This program operates with a three (3) star license, issued September 22, 2023, earning four (4) points for education, two (2) points for program and one (1) quality point for complying with the programmatic option. License restrictions include daytime and overnight; ages 0 through 12, capacity of 38 children, meets enhanced ratios and children under 2 ½ years old in classrooms with direct exits only. Catondra Lebdetter, owner/administrator, assisted me with the visit. The last annual compliance visit was conducted on 2/12/2025. The last sanitation inspection was conducted on July 8, 2025, earning a superior rating. The last fire inspection was conducted on June 10, 2025. Due to issues with our Regulatory system, I was not able to review the facility’s compliance history during the visit. I will review it with the administrator at a later date via email. I visited each indoor and outdoor space. Children enrolled were observed in free play in the centers, outdoor play, participating in teacher directed activities and personal care routines. Infants enrolled were observed during tummy time, personal care routines, free play and teacher directed activities. Teachers were interacting with the children and attending to their needs. Lunch consisted of whole wheat tortillas, Chicken sausage, saltine crackers, tossed salad, sliced grapes and milk. The following violations were observed during the visit: Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. Albuterol for J. Dimastabon was not in it's original labeled container. .0803(2)(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before February 5, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. In order to ensure that the proper dosage, times given, validation dates and any other important information is followed prior to administering any type of medication, ensure that a completed medical action plan (if applicable), permission to administer form and medication administration log has been completed by the parent and staff and stored with the medication (on in the child’s file). Also make sure that any medication received is in the original container and box with the prescribed information if applicable. During this visit we discussed the QRIS Modernization Pathways to the Stars. Mrs. Ledbetter has decided to follow the option #2 Program Assessment for Child Care Centers and Centers Located in a Residence (CLIAR). This pathway includes the following: • Enhanced Ratio(required) and/or Enhanced Space (optional) • 50% of all Lead Teachers and 50% other educators meet 5 Star Education Standards • Family and Community Engagement Foundational Practices AND 3 additional choices • CQI Plan for Facility and Individuals • Approved curriculum for all ages served • Child Observations and assessments • Coaching/training options for administrators and lead teachers. • NCRLAP Self Study I also left copies of the Continuous Quality Improvement and Professional Development Plan, work experience form, Pathway #2 -Classroom and Instructional Quality for Child Care Centers and Centers Located in a Residence (keep those copies and continue working on completing them prior to your next visit.) You stated that you will review the approved curricula and assessment tools at a later date and decide if you are going to continue using your current curriculum or go with another option. Mrs. Ledbetter stated that they have already begun reviewing on the Facility CQI plan for both the center and the staff. She will also provide her staff with the link for the QRIS modernization page as well as NCRLAP so that they will familiarize themselves with the new process. The facility operated with a Three-Star License, issued September 22, 2023, earning four points in the education component, two points in the program component and one quality point for complying with the programmatic option. License restrictions include daytime and overnight, meets enhanced ratios, and children under 2 ½ years old in classrooms with direct exits only. Catondra Ledbetter, administrator/owner, assisted me with the visit. The last annual compliance visit was conducted March 15, 2024. The last sanitation inspection was conducted on December 18, 2024 with a superior rating. The last fire inspection was conducted on June 19, 2024. The program’s compliance history was ninety-five (95) percent as of 2/11/2025. I visited each indoor and due to active precipitation, I was not able to monitor the outdoor spaces. Children enrolled were observed during teacher directed activities, free play, routines such as diapering, toileting and handwashing and nap/rest time. Lunch was observed and consisted of black beans, cheese, tortilla chips, grapes, salad and milk. The following violations were cited during the visit: Violation Number Comment Rule 1898 Staff did not complete the health and safety training within one year of employment. W. Beck did not complete the training within one year of employment. .1102(a) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Two staff members did not complete the Recognizing and Responding to Suspicions of Child Abuse and Neglect within five years of the previous training date. .1103(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. I reviewed the allegations with the administrator/owner, Catondra Ledbetter and she was given the opportunity to respond. The allegations are as follows: Some staff do not have criminal background checks. Some staff working with infants do not have SIDS training as required. Children receive sweet cake for snack on a regular basis. A menu is not posted for parents as required. Meals lack variety. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, the license and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. One new staff has been employed since my last visit and I reviewed that file. One violation was found in that file. I observed the indoor and outdoor spaces used by the children, A total of twenty-two (22) children were present. I observed the educators and children in the classroom used by the preschoolers during lunch, handwashing and toileting routines as well as nap. Infants and toddlers enrolled were observed during transitioning from lunch to nap, tummy time and free play. Lunch consisted of fish sticks, green beans, peaches, sliced wheat bread and milk. Based on information provided, November 19, 2024, I interviewed the administrator and two staff members. During my interview with the administrator, Catondra Ledbetter, she stated that usually works everyday beginning at 5:30 am and until about 8:00 pm when the second shift children leave for the evening. She stated that if she has to leave that she will usually leave around nap time to complete errands so that she can be back to