Chasatie Toomer, owner/administrator, assisted me with the visit. The facility currently operates with a Four-Star License, issued April 28, 2023, earning three points in the education component, six points in the program component and one quality point for complying with the programmatic option by using Creative Curriculum. License restrictions include first and second shift, meets enhanced ratios and meets enhanced space. The last annual compliance visit was conducted on July 23, 2025. The last sanitation inspection was conducted on February 5, 2026, earning a superior rating. That last fire inspection was conducted on March 23, 2026. The facility’s compliance history was ninety-eight (98) percent as of July 6, 2026, and was reviewed with Ms. Toomer. The North Carolina Secretary of State’s website was reviewed during the visit and Glorious Community Childcare Center, llc., was listed as current-active. I visited each indoor and outdoor space. Children enrolled were observed in free play in the centers, outdoor play, participating in teacher directed activities and personal care routines. Teachers were interacting with the children and attending to their needs. Infants present were observed during tummy time and free play with the teachers interacting and responding positively to their needs. Lunch was observed and consisted of spaghetti, corn, cucumbers with ranch dressing, and milk. The following violations were observed during the visit: Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. One caregiver was present on the playground with fifteen children that included two one year olds. GS 110-91(7);.0713(a-d) 315 When combining age groups, the staff/child ratios for the youngest child in the group was not maintained for the entire group. Two (2) one year olds were grouped with thirteen (13) other children on the playground with one caregiver present. .0713(a)(1) 318 Children between 12 and 24 months of age were grouped with children 3 years of age or older. Two (2) one year olds were present with thirteen other children that included three (2) year olds, seven (7) three year olds, two (2) four year olds and one (1) five year old. 10A NCAC 09 .0713(a)(6) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The administrator was reminded to ensure that all staff have updated WORKS letters on file including new staff. Children enrolled were observed in naptime and transitions from nap to snack and free play and toileting and handwashing and eating lunch. Infants enrolled were observed during tummy time, personal care routines, diapering and free play. Lunch consisted of turkey and cheese sandwiches, applesauce and milk. The following violations were observed during the visit: Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. T. Toomer who was hired on 7/11/2025, has not been entered into ABCMS. During the visit I was able to observe the children in the spaces used by the infants, toddlers and preschoolers during freeplay, personal care routines, outdoor play, meal times and napping. Sign in and sign out sheets and attendance sheets for each class were also reviewed. I also checked the ABCMS portal to verify that each employee had current background checks. I also spoke with the Criminal Background Check Unit and they were able to verify employee’s current records. The facility currently operates with a Four-Star License, issued April 28, 2023, earning three points in the education component, six points in the program component and one quality point for complying with the programmatic option by using Creative Curriculum. License restrictions include first and second shift, meets enhanced ratios and meets enhanced space. The last annual compliance visit was conducted September 11, 2023. The last sanitation inspection was conducted on July 12, 2024 earning a superior rating. The last fire inspection was conducted on April 8. 2024. The program compliance history was eighty-nine (89) percent as of 12/10/2023. The NC Secretary of State website was reviewed on August 13, 2024 and Glorious Community Childcare Center, LLC. was listed as current- active. I visited each indoor and outdoor space. Children enrolled were observed in free play in the centers, outdoor play, participating in teacher directed activities and routines such as diapering, toileting and handwashing and eating lunch. Infants enrolled were observed during tummy time, personal care routines, diapering and free play. Lunch was observed and consisted of ground beef with fish sticks, baked beans, pears sliced wheat bread and milk. The following violations were observed during the visit: Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Three bottles were labeled with the child's name but had no date. 15A NCAC 18A .2804(d) 542 The written feeding plan was not modified as the child's needs changed. The plan for child born 7/18/2023 was not modified and per the teacher report the child eats solid foods. 10 NCAC 09 .0902(a) 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. The Child Care Commission is currently in the process of drafting proposed child care rule language to begin the rulemaking process for implementing the new requirements. During today’s visit I also verified the following violations cited during a complaint visit conducted on 9/1/2023 as being in compliance: #303 - Children were not adequately supervised at all times. #852 - Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. #908 - Discipline was not appropriate for the child's age and development. #1048 - All staff did not successfully complete certification in First Aid appropriate to the age of children in care. #1049 – All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. #1897 - The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment.0020 Lunch was observed and consisted of meatballs, brown rice, mixed veggies, rolls and milk. The following violation was observed during the visit: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. On the NCRLAP website, there are training videos, sample questions and clarifying information regarding the assessments you may find helpful. Your facility is in Cohort 3 of the rated license process. Your prep year will begin 7/1/2025-6/30/2026. Name of Operation: GLORIOUS COMMUNITY CHILDCARE CENTER Facility ID: 53000351 Consultant: TURKESHIA MCCORMICK Operation Type: Center Case Number: 0823-380L Visit Date: 9/1/2023 Number Present: 7 Completed Date: 9/1/2023 Age: From 1 To 4 Total Minutes: 225 Time In: 09:45 AM Time Out: 01:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to obtain information regarding alleged violations of child care requirements. I reviewed the allegations with the director, Chasatie Toomer, and she was given the opportunity to respond. The allegations are as follows: There are concerns of the children are being cared for in an unsafe environment. There are also concerns that children are not being adequately supervised. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, the license and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. Two new staff members have been employed since the last visit. Those files were reviewed and violations were documented. Additionally, I observed the indoor and outdoor spaces used by the children, A total of seven children were present. I observed the educators and children in the classroom used by the toddlers in free play and routines such as handwashing and diapering and toileting. No infants were present. I also observed the educators and children from the spaces used by the preschoolers and toddlers in outdoor play, free play and lunch. Today’s menu consisted of hotdogs, peas, pineapples, buns and milk. Based on information provided, August 29, 2023, there were concerns that the children were not being cared for in an unsafe environment and not being adequately supervised. I interviewed the director. During my interview with the director, she stated that the facility recently had a break in the spaced used by the infants in which the window was broken and glass was in the classroom and the outside of the building. She stated that this incident happened on a Sunday and that the room was closed and properly cleaned prior to allowing any children to go back into that space. During the visit, I observed the children in the space used by the toddlers. One child was observed standing on the play dresser located in dramatic play while the educator in that space sat over in the reading area with two other children. The educator told the child to get down. I also observed a child climb on top of the cots located by the sink. Another child came over and climbed on top of the cots as well and proceeded to turn the lights off. It was not until the second child turned the lights off that the teacher noticed that both children were standing on top of the cots. While I was in the office speaking with the director about the visit, a child left out of the space used by the toddlers and walked down the hall into the office where we were. The child had been in the office for about 15 seconds before the teacher called out to ask to the director if the child had come into the office. I was also able to review the attendance records. There were not any incident reports or logs available. During my walkthrough of each space as well as the outside of the building I did not find any broken glass or other broken materials. Based on the above information, this allegation concerning the children not being adequately supervised was substantiated. The allegation regarding concerns of the children being cared for in an unsafe environment was unsubstantiated. The following violations were observed: Violation Number Comment Rule 303 Children were not adequately supervised at all times. A child in the space used by the toddlers walked out of the class alone during the visit and came into the office with the director and consultant. .1801(a)(1-5) 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. Per the director's report, incident reports for biting incidents in the space used by the toddlers have not been completed. .0802 (e) 908 Discipline was not appropriate for the child's age and development. A child in the space used by the toddlers was placed in a high chair after they had bitten another child. .1803(b) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One educator who began employment on 5/15/2023 did not completed First Aid Training by the 8/13/2023 due date. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One educator who began employment on 5/15/2023 had not completed CPR Training by the 8/13/2023 due date. .1102(d) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. The training was not completed by the 8/13/2023 due date for an employee who began employment on 5/15/2023. .1102(g) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Your compliance history was ninety-six (96) prior to today’s visit. The violation(s) documented must be corrected immediately. On or before September 18, 2023, (due to the Holiday) I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. If you email the compliance letter, it must be sent from the email address registered with the DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Please note that because the allegation was substantiated, an Administrative Action may be issued and a follow-up visit may be conducted. A follow-up visit will be conducted within the next two weeks. Technical assistance was provided on supervision. Supervision describes the way caregivers help protect children from injury or other harm. Supervision is an active process. It involves watching, listening, interacting, monitoring, moving about and preventing problems. According to the American Academy of Pediatrics, “active and positive supervision” involves: 1. *Knowing each child’s abilities 2. *Establishing clear and simple safety rules 3. *Being aware of and scanning for potential safety hazards 4. *Standing in a strategic position 5. *Scanning play activities and mingling or moving around the classrooms. 6. *Focusing on the positive rather than the negative to teach what is safe for the child and other children. You can see that supervision is much more than watching. It involves all your skills as a teacher and caregiver. Supervision is your responsibility and commitment to the families who have trusted you with their children. It is also your responsibility and commitment to the children who rely on you for guidance, nurturing, protection and support. Technical assistance was also offered on revising the center’s personnel handbook book to include a policy on how to handle such behaviors as biting and hitting. By incorporating a policy for the staff, it can help to be more proactive when dealing with what is concerned typical behaviors amongst children Technical assistance was provided on incorporating themes for activity plans through-out the centers in the space. For instance, if the weekly/monthly theme is winter, you could add materials in dramatic play to turn it into a coffee shop. You could also add camping items into the other centers. By incorporating the themes and adding materials throughout the space and into the centers, the children can experience what you want them to learn and are teaching on a deeper and more personal level. Pinterest is a great source for ideas on how to add themes. Just search topics such as winter themes in dramatic play, winter themes in the science or block areas. You can also get input from the children on what they would like to see in the space that is related to the themes. Enhancing the children’s learning experience also helps to keep them actively involved and may decrease the chances of certain behaviors occurring. Please note that one employee who began employment on 5/15/2023 must complete CPR First Aid training and well as Recognizing and Responding to Suspicions of Child Maltreatment immediately as it was due 8/13/2023. At the completion of the visit, this visit summary was printed, reviewed, and a copy was left with the director/owner, Chasatie Toomer.
