Jina Reid, owner/operator, was present during today’s visit. The facility operated with a Four-Star license, issued July 7, 2017, earning 7 points in education standards, 2 points in program standards and 1 quality point for complying with the programmatic option. License restrictions include First, Second and Third Shifts, maximum of 5 preschool children at anytime and infant capacity restricted to no more than 2 children under 1 year of age. The last annual compliance visit was conducted on April 15, 2025. The program’s compliance history was ninety-nine (99) percent as of 3/30/2026 and was reviewed with the operator during the visit. The Caseworker assist portal was reviewed and there were no findings. A walk-through of the family childcare home was completed today, and all indoor and outdoor areas were monitored. I observed one child in the indoor learning environment and found supervision and staff/child ratios to be in compliance. The child was observed in free play and personal care routines such as diapering and handwashing. Lunch today consisted of turkey sandwiches, celery, sliced peaches and milk. The following violations were cited during today's visit: Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. A playground safety check for was not completed for February 2026. 10A NCAC .1721(e)(5)(A-F) 1853 The operator did not conduct a monthly fire drill. A monthly fire drill has not been conducted in 2026. .1719(a)(15) & .1721( e)(2) 2058 The family child care home operator did not collect and submit samples of water from each outlet used for drinking or food preparation for lead analysis to the local health department or a laboratory certified to analyze for lead in drinking water by the North Carolina State Laboratory of Public Health. As of today's visit, the provider has not sent off the water sample for lead analysis. 10A NCAC 09 .1725(a)(2) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before April 15, 2026, (an extra day was given due to the holiday) I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. If sufficient information is not received by the due date, a follow-up visit will be conducted. You can also prevent serious injuries or mishaps by conducting monthly inspections. By conducting monthly fire drills, you help the children as well as yourself prepare for emergencies. You both already know the routine and where to go and what to do and it would make the transition if a real fire or other emergency happened go smoother. As a reminder, you are required to have your water tested every three (3) years for lead analysis. At the time of the visit, you had the container from RTI but you had not submitted the sample for an analysis. Please follow the instruction that were included with the sample container and submit it as soon as possible. No additional enrollment restrictions in addition to licensed capacity. • Operator meets 4 Star Education Standards • Family and Community Engagement Foundational Practices AND 2-4 additional choices (depending on the star rating level) • CQI Plan for Operator and Additional Caregivers (if applicable) • Approved curriculum and assessments (if applicable to the star rating level), and coaching/training options for operators (if applicable to the star rating level). • Ms. Reid staff will also begin working on the self-assessment study. Ms. Reid will continue to work on the CQI plan and begin documenting family and community engagement standard practices that the facility already practice. The following violations were documented during today's visit: Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). J. Reid had a qualification that expired on 1/4/2026. C. Reed had a letter on file that expired 12/22/2025. G.S. 110-90.2 & .2703(n) & (o) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. A walk-through of the family childcare home was completed today, and all indoor and outdoor areas were monitored. I observed one child in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. The child was observed completing homework tasks, eating snack and personal care routines. Afternoon snack was observed and consisted of graham crackers and milk. The following violations were observed during today’s visit: Violation Number Comment Rule 508 Operator did not successfully complete a first aid course as referenced in Rule.1702(b)(2) First aid training was not renewed on or before the expiration of the certification. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical Assistance was offered on creating a system for staff that allows them to periodically check required documentation to ensure that the dates are valid and that all necessary information is included. We discussed requesting Environment Rating Scale (ERS) assessments in an effort to increase the program's star rating. Resources such as Child Care Resource and Referral and Smart Start can be valuable when preparing for the ERS. “All about FCCRS” and the NCRLAP website (www.ncrlap.org) can also be helpful as staff prepare for the ERS. On the NCRLAP website, there are training videos, sample questions and clarifying information regarding the assessments you may find helpful. Your facility is in Cohort 1 of the rated license process. Your prep year began on 7/1/2023 and will continue until 6/30/2024. Please consider contacting the Partners for Families and Children of Lee County for help with technical assistance, room arrangements, mock observations, self-observations and in any other areas that will help prepare you for the assessments as well as increase/maintain the quality level of care that your staff provide.
