Barbara Byrd, administrator, assisted me with the visit. The facility operated with a Five-Star License, issued August 8, 2019, earning five points in the education component, seven points in the program component and one quality point for complying with the education option. License restrictions include first and second shift, meets enhanced ratios minus one and meets enhanced space. The last annual compliance visit was conducted on July 1, 2025. The last sanitation inspection was conducted on August 28, 2025. The last fire inspection was conducted on October 9, 2025. The program compliance history was ninety-three (93) percent as of 5/14/2026 and was reviewed with Mrs. Byrd. I visited each indoor and outdoor space. Children enrolled were observed in free play in the centers, outdoor play, participating in teacher directed activities and personal care routines. Teachers were interacting with the children and attending to their needs. Lunch was observed and consisted of bologna and cheese sandwich, bananas, carrots and milk. The following violations were observed during the visit: Violation Number Comment Rule 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. D. Alston had annual evaluations and development plans that expired in 2/2025 and 3/2025. 10A NCAC 09 .0514(f) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Medical action plans are required when a child has a chronic medical condition that requires it to possibly be administered while the child is in care at the facility. Along with that medical action form, permission to administer form needs to be completed and on file so that the medication can be safely administered if needed. If the medication has both forms on file and has been administered, then a medication log needs to be completed as required. These things are put in place to not only ensure that the medication is administered properly when needed but also as possible a safeguard for staff if it is followed as it should be. The medication is also required to be in its original container. This ensures that the right child is receiving the correct medication that is prescribed to them. Technical assistance was also offered on the ECERS-3: Teachers were encouraged to incorporate open-ended questions and conversations to extend children's thinking while in the activity areas. Such as, during book reading you can ask "What do you think is going to happen next? Why?" or "What would you do if you were in this story?" These types of questions encourage children to explain their thinking and promote problem-solving skills. The program also completed SACERS-U ON 10/29/2026. The program also completed an ECERS-3 Outreach Assessment on 10/30/2025. I will request the ITERS-3, ECERS-3 and SACERS-U for licensure upon the completion of once the violations that were cited during this visit have been corrected. Barbara Byrd, administrator, assisted me with the visit. The facility operated with a Five-Star License, issued August 8, 2019, earning five points in the education component, seven points in the program component and one quality point for complying with the education option. License restrictions include first and second shift, meets enhanced ratios minus one and meets enhanced space. The program’s compliance history was ninety-eight percent (98%) as of 3/20/2025 and was reviewed with Mrs. Byrd. The last annual compliance visit was conducted on July 10. 2024. The last sanitation inspection was conducted on November 25, 2024, earning a superior rating. The last fire inspection was conducted on April 10, 2024. The NC Secretary of State website was reviewed on March 20, 2025 and Sanford Child Care, Inc. was listed as current-active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, outdoor play, transitions and personal care routines. Infants were engaged in tummy time, napping, diapering routines and eating. Sleep checks were completed, recorded, and maintained as required. The caregivers were interacting and meeting the developmental needs for each of the children. There have been no new employees since my last visit. Lunch today consisted of ham and cheese sandwiches, sliced carrots, sliced bananas and milk. Violation Number Comment Rule 871 Center staff did not comply with the safe sleep policy. I observed an eighth month old in the space used by the infants, asleep in a bouncy seat. The staff reported that the child had been asleep in the seat since 10:00am. 10A NCAC 09 .0606(a) Because the violation was corrected during the visit and compliance letter is not required at this time. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical assistance was offered on transitioning toddlers from a sippy cup to a regular cup. Although there is not a “best time” to transition to a regular cup, most children have transitioned from a sippy cup to a regular cup by age two. If the desired age to have the children enrolled use a regular cup by the age of two, there are a few things that can be done prior to the transition that will make the transition easier. First, ensure that infants and toddlers are developing their fine motor muscles and skills but using such materials as puzzles, crayons, paint brushes, and sorters just to name a few. Periodically allow the children to use “open” cups prior to making the full transition. Encourage the children by using hand over hand methods if necessary and using encouraging language. Barbara Byrd, administrator, assisted me with the visit. The facility operated with a Five-Star License, issued August 8, 2019, earning five points in the education component, seven points in the program component and one quality point for complying with the education option. License restrictions include first and second shift, meets enhanced ratios minus one and meets enhanced space. The program’s compliance history was ninety-eight percent (98%) as of 3/20/2025 and was reviewed with Mrs. Byrd. The last annual compliance visit was conducted on July 10. 2024. The last sanitation inspection was conducted on November 25, 2024, earning a superior rating. The last fire inspection was conducted on April 10, 2024. The NC Secretary of State website was reviewed on March 20, 2025 and Sanford Child Care, Inc. was listed as current-active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, outdoor play, transitions and personal care routines. Infants were engaged in tummy time, napping, diapering routines and eating. Sleep checks were completed, recorded, and maintained as required. The caregivers were interacting and meeting the developmental needs for each of the children. There have been no new employees since my last visit. Lunch today consisted of ham and cheese sandwiches, sliced carrots, sliced bananas and milk. Violation Number Comment Rule 871 Center staff did not comply with the safe sleep policy. I observed an eighth month old in the space used by the infants, asleep in a bouncy seat. The staff reported that the child had been asleep in the seat since 10:00am. 10A NCAC 09 .0606(a) Because the violation was corrected during the visit and compliance letter is not required at this time. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical assistance was offered on transitioning toddlers from a sippy cup to a regular cup. Although there is not a “best time” to transition to a regular cup, most children have transitioned from a sippy cup to a regular cup by age two. If the desired age to have the children enrolled use a regular cup by the age of two, there are a few things that can be done prior to the transition that will make the transition easier. First, ensure that infants and toddlers are developing their fine motor muscles and skills but using such materials as puzzles, crayons, paint brushes, and sorters just to name a few.
