Upon arrival, I was greeted by you, Grace Fullbright, Lead Teacher, and Jessica Moscardini, Teacher, and I stated the reason for my visit. Currently, this facility operates with a temporary license issued from August 22, 2025, through February 22, 2026. Day time care only is the restriction listed on the license. The facility’s temporary license, NC Summary of Child Care Law poster Classroom Staff to Child Ratio chart, safe arrival and departure procedures, emergency medical care plan, evacuation plan, daily schedule, activity plan, and emergency phone numbers were posted in the classroom. The students were observed eating their lunch, writing and drawing pictures to Santa Clause, playdoh activities, hand-washing procedures, story time and departure procedures. Positive interactions were observed throughout today’s visit. There are no new staff since my last visit. A portion of the children’s files were monitored today. Supervision, adequate approved space use, capacity, group size, staff/child ratios and permit restrictions were monitored. I monitored hazardous product storage, medication storage, medication administration, general safety, and discipline practices. The last sanitation inspection was conducted on November 19, 2025 with no demerits and a superior rating, and the last fire inspection was conducted on July 29, 2025. The last monthly fire drill was conducted on December 16, 2025, and the last emergency drill was conducted on October 16, 2025. The last outdoor inspection was conducted on August 18, 2025. You stated that you were unaware that outdoor inspections needed to be documented monthly. The outdoor play area was monitored. I monitored NC PreK requirements during today’s visit. This program uses DIAL-4 as the developmental screening tool, Teaching Strategies Gold as the formative assessment tool, and Creative Curriculum for Preschool, Fourth Edition, as the approved curriculum. The health assessment including the physical examination, immunizations, vision screening, hearing screening, and dental screening are used to refer children for additional services as needed. The Parent Involvement Log was monitored during today’s visit. Rated License Discussion – I received the “Application for Assessment for a Rated License for Centers” on November 5, 2025. This program is choosing Classroom and Instructional Quality Assessment (Pathway 2). On the application, it was checked that this program would meet enhanced reduced staff/child ratio and enhanced space. During today’s visit you showed me a ring binder in which you have all the QRIS documents and notes in which I monitored and you had an opportunity to ask questions. Ms. Kelly Street, Director of Early Childhood, is in the process of completing the following: 1) Staff Education Standards: Please make sure to have the QRIS Staff Information and Education Worksheet to me no later than the end of the business day on Friday, January 9, 2026. I monitored DCDEE WORKS and the Lead Teacher and Teacher have current status letters. The principal/administrator is showing that her account is in process. The WORKS evaluations is how we determine the stars that will be awarded for education. 2) Family and Community Engagement Standards Child Care Centers Please make sure to have the Family and Community Engagement Standards Child Care Centers / Centers Located in a Residence document to me no later than the end of the business day on Friday, January 9, 2026. 3) Facility Continuous Quality Improvement (CQI) Plan Please make sure to have the facility CQI Plan document to me no later than the end of the business day on Friday, January 9, 2026. 4) Please make sure to have the Continuous Quality Improvement (CQI) and Professional Development (PD) Plan for staff and administration in their files for review no later than the end of the business day on Friday, January 9, 2026. 5) Approved Curriculum* You stated that the approved curriculum used in the classroom is Creative Curriculum. I verified use today. 6) Approved Formative Assessment* The approved formative assessment used is Teaching Strategies Gold. I verified use today. 7) Coaching/training options for administrators and lead teachers *Documentation that the administrator and lead teachers received training related to the curriculum and formative assessment or the plan that has been put in place to ensure that the administrator and lead teachers receive training related to the curriculum and formative assessment will need to be submitted to me no later than the end of the business day on Friday, January 9, 2026. Per child care rule .3205(f)(10) pertaining to coaching, mentoring, and training options for the administrator, the administrator should plan to participate in one of the following activities regarding classroom and instructional quality practices: (A) annual completion of five hours of coaching or mentoring by an administrator of another licensed child care center with a star-rating of five stars, or a community resource partner; (B) annual completion of five on-going training hours, in additional to applicable requirements in child care rule .1103 and child care rules .3211, .3212, .3213, .3214, .3215, .3216 and .3218; or (C) annual completion of 0.5 continuing education unit, in addition to applicable requirements in child care rule .1103 and child care rules .3211, .3212, .3213, .3214, .3215, .3216 and .3218. Per child care rule .3205(f)(11) pertaining to coaching, mentoring, and training options for the lead teacher, all lead teachers should plan to participate in one of the following activities regarding classroom and instructional quality practices: (A) annual completion of five hours of coaching or mentoring by the administrator, another lead teacher in the center, a lead teacher in another licensed child care center with a star-rating of five stars, an administrator of another licensed child care center with a star-rating of five stars, or a community resource partner; (B) annual completion of five on-going training hours, in addition to applicable requirements