The program operated with a three-star license, issued March 6, 2018, earning four points for education, two points for program standards and one point for complying with the programmatic option. License restrictions include first and second shift and meets enhanced space. Dorothy McIver, administrator/owner, assisted me with today’s visit. The last annual compliance visit was conducted on July 9, 2025. The last sanitation inspection was conducted on December 19, 2025, earning a superior rating. The last fire inspection was conducted on May 21, 2026 The program’s compliance history was ninety-seven(97) percent as of 6/22/2026. I visited each indoor and outdoor space. Children enrolled were observed in outdoor play, eating morning snack, free play, personal care routines and transitioning from lunch. No infants are currently enrolled. Lunch today chicken nuggets, corn, wheat bread, applesauce and milk. The following violations were observed during today’s visit: Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). T. Jones had a qualification letter that expired on 5/26/2026 on file that had not been renewed as of today's visit. G.S. 110-90.2(b) & .2703(n)&(o) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before July 9, 2026 (an extra day was given due to the upcoming Holiday), I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. The administrator/owner, Dorothy McIver, assisted me with the visit. The last annual compliance visit was conducted on July 9, 2025. The last sanitation inspection was conducted on December 19, 2025, earning a superior rating. The last fire inspection was conducted on May 29, 2025. The program compliance history was ninety-eight (98) percent as of 1/14/2026 and was reviewed with the administrator. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today and all indoor and outdoor areas were monitored. I observed children in the indoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in teacher directed activities, free play in activity areas, personal care routines such as handwashing and toileting, transitions and outdoor play. The caregivers were interacting and meeting the developmental needs for each of the children. One new employee has been employed since my last visit. That file was reviewed and one violation was documented. The facility does not currently have any infants enrolled. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. Morning snack consisted of wheat thins and milk. The following violation was cited during today’s visit: Violation Number Comment Rule 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. L. Quick began employment on 9/4/2025 and hasn't completed the training. .1102(g) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Technical assistance was also offered on offering and using the “chubby” paint brushes and pencils for the younger children. You can help ensure that infants and toddlers are developing their fine motor muscles and skills but using such materials as puzzles, crayons, paint brushes, and sorters just to name a few. Also, periodically allow the children to use “open” cups prior to making the full transition. Encourage the children by using hand over hand methods if necessary and using encouraging language. Also remain calm during spills or accidents letting the child know that it is okay and that they can keep trying. Also encourage families to use the same methods at home. During this visit we discussed the QRIS Modernization Pathways to the Stars. Mrs. McIver has decided to follow the option #2 Program Assessment for Child Care Centers and Centers Located in a Residence (CLIAR). This pathway includes the following: • Enhanced Ratio(required) and/or Enhanced Space (optional) • 50% of all Lead Teachers and 50% other educators meet 5 Star Education Standards • Family and Community Engagement Foundational Practices AND 2-4 additional choices (depending on the star rating level) • CQI Plan for Facility and Individuals • Approved curriculum and assessments (if applicable to the star rating level), and coaching/training options for administrators and lead teachers (if applicable to the star rating level). • Mrs. McIver and her staff will also begin working on the self-assessment study and develop goals to work towards. Mrs. McIver will also work on the CQI plan for both the center and staff and documenting family and community engagement standard practices that the facility already practice. The program compliance history was one hundred percent (100) as of 7/28/2024. I visited each indoor space. I was not able to monitor the outdoor space due to active precipitation. Children enrolled were observed in free play in the centers, outdoor play, participating in teacher directed activities and routines such as diapering, toileting and handwashing and eating lunch. Infants enrolled were observed during nap time, tummy time, diapering and meal time. Lunch was observed and consisted of ground beef with meatballs, green beans, mashed potatoes, rolls and milk. The following violation was observed during the visit: Violation Number Comment Rule 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The current EPR plan had emergency information for staff members and children who were no longer employed or enrolled at the center. .0607(e) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately on or before August 12, 2024. I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. If you email the compliance letter, it must be sent from the email address registered with the DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance was offered on creating a system for the administrator and staff that allows them to periodically check required documentation to ensure that the dates are valid and that all necessary information is included as well as ensuring that compliance is maintained throughout the facility. Utilize the staff file checklist and staff training worksheets that are available on the DCDEE website to assist with keeping track of the required documentation and the timelines. Update the emergency preparedness manual to include new staff documentation as well as remove documentation that is no longer needed. As a reminder, CPR & First Aid training is due for four (4) staff members by October and December 2024. The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. The Child Care Commission is currently in the process of drafting proposed child care rule language to begin the rulemaking process for implementing the new requirements. The rulemaking process will include publishing of proposed revised rules as well as a public hearing and public comment period. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs.