the facility before nap is over. She also stated that she has recently enrolled in the Child and Adult Care Food Program (CACFP) and has been following those guidelines as required. She stated that there may be times when a child has a birthday and store-bought snacks are brought in by the family. She stated that when this happens, she still offers the center’s regular snack to all of the children just to ensure that they are offered a healthy option that meets the required guidelines. She also stated that A. Woods had been a volunteer from March 2024 until November 21, 2024. She stated that once the criminal background check for A. Woods came in on November 22, 2024, then she began in the space used by the infants as a teacher assistant. Mrs. Ledbetter also stated that the kitchen staff will work in the space used by the infants and other spaces during break time and that Mrs. Ledbetter herself will come back and break the staff if needed. She also stated that staff hardly ever leave out of the class to take a break but that they can and they are not required to work through break but that they just prefer to stay in the class while the children are resting. During my interview with staff member A, she stated that the administrator sometimes leaves the facility around at the beginning of nap to run errands but that she will return before the end of nap. During my interview with staff member B, she stated that the follow the menu daily. Upon my arrival to the center, I observed one of the teacher’s in the space used by the infants at their car in the parking lot before returning to the building. Once I entered the building for the beginning of the visit, I observed that teacher and the other teacher in the space used by the infants with seven (7) children present ranging in ages from zero (0) to one (1). I also observed one teacher in the space used by the preschoolers with 10 children ranging in ages from two (2) to four (4) years old. I also observed one teacher in the space used by the toddlers with five (5) two (2) year olds. Once I left the space used by the toddlers and reentered the space used by the infants, one of those teachers had stepped into another part of the building again leaving the other teacher who did had not had ITS-SIDS training alone with seven children ranging in ages from zero (0) to one (1). I was able to review the center’s menus and saw that they were posted in the visibly in the front office. I also reviewed the eight remaining staff files for current criminal background checks and ITS’S SIDS for those who work with the infants. Based on the above information, the allegations that some staff do not have criminal background checks, children receive sweet cake for snack on a regular basis, a menu is not posted for parents as required and meals lack variety were unsubstantiated. The allegation that some staff working with infants do not have ITS-SIDS training as required was substantiated. The following violations were observed during today’s visit: Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. There was ten children ranging in ages from two to four with one teacher in the space used by the preschoolers. In the space used by the infants, that class had seven children ranging in ages from zero to one with one teacher. 10A NCAC 09 .2818 1831 At least one child care provider, who has completed ITS-SIDS training was not present in the infant room, while children were in care. Upon the my arrival to the facility, I observed one teacher in the space used by infants in the parking lot at her car leaving a teacher who had not had ITS-SIDS training in the class with seven children, three of whom were under age one, alone with those children. Once I entered the building, I observed that teacher once again step out of the space used by the infants leaving the other teacher in that space alone again with the same seven children. .01102 (f) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. One employee who began employment on 11/22/2024 had not signed the statement prior to working with the children. .0608(d)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Technical assistance was offered on the importance of maintaining staff child/ratio. During children’s early years they require a lot of guidance, supervision, instruction, and individual attention. By maintaining ratios, teachers can adequately monitor the health and safety of the class. This can also help them to form better relationships with each child. It can also help to foster more meaningful connections and help the teacher understand a child’s development on a deeper level. Maintaining staff/child ratios is also a requirement that was established to meet the highest safety, welfare and developmental standards for the children present. By following these and other requirements, a facility can maintain their credibility. Technical assistance was also offered on ensuring that all staff are aware of the childcare rules. When staff are aware of the rules and the reasoning behind the rules, it helps them to understand why the rules are in place. By periodically reviewing staff/child ratio and other rules with the staff, you can help ensure that they are following those rules even when administration is not present. One staff member who has completed the ITS-SIDS training must be in the space for the infants at all times if there are children who are under age one enrolled/present. Technical assistance was offered on the importance of maintaining staff child/ratio. During children’s early years they require a lot of guidance, supervision, instruction, and individual attention. By maintaining ratios, teachers can adequately monitor the health and safety of the class. This can also help them to form better relationships with each child. It can also help to foster more meaningful connections and help the teacher understand a child’s development on a deeper level. Maintaining staff/child ratios is also a requirement that was established to meet the highest safety, welfare and developmental standards for the children present. By following these and other requirements, a facility can maintain their credibility. Technical assistance was also offered on ensuring that all staff are aware of the childcare rules. When staff are aware of the rules and the reasoning behind the rules, it helps them to understand why the rules are in place. By periodically reviewing staff/child ratio and other rules with the staff, you can help ensure that they are following those rules even when administration is not present. One staff member who has completed the ITS-SIDS training must be in the space for the infants at all times if there are children who are under age one enrolled/present. This