The facility operates with a Five-Star License, issued August 16, 2024, earning seven (7) points in the education component, six (6) points in the program component and one quality point for complying with the education option. License restrictions include daytime care only, children 2 ½ years and under in rooms with direct exits only, meets enhanced ratios and meets enhanced space. The administrator, April Donithan, assisted me with the visit. The last annual compliance visit was conducted on August 4, 2025. The last sanitation inspection was conducted on December 16, 2025, earning a superior rating. The last fire inspection was conducted on November 14. 2025. The program compliance history was ninety-nine (99) percent as of 1/27/2026 and was reviewed with the administrator. The NC Secretary of State website was reviewed on January 28, 2026, and KSH, INC. was listed as current-active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today and all indoor and outdoor areas were monitored. I observed children in the indoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in teacher directed activities, free play in activity areas, personal care routines such as handwashing and toileting, transitions and outdoor play. Infants were observed during tummy time, free play, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. Two new employees have been employed since my last visit. Those files were reviewed and no violations were documented. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. Lunch consisted of peanut butter sandwiches, mixed veggies, peaches and milk. The following violation was cited during today’s visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The teacher for the space used by the younger toddlers had an activity plan posted dated for 1/18-1/24/2026. GS 110-91(12); .0508(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical Assistance was offered on sharing more information with the families during arrivals and departures. Information shared between the families and the staff during these times can be beneficial during a later time in the child’s day. For example, if a child did not sleep well during nap and gets cranky early in the evening while in the family’s care, that information may have helped to explain the child’s behavior during that time. Gathering information in the mornings during arrivals can also explain behaviors that may be exhibited through a child’s day. It also helps both the children and families feel welcome in the class and center. During this visit we discussed the QRIS Modernization Pathways to the Stars. Mrs. Donithan is still undecided at this time about which pathway the facility will follow. She was given information on option #1 Pathway Assessment for Child Care Centers as well as option #2 Classroom and Instructional Quality for Child Care Centers and Centers Located in a Residence (CLIAR). This pathway #1 includes the following: • Enhanced Ratio(minus one required) and/or Enhanced Space (optional) • 50% of all Lead Teachers and 50% other educators meet 5 Star Education Standards • Family and Community Engagement Foundational Practices AND 2-4 additional choices (depending on the star rating level) • CQI Plan for Facility and Individuals • Approved curriculum and assessments (if applicable to the star rating level), and coaching/training options for administrators and lead teachers (if applicable to the star rating level). • Mrs. Donithan and her staff will also begin working on the self-assessment study and develop goals to work towards. Pathway option #2 includes • Enhanced Ratio(required) and/or Enhanced Space (optional) • 50% of all Lead Teachers and 50% other educators meet 5 Star Education Standards • Family and Community Engagement Foundational Practices AND 2-4 additional choices (depending on the star rating level) • CQI Plan for Facility and Individuals • Approved curriculum and assessments (if applicable to the star rating level), and coaching/training options for administrators and lead teachers (if applicable to the star rating level). • Program Assessment conducted with an average score of 4.0 for ITERS-3 and ECERS-3 and average score of 5.0 for SACERS-U Mrs. Donithan will also work on the CQI plan for both the center and staff and documenting family and community engagement standard practices that the facility already practice. April Donithan, Administrator, assisted me with today’s visit. The facility operated with a Five-Star License, issued June 29, 2017, earning five (5) points in the education component, second (7) points in the program component and one quality point for complying with the education option. License restrictions include daytime care only, meets enhanced ratios minus one and meets enhanced space. The program compliance history was one hundred (100) percent as of 8/14/2024. The NC Secretary of State website was reviewed on August 15, 2024 and KSH, Inc. was listed as current- active. Your program was also monitored for compliance with implementing an approved curriculum Teaching Strategies as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on September 6, 2023. The sanitation inspection was completed on June 21, 2024 with a “Superior” classification. The last fire inspection was conducted on November 8, 2023. I observed the children eating lunch and participating in free play, handwashing routines, small group and outdoor play. Infants enrolled were observed in free play, tummy time and teacher directed activities. Lunch consisted of sliced hotdogs, baked beans, pears and milk. The children enrolled in the NC Pre-K classrooms had not started the school year yet. Therefore, I was unable to monitor the NC Pre-K requirements in section .3000 of the child care rules. The program is currently completing Dial 4 assessments prior to the beginning of the school year on 8/30/2024. The last environment rating scale scores were received on 4/15/2024 and the following were the scores: ITERS-R (5.3), ECERS-R(6.32), ECERS-R (5.43)and SACERS-R (6.41). The overall average assessment scores for four classrooms was 5.87. The following violation was documented during today’s visit: Violation Number Comment Rule 531 Bottles were propped. An eight month old was seated in a bouncy seat with a bottle propped using a pillow. 10A NCAC 09 .0902(b) Because the violation was corrected during the visit, a compliance letter is not required at this time.