in child care rule .1103 and child care rules .3211, .3212, .3213, .3214, .3215, .3216 and .3218; or (C) annual completion of 0.5 continuing education unit, in addition to applicable requirements in child care rule .1103 and child care rules .3211, .3212, .3213, .3214, .3215, .3216 and .3218. Please choose the option in which the administrator and the lead teacher plan to participate and begin working to complete these requirements by August 22, 2026. The star rating of this facility will be determined after January 9, 2026, when I receive the required documentation. Star Rated License Child Care programs are expected to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent at all times [NC GS 110-90(4)(d)]. After today's visit the facility's compliance history is at 95%. In addition to the star rated license assessment, the Early Childhood Environmental Rating Scale (ECERS) 3 assessment will need to be completed. I received the “Rated License Assessment Request Review” form on November 5, 2025, and the request for the assessment has been submitted. The ERS window has been scheduled between January 12, 2026, to February 6, 2026. The following violations were observed during today. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. Arrival and departure times for the children present on 12/15 and 12/16 was not documented. 10A NCAC 09 .0302(d)(4) 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. There was one child's water bottle that did not have his name on it. 15A NCAC 18A .2804(d) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The last playground inspection completed was on 8/18/25. .0605(q) TECHNICAL ASSISTANCE: -We discussed keeping masking tape and a marker close to the door so that you and staff can label water bottles as children arrive if families did not label them. -You stated that were not aware that outdoor inspections needed to be documented monthly. You will want to make sure to complete the DCDEE outdoor inspection form monthly. It may be helpful to place a reminder on your calendar as a reminder. -You stated that you were unable to make copies of the arrival/departure forms this week and were writing the children present on a piece of paper but due to a crazy week with the holidays coming the times were not documented. It is important that this is completed daily and it may be helpful to have an abundance forms available. It also may be helpful to take with you when you when you greet the children in the morning and when you take the children to the car rider line in the afternoon. CONSULTATION for the ECERS-3 ASSESSMENT: 1. Remember that the ECERS-3 assessment will be completed in three hours with no interview process. If the assessors do not observe an indicator in action or posted as required, there will be no opportunity to explain the omission. 2. If your current classroom schedule and routine allows for outside play after 11am, you may want to revise your classroom schedule to include outside play in the first three hours of the school day. If you choose to make a schedule change, I suggest you make those changes as soon as possible and to begin to train the children and staff in the new routine. 3. Allow bleach water to sit on surfaces at least two minutes before wiping off or using the surface. 4. Ensure that children wash hands upon arrival, before and after water play, after messy art/play dough play, after coming in from outside, before eating, after toileting/diapering and after coughing/sneezing, etc. 5. Ensure that learning centers are accessible a substantial portion of the day and that your daily schedule reflects it. Areas that must be made available for a minimum of 2hrs and 10 minutes to meet the “Substantial Portion of the Day” requirement are: Fine Motor, Art, Blocks, Dramatic Play, Nature/Science/Exploration, Math/Numbers, and Books. 6. Ensure that children go outside every day weather permitting. 7. Ensure that your block centers are protected from foot traffic, large enough for three children to build a sizeable structure and enough blocks for three children to build. 8. Ensure that books and materials are in good repair and no books depict violent images (guns, swords, teeth bearing). 9. Ensure that the majority of direction/redirection is positive instead of negative. 10. Ensure that staff member's pocket books are stored in locked storage since they almost always contain medications or lotions that must be inaccessible to children. 11. Ensure that staff are interacting with children, engaging in meaningful conversations, and helping children work through altercations and conflicts as they occur. RATED LICENSE CONSULTATION: 1. All staff must have their current education evaluated by the Education Workforce Unit in Raleigh. No action is needed on your part if you have already completed the ABCMS Provider Portal training and are currently using the ABCMS Provider Portal to update information regarding veteran and new hires. Be sure this information is updated in ABCMS on an ongoing basis as staff members are hired and terminated. If you are not already using the ABCMS Provider Portal to update information regarding veteran and new hires, you will need both an Individual NCID and a Business NCID. You will need an email address for your Individual NCID and a different email address for your Business NCID account. Once you have created your Business NCID, complete the ABCMS Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. Once the training has been completed and access has been given to you, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. These actions satisfy the requirement to notify the Division within five business days of new child care providers working who were hired or moved into the child care facility. Currently, this facility operates with a temporary license issued from August 22, 2025, through February 22, 2026. Day time care only is the restriction listed on the license. The facility’s temporary license, NC Summary of Child Care Law poster Classroom Staff to Child Ratio chart, safe arrival and