Estelle Lindsey, owner, and Quanik James, administrative assistant, assisted me with the visit. The facility operated with a Five-Star License, issued September 1, 2020, earning five points in the education component, seven points in the program component and one quality point for complying with the education option by using the approved curriculum, Teaching Strategies. License restrictions include ages 0-12, first shift, daytime care only, meets enhanced ratios minus one, meets enhanced space and children under 2 ½ in rooms with direct exits only. The last annual compliance visit was conducted June 4, 2025. The last sanitation inspection was conducted on December 11. 2025. The last fire inspection was conducted on August 6, 2025. The program compliance history was ninety-six (96) percent as of 5/5/2026 and was reviewed with Lindsey. I visited each indoor and outdoor space. Children enrolled were observed in free play in the centers, outdoor play, participating in teacher directed activities and personal care routines. Infants enrolled were observed during tummy time, personal care routines, free play and teacher directed activities. Teachers were interacting with the children and attending to their needs. Lunch consisted of ham, macaroni and cheese, green beans, mixed fruit and milk. The following violations were observed during the visit: Violation Number Comment Rule 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. B. Marks began employment on 12/5/2025 and had not completed the training. .1102(g) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before May 20, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. During this visit we discussed the QRIS Modernization Pathways to the Stars. Mrs. Lindsey has decided to follow the option #2 Program Assessment for Child Care Centers and Centers Located in a Residence (CLIAR). This pathway includes the following: • Enhanced Ratio(required) and/or Enhanced Space (optional) • 50% of all Lead Teachers and 50% other educators meet 5 Star Education Standards • Family and Community Engagement Foundational Practices AND 3 additional choices • CQI Plan for Facility and Individuals • Approved curriculum for all ages served • Child Observations and assessments • Coaching/training options for administrators and lead teachers. • NCRLAP Self Study I reviewed your Continuous Quality Improvement and Professional Development Plan, work experience form, Pathway #2 -Classroom and Instructional Quality for Child Care Centers and Centers Located in a Residence (keep those copies and continue working on completing them prior to your next visit.) Mrs. Lindsey stated that they have already begun reviewing on the Facility CQI plan for both the center and the staff. The last annual compliance visit was conducted July 19, 2023. The last sanitation inspection was conducted on June 27, 2024 earning a superior rating. The report was received and entered in Regulatory during the visit. The last fire inspection was conducted on August 14, 2023. The fire inspection month was changed from April to June in the regulatory system during this visit. The program compliance history was ninety-four (94) percent as of 6/30/2024. I checked the NC Secretary of State website on 7/1/2024 and ESTELLE’S CHILDCARE, INC. was listed as current-active I visited each indoor and outdoor space. Children enrolled were observed in free play in the centers, outdoor play, participating in teacher directed activities, routine handwashing and diapering and toileting. Infants enrolled were observed during tummy time, snack time and free play. Lunch was observed and consisted of fried chicken, baked beans, mixed fruit, sliced wheat bread and milk. The following violations were observed during the visit: Violation Number Comment Rule 542 The written feeding plan was not modified as the child's needs changed. The feeding plan for an infant born 10/17/2023 was not modified to include baby food per the teacher report. 10 NCAC 09 .0902(a) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In the space used by the older preschoolers, albuterol for M. Norman expired 2/2024. Authorization to administer albuterol and an epi pen for D. Spencer expired 2/21/2024. .0803(12) 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. Technical assistance was also offered on reviewing both staff and children’s records as well as classroom documentation such as permission to administer forms to ensure that all required documentation is on complete and on file. Try reviewing these files periodically to keep up with deadlines for trainings and any other pertinent information. We discussed requesting Environment Rating Scale (ERS) assessments in an effort to maintain the program's star rating. Based on information provided, April 9, 2024, I observed arrivals for six children as well as attendance records and sign-in and sign-out sheets for each class. Upon my arrival to the facility into the parking lot at 8:10 am, two children had just arrived with a parent. Two more children entered the building behind me at 8:12 am and were taken to their respective classrooms by their parent who immediately left afterwards. I entered the space used by the infants at 8:15 and observed two staff members along with two children who were seated in highchairs eating cheerios and pears. I completed a walkthrough of the remaining five classrooms. At 8:30 am, a parent with two children arrived in the space used by the infants and the space used by the two-year-old class. I reentered the space used by the infants at 8:35 am and observed a one year old who had just arrived in a highchair eating cereal and sippy cup with a blue top was in the bottle warmer. At 9:05 am the same sippy cup was in the bottle warmer and the teachers and the children in that space were on the floor engaged in free play. The bottle warmer was on and the timer was not set. The attendance record for the Month of April in space #3 used by the young preschoolers, attendance for 4/9/2024 had not been taken. After reviewing the sign in times for the morning of the visit, child A who arrived at 8:12 am had been signed in at 8:08 am by the teacher in space #3. The facility operated with a Three-Star License, issued October 25, 2017, earning four points in the education component, two points in the program component and one quality point for complying with the education option. License restrictions include first shift, daytime care only, ages 2-12, only children 2 ½ and older in care, only children 5-12 may use the large climbing structure, and meets enhanced space. The administrator, Tanya Williams, assisted me with the visit. The last annual compliance visit was conducted on July 19, 2023. The last sanitation inspection was conducted on July 21, 2023 earning a superior rating. The last fire inspection was conducted on August 14, 2023. The program compliance history was ninety-five (95) percent as of 3/4/2024. The NC Secretary of State website