This is a religious sponsored that operated under a G.S. 110-106. The Notice of Compliance was issued December 6, 2018. The current restrictions are daytime care only and disposable diapers only. Age range 0-6 years; capacity is 397. Jeannie Garrell, administrator, assisted me with the visit. The facility's compliance history was one hundred percent (100%) as of 9/15/2025 and was reviewed with the administrator. The NC Secretary of State website was reviewed during the visit and Grace Chapel Christian Church was listed as current-active. The license was posted, and the restrictions were in compliance. The last annual compliance visit was conducted on October 22, 2024. The last sanitation inspection was conducted on December 18, 2024, earning a superior rating. The last fire inspection was conducted on April 10, 2025, and this date was changed in Regulatory to reflect the latest inspection date. I visited each indoor and outdoor space. Children enrolled were observed in free play in the centers, outdoor play, participating in teacher directed activities and routines such as toileting and handwashing and eating lunch. Lunch was observed and consisted of chicken bites, rice, corn, mandarin oranges, rolls and milk. The following violations were observed during today’s visit: Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Medication authorization for F. Hines expired on 8/7/2025 and the medication was not returned to the parent. .0803(12) 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. Three incident reports completed in 2025 did not include either the steps to prevent reoccurrence, witness to the incident or was not signed by the individual completing the report. .0802 (e) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before September 30, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. One way for staff to take responsibility and keep up with when they should notify parents and administrative staff that certain forms will need to be renewed and/or filed is to create calendar reminders or create a paper list and post it in a place that they can view easily. Make sure that no identifying information is on the posted list. If you chose to post a reminder note, use phrases such as August 1st-review all permission to administer forms. The last fire inspection was conducted on October 13, 2023. I visited each indoor and outdoor space. Children enrolled were observed in free play in the centers, outdoor play, participating in teacher directed activities and routines such as toileting and handwashing and eating lunch. Lunch was observed and consisted of tacos, corn, mandarin oranges and milk. The following violations were observed during the visit: Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Two staff members did not have First Aid Training Certification on file. .1102(c) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent.