departure procedures, emergency medical care plan, evacuation plan, daily schedule, activity plan, and emergency phone numbers were posted in the classroom. The students were observed eating their lunch, writing and drawing pictures to Santa Clause, playdoh activities, hand-washing procedures, story time and departure procedures. Positive interactions were observed throughout today’s visit. There are no new staff since my last visit. A portion of the children’s files were monitored today. Supervision, adequate approved space use, capacity, group size, staff/child ratios and permit restrictions were monitored. I monitored hazardous product storage, medication storage, medication administration, general safety, and discipline practices. The last sanitation inspection was conducted on November 19, 2025 with no demerits and a superior rating, and the last fire inspection was conducted on July 29, 2025. The last monthly fire drill was conducted on December 16, 2025, and the last emergency drill was conducted on October 16, 2025. The last outdoor inspection was conducted on August 18, 2025. You stated that you were unaware that outdoor inspections needed to be documented monthly. The outdoor play area was monitored. I monitored NC PreK requirements during today’s visit. This program uses DIAL-4 as the developmental screening tool, Teaching Strategies Gold as the formative assessment tool, and Creative Curriculum for Preschool, Fourth Edition, as the approved curriculum. The last playground inspection completed was on 8/18/25. .0605(q) TECHNICAL ASSISTANCE: -We discussed keeping masking tape and a marker close to the door so that you and staff can label water bottles as children arrive if families did not label them. -You stated that were not aware that outdoor inspections needed to be documented monthly. You will want to make sure to complete the DCDEE outdoor inspection form monthly. It may be helpful to place a reminder on your calendar as a reminder. -You stated that you were unable to make copies of the arrival/departure forms this week and were writing the children present on a piece of paper but due to a crazy week with the holidays coming the times were not documented. It is important that this is completed daily and it may be helpful to have an abundance forms available. It also may be helpful to take with you when you when you greet the children in the morning and when you take the children to the car rider line in the afternoon. CONSULTATION for the ECERS-3 ASSESSMENT: 1. Remember that the ECERS-3 assessment will be completed in three hours with no interview process. If the assessors do not observe an indicator in action or posted as required, there will be no opportunity to explain the omission. 2. If your current classroom schedule and routine allows for outside play after 11am, you may want to revise your classroom schedule to include outside play in the first three hours of the school day. If you choose to make a schedule change, I suggest you make those changes as soon as possible and to begin to train the children and staff in the new routine. 3. Allow bleach water to sit on surfaces at least two minutes before wiping off or using the surface. 4. Ensure that children wash hands upon arrival, before and after water play, after messy art/play dough play, after coming in from outside, before eating, after toileting/diapering and after coughing/sneezing, etc. 5. Ensure that learning centers are accessible a substantial portion of the day and that your daily schedule reflects it. Areas that must be made available for a minimum of 2hrs and 10 minutes to meet the “Substantial Portion of the Day” requirement are: Fine Motor, Art, Blocks, Dramatic Play, Nature/Science/Exploration, Math/Numbers, and Books. 6. Ensure that children go outside every day weather permitting. 7. Upon arrival, I was greeted by school office staff who directed me to classroom 304 where the NC Pre-K program is located. I was greeted by you, Grace Albright, Lead Teacher, and Jessica Moscardini, Teacher, in the cafeteria as the children were finishing up their lunch. You stated that you would be having an observation at 11:30 by Ms. Marrero, Assistant Principal, and she arrived as we entered the classroom from the cafeteria. A speech therapist also arrived to work with a student. Currently, this facility operates with a temporary license issued from August 22, 2025, through February 22, 2026. Day time care only is the restriction listed on the license. The facility’s temporary license, NC Summary of Child Care Law poster Classroom Staff to Child Ratio chart, safe arrival and departure procedures, emergency medical care plan, evacuation plan, daily schedule, activity plan, and emergency phone numbers were posted in the classroom. The students were observed eating their lunch, group time and free choice activities. Positive interactions were observed throughout today’s visit. Supervision, adequate approved enhanced space use, enhanced capacity and group size, enhanced staff/child ratios and permit restrictions were monitored. I monitored hazardous product storage, medication storage, medication administration, general safety, and discipline practices. The last sanitation inspection was conducted on August 1, 2025, and the last fire inspection was conducted on July 29, 2025. The last monthly fire drill was conducted on September 25, 2025, and the last emergency drill was conducted on September 15, 2025. The last outdoor inspection was conducted on September 18, 2025. The outdoor play area was monitored. We discussed that now that we know that there are no lead hazards and the outside wall areas have been repainted to prevent peeling and chipping that this outdoor environment can be used by the children. You stated that this facility does not provide transportation. A portion of the student files and staff files were monitored. You stated that you have been working on the Staff and Training Worksheet and need to finish adding the principal's information. Please submit to me not later than Monday, October 6th.