program currently operates with a temporary license effective March 21, 2023, to September 21, 2023. Applicable childcare requirements were discussed with Ms. Catondra Ledbetter, Owner/Administrator. The last approved fire inspection was conducted on June 21, 2023. The last sanitation Inspection was conducted on August 2, 2023, the classification was superior. All indoor and outdoor spaces currently being used for child care were monitored today. The facility has a total of four (4) classrooms being used for child care. The children observed today were interacting with assigned staff, playing in free choice areas such as fine motor materials, books, blocks, and dramatic play materials. The infants were positioned on the floor for tummy time along with assigned staff. A group of children were observed playing outside. Teachers were observed interacting with children during routine activities and during free-play inside and outside. Teachers adequately supervised children. The following were monitored during today’s visit: supervision; staff/child ratio; CPR/FA; SIDS; hazardous product storage; adequate/approved space; two (2) new staff records; program records (fire drill, outdoor play inspection, EMC, and menu); posted license and; permit restrictions. All center program records were also monitored. It is the center’s responsibility to review all files for completeness. During today’s visit the children were served, sliced oranges, toast, and milk for morning snack. Five (5) indoor rooms were measured on May 24, 2011. However, according to the Building Inspection conducted on August 1, 2023, only four (4) rooms were approved for child care. During today’s visit I checked the measurements of those rooms and the playground. There were no changes to the structure of the building so, the indoor and outdoor measurements will remain the same. However, because only four (4) rooms were approved this will change the total capacity from 45 to 38. The indoor floor plan was revised showing four (4) rooms approved. Indoor measurements: Space #1 11 children @25sq. ft. 9 children @ 30sq. ft. 8 children @ 35sq. ft. Space #2 7 children @ 25sq. ft. 5 children @ 30 sq. ft. 5 children @35sq. ft. Space #3 8 children @25sq. ft. 7 children @30sq. ft. 6 children @35sq. ft. Space #4 12 children @25sq. ft. 10 children @30sq. ft. 9 children @35sq. ft. Outdoor measurements: Space #1 54 children @ 75sq. ft. 41 children @ 100sq. ft. Space #2 76 children @75sq. ft. 57 children @100sq. ft. Overall attendance and enrollment were collected today. The attached space, capacity and enrollment worksheet reflects the enrollment and attendance information along with the staff that were present and caring for children today. According to Ms. Ledbetter her contact information has not changed. At the completion of the visit, the Visit Summary was reviewed with Ms. Ledbetter. Violation Number Comment Rule 603 All floors and floor coverings were not constructed of nonabsorbant, easily cleanable material. There were areas of the flooring that were not in good repair throughout the center because areas of the flooring is cracked. 15A NCAC 18A .2824(a)&(b) 620 All walls and ceilings including doors and windows were not kept clean and in good repair. There were areas of the wall throughout the center that were not in good condition because areas of sheet rock were exposed. 15A NCAC 18A .2825(a) The violation/s documented above must be corrected immediately. A signed and dated letter of compliance must be mailed to me by August 21, 2023, stating that the violation/s have been corrected. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today’s visit, may negatively affect your compliance score. As of today’s, visit your compliance history is 94%. In order to ensure that your rated license is issued before your temporary license expires on September 21, 2023, I must receive this information no later than August 21, 2023. If you feel this information cannot be completed by this date, then you should contact me immediately. Your temporary license cannot be extended. If it should expire before your rated license is issued, then you are no longer eligible for subsidy and the appropriate action could be taken against your license. Rated License Assessment: I cannot process your star rated license until I have received your compliance letter. On June 12th, Governor Cooper signed Senate Bill 291, extending the Child Care Hold Harmless legislation until June 30, 2024. Also, when a child care center undergoes a change of ownership, the agency will use previous points achieved from the facility’s last star rated license assessment to assign a star rating at the end of the temporary license. The previous facility’s points in each area earned: Staff Education: 4 out of 7 points Program Standards: 2 out of 7 points Quality Point: 1 out of 1 point The total points earned: 7 out of 15 points Therefore, your facility will earn a 3star rated license effective September 22, 2023. This change in procedure supports the transitional process of resuming the star rated license through a cohort model. The new owner/facility will be expected to go through a star rated license reassessment during the cohort year established under the previous facility license. Your facility will fall under the second cohort. The following information was shared during a webinar and the DCDEE newsletter. The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024, and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each include a preparation year and a reassessment year. While the first reassessment year won’t begin until July 1, 2024, the preparation year for cohort one begins July 1, 2023. Your consultant will reach out to you prior to the beginning of the preparation year of your assigned cohort and will provide guidance to assist you as you begin the process. Technical Assistance: Required In-service training hours for staff were discussed with you along with how to calculate in-service training hours. All in-service training hours should be logged on the On-Going Training Documentation Form. Copies of certificates should be kept in each staff’s file and available for review by a representative of DCDEE. Emergency Preparedness and Emergency Medical Care Plan should be reviewed with staff annually. Documentation of this should be on file for all staff. Their fire and sanitation inspections were current; however, it was discussed with Ms. Ledbetter that when she receives her fire inspection, she should send it to her consultant within one (1) week of the inspection. She will also need to send her sanitation inspection to her consultant. I also discussed with her that it is her responsibility to ensure that inspections are conducted annually.
