The assistant director, Lannesse Carroll and Jeannie Garrell, administrator, assisted me with the visit. This is a religious sponsored that operated under a G.S. 110-106. The Notice of Compliance was issued October 31, 2023. The current restrictions are daytime care only. The last sanitation inspection was conducted on May 25, 2025, and received a superior rating. The last fire inspection was conducted on July 3, 2025. The program compliance history was one hundred (100) percent as of 3/29/2026. The NC Secretary of State website was reviewed on March 30, 2026, and Grace Chapel Christian Church was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in free play in activity areas, transitions, personal care routines and outdoor play. Infants were engaged in tummy time, napping, and diapering routines. Sleep checks were completed, recorded, and maintained as required. The caregivers were interacting and meeting the developmental needs for each of the children. I reviewed staff files for one (1) new employee who has been hired since my last visit. That file was reviewed, and no violations were found. Breakfast consisted of french toast, oranges and milk. The following violation was observed today: Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. The window seal in the space used by the toddlers (toddler A) had peeling paint. 15A NCAC 18A .2825(a) The following violation was observed today: The violation documented was corrected during the visit therefore a letter is not required at this time. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical assistance was offered on including doorways and window frames on the weekly/monthly classroom checklists. By routinely inspecting entry points that are commonly used by everyone entering the space and other furniture used daily inside the space, you can provide the proper maintenance ahead of time that could possibly prevent injuries or accidents. I observed children in the indoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. The infants were observed in tummy time, routines and napping. The caregivers were interacting and meeting the developmental needs for each of the children. I reviewed three new staff files and no violations were found. Lunch today consisted of cheeseburgers, sweet potato fries, sliced pears and milk. The following violations were observed today Violation Number Comment Rule 871 Center staff did not comply with the safe sleep policy. An infant whose date of birth was 6/7/2023 in space MD 4 used for infants was in a crib asleep at 10:20 am and the last documented check was at 9:40 am. 10A NCAC 09 .0606(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical assistance was offered on performing sleep checks as required and documenting those checks. By following the safe sleep policy and what was learned in the ITS-SIDS training we can take necessary measures to possible prevent a sudden infant death while the children are in care. Technical assistance was also offered on possibly using both sinks in the toddler classrooms when transitioning from outdoors to indoors after outdoor playtime. Although there are two sinks in the class, one used for handwashing and one used for food prep, using them both during these transition times you can prevent behavior or other classroom issues that arise when the children have to have for long periods of time. A suggestion would be to properly clean and sanitize the sinks before leaving the space to go outside and then upon reentry into the classrooms use them both for two groups of children. Be sure to properly clean and sanitize each sink afterwards. Technical assistance was also offered on transitioning in smaller groups. When transitioning from indoors to outdoors in large groups, the actual wait times can be lengthy. The time the first child has had to wait in line for all for the other children to complete tasks such as putting on coats, cleaning up, and administering sunscreen just to name a few, can total over three minutes. Consider transitioning outside using two or more smaller groups. The first group can go prepare to go out with one staff member, while the other group completes other tasks or activities and then the second group can begin the transition. If this method is not an option or a desired method, then consider asking for additional staff to come and assist during those transition times which may include, potty times, transition to and from the playgrounds and classroom activities such as art projects. By using smaller groups to transition, you can also prevent possible behavior issues that may be caused by lengthy wait periods.
