The allegations are as follows: There are concerns that there are violations of child care requirements regarding sanitation and discipline. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the outdoor environment were monitored. The license and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. Files for new staff were reviewed. Additionally, I observed the indoor and outdoor space used by the children. I observed the teachers actively supervising and interacting with the children in the classrooms. I observed the children participating in free play, teacher directed activities, group times, personal care routines and transitions. Today’s menu consisted of broccoli and ham alfredo, green beans, pears and milk. Based on information provided, on January 8, 2026, the doorframes in the shared bathroom in between the spaces used by younger and older toddlers was rusted and had jagged edges. The administrator, Karen Barnes, was not present during the visit, however I did speak with her by the phone and she stated that she was not aware of the issues with the doorways in that bathroom. I also interviewed the assistant administrator, Mary Cole, and she stated that was also not aware of any issues with the doorways to the bathroom in between those spaces. During my interviews with two of the staff members that care for the children in the space used by the older toddlers, they both stated that they were not aware of the state of the bottom of the bathroom door. During my interview with the third teacher in that space, she stated that she did notice that sometimes the mop would get stuck on the bottom part of that door frame when she cleaned the floor. She also stated that she did not get a closer look at the frame and that she has not noticed any children touching that portion of the frame. During my walk-through of the spaces used by the younger and older toddlers, I observed that the bottom of the both door frames of the shared bathroom that leads into the spaces used by the younger toddlers and the older toddlers has jagged edges and a small amount of rusting. I also completed a walkthrough each of the remaining classrooms and bathrooms and found no additional issues. Based on the above information, this allegation was substantiated. The following violation was observed. Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. The doorway frames to the bathroom in the spaces used by the younger and older toddlers has jagged edges and rusting on the bottom portions. Rule reference .0601 (C). .0601(c) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before January 27, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. The NC Secretary of State website was reviewed on November 19, 2025, and First Presbyterian Church of Sanford, North Carolina, Presbyterian Church (u.s.a) was listed as current-active. The license was posted, and the restriction was in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. This is a religious sponsored that operated under a G.S. 110-106. The Notice of Compliance was issued November 17, 2006. The current permit restriction is for daytime care only. Karen Barnes, administrator, assisted me with the visit. The last annual compliance visit was conducted on June 27, 2024. The last sanitation inspection was conducted on July 8, 2024, earning a superior rating. The last fire inspection was conducted on November 3, 2023. The program’s compliance history was ninety-nine (99) percent as of 6/1/2025. The NC Secretary of State website was reviewed on June 2, 2025, and First Presbyterian Church of Sanford was listed as current- active. I visited each indoor and outdoor space. Children enrolled were observed eating lunch, personal care routines and transitioning from lunch to nap. Infants enrolled were observed during nap time and tummy time. Lunch was also observed and consisted of beef with gravy and rice, sliced carrots and mandarin oranges and milk. The following violation was observed during today’s visit: Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The facility did not notify the Division of four current employees, T. Whitaker, C. Jones, B. Barhem and M. Whitley-Selig. G.S. 110-90.2 & .2703(r) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before June 16, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected.