was reviewed on March 5, 2024 and Estelle’s Child Care, Inc. was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. The infants enrolled were observed in tummy time, routines, snack time and free play. The caregivers were interacting and meeting the developmental needs for each of the children. The NC Pre-k class was closed on today due to a teacher workday. One staff file was reviewed for one new employee and had no violations. Lunch today consisted of fish stick/fillet with bun, fries, sliced pears and milk. The following violations were observed today: Violation Number Comment Rule 415 A current schedule was not posted for each group of children for reference. A daily schedule was not posted in the space used by the infants and the teacher reported that it was not available. GS 110-91(12);.0508(a) 445 For children under three years of age, developmentally appropriate toys and activities were not provided as outlined in Rule. A six month on old infant was in a bouncey seat for twenty minutes without any materials. .0510 (e ) (1)(A-G) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical assistance was also offered on playing music during free play or other group activities without an intended purpose. Sometimes when music is played in the background as a filler, it can take away the opportunity for meaningful conversations and interactions to take place. At other times it can cause the noise level in the classroom to be higher than normal because individuals have to speak over the music in order to be heard. Try limiting times when music is played to music/movement time, as a transitional prompt or during rest periods. Technical assistance was also offered on using verbal prompts for transitions such as “in 5 minutes we are going to clean up” or “1,2,3 eyes on me” and also using other cues such as clapping in patterns or turning the lights off to gain the children’s attention for the next activity. Technical assistance was also on staff refraining from using free play time and structured group activities or teacher directed activities to work on teacher responsibilities such as labeling or restocking routine class materials. The time children use as learning time in the classrooms are opportunities for all staff present to engage with the children and enhance their learning experience. Even when staff/child ratios are low and adequate supervision is being provided, it is still best practice for all staff present to be hands on with the children’s learning experiences. We discussed requesting Environment Rating Scale (ERS) assessments in an effort to maintain the program's star rating. The NC Secretary of State website was reviewed on March 5, 2024 and Estelle’s Child Care, Inc. was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. The infants enrolled were observed in tummy time, routines, snack time and free play. The caregivers were interacting and meeting the developmental needs for each of the children. The NC Pre-k class was closed on today due to a teacher workday. One staff file was reviewed for one new employee and had no violations. Lunch today consisted of fish stick/fillet with bun, fries, sliced pears and milk. The following violations were observed today: Violation Number Comment Rule 415 A current schedule was not posted for each group of children for reference. A daily schedule was not posted in the space used by the infants and the teacher reported that it was not available. GS 110-91(12);.0508(a) 445 For children under three years of age, developmentally appropriate toys and activities were not provided as outlined in Rule. A six month on old infant was in a bouncey seat for twenty minutes without any materials. .0510 (e ) (1)(A-G) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical assistance was also offered on playing music during free play or other group activities without an intended purpose. Sometimes when music is played in the background as a filler, it can take away the opportunity for meaningful conversations and interactions to take place. At other times it can cause the noise level in the classroom to be higher than normal because individuals have to speak over the music in order to be heard. Try limiting times when music is played to music/movement time, as a transitional prompt or during rest periods. Technical assistance was also offered on using verbal prompts for transitions such as “in 5 minutes we are going to clean up” or “1,2,3 eyes on me” and also using other cues such as clapping in patterns or turning the lights off to gain the children’s attention for the next activity. Technical assistance was also on staff refraining from using free play time and structured group activities or teacher directed activities to work on teacher responsibilities such as labeling or restocking routine class materials. The time children use as learning time in the classrooms are opportunities for all staff present to engage with the children and enhance their learning experience. Even when staff/child ratios are low and adequate supervision is being provided, it is still best practice for all staff present to be hands on with the children’s learning experiences. We discussed requesting Environment Rating Scale (ERS) assessments in an effort to maintain the program's star rating. Resources such as Child Care Resource and Referral and Smart Start can be valuable when preparing for the ERS. “All about ECERS-R” and the NCRLAP website (www.ncrlap.org) can also be helpful as staff prepare for the ERS. On the NCRLAP website, there are training videos, sample questions and clarifying information regarding the assessments you may find helpful. As a reminder, M. Roper’s CPR and First Aid training will expire on 3/7/2024. In order to ensure that compliance is maintained, please enroll them in the appropriate training asap. Your facility is in Cohort 1 of the rated license process. Your prep year began 7/1/2023 and will continue until 6/30/2024. Please consider contacting the Partners for Families and Children of Lee County for help with technical assistance, room arrangements, mock observations, self observations and in any other areas that will help prepare you for the assessments as well as increase/maintain the quality level of care that your staff provide. The last annual compliance visit was conducted August 2, 2022. The last sanitation inspection was conducted on November 2, 2021 earning a superior rating. The last fire inspection was conducted on August 19, 2022. Although the sanitation inspection is delinquent, the violation was given during a previous visit and the facility has been in contact with the environmental health consultant to inquire about a visit. Tanya Williams, director, assisted me with this visit. The program compliance history was ninety-four (94) percent as of 7/19/2023. A preservice requirement for administrator form was completed during the visit by Mrs. Williams and the information was entered in Regulatory.
