewed, and one violation was cited. The following were also observed during today’s visit: supervision; staff/child ratios; CPR/FA; storage of hazardous products & medication; adequate/approved space; posted license; program records; program records; and permit restrictions. Overall attendance and enrollment were collected today. The attached space, capacity and enrollment worksheet reflects the enrollment and attendance information along with the staff that were present and caring for children today. Lunch today consisted of chicken and rice, pinto beans, pears and milk. The following violation was documented during today’s visit: Violation Number Comment Rule 1757 A valid qualification letter was not on file and available to review at the facility. M. Figueroa-Salinas did not have a letter on file.. G.S. 110-90.2(b) & (d) & .2703(e) The violation/s documented above was corrected during the visit therefore a compliance letter is not required at this time. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today’s visit, may negatively affect your compliance score. As of today’s visit your compliance history is ninety-nine percent (99%). Rated License Assessment: In order to maintain subsidy payments at the end of the temporary time period, the facility must transition to a three to five star rated license. An application for a star rated license was submitted on November 5, 2025. Once I return to the office, I will process the education and program standards and contact you for further discussion. Quality Bonus Point: The quality point will also be discussed prior to processing the new license. At this time, Mrs. Huffer is continuing to work on submitting paperwork for the COGNIA accreditation. Once that paperwork and that process are complete and accreditation paperwork is received, Mrs. Huffer will contact me to schedule a visit to complete Pathway #3 Accreditation paperwork. As a reminder, you will need to add M. Figueroa-Salinas to the ABCMS portal by the close of business on Friday, November 7, 2025. Seventeen (17) staff files and fourteen (14) children’s files were reviewed and found to be in compliance. Program records, including fire drills, monthly outdoor inspections were completed and current. Transportation was also monitored. Lunch today consisted of chicken and rice, pinto beans, pears and milk. The following violations were documented during today’s visit: Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. D. OlveraDominguez who began employment on 8/4/2025 did not have a criminal background check. G.S. 110-90.2(b) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The facility did not have any staff listed in the ABCMS portal. G.S. 110-90.2 & .2703(r) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before September 24, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Remember to remove any former employees once they are terminated. Add new employees as soon as they begin employment. Both of these tasks should be done within five (5) business days. Make sure that any aliases or other names are listed on the application as well. This makes it easier to find employees who may have gotten married and have not had a chance to change their last name within the system. At this time, the facility does not have any staff listed. Remember to complete the ABCMS training in moodle. You will not be able to use the system if this training has not been completed.
