Your last Annual Compliance visit was conducted on November 7, 2024. Your facility’s compliance history score prior to today’s visit was 94%. You, Allison Stirewalt, Lead NC Pre-K Teacher, assisted me with the visit. I reviewed with you today the information found in our system. You stated there have been no changes to your phone number, email address, mailing address, administrator, or owner. I reviewed the facilities permit with you today. This facility operates with a five-star license and is licensed for 36 children on first shift, ages 3-5 with an effective date of June 27, 2019. The following restrictions are listed: first shift (daytime care), meets enhanced space, and meets highest enhanced ratios. I used the “Annual Compliance Monitoring Visit Checklist for Child Care Centers” during today’s visit. I observed your Five-Star License, NC Child Care Summary of the Law, menu, Emergency Medical Care Plan, Sanitation placard, daily schedule, activity plan, staff-child ratio worksheet, First Aid poster, tobacco free policy signage and evacuation plans posted as required. Child to staff ratios, group size, supervision, permit restrictions, and adequate and approved space were monitored. A walk-through of the indoor and outdoor spaces was completed. I observed the children participating in free choice activities indoors. Positive interactions between teachers and children were observed. There were a variety of materials available to children. Materials and furnishings were found to be developmentally appropriate. All medications and accompanying documentation were monitored. You stated your program does not provide transportation. Program records were monitored, including emergency drills, fire drills, monthly outdoor inspections, and incident logs. A sanitation inspection was conducted on April 17, 2024, with four (4) demerits and a superior classification. A satisfactory fire inspection was conducted on August 28, 2024. An unsatisfactory fire inspection was conducted on August 19, 2025. There have been no new staff members hired since the last visit. I monitored a portion of existing staff files during today’s visit. There are 16 children enrolled. I monitored a portion of the children’s records. The developmental screenings, health assessments, vision screenings, hearing screenings and dental screenings required for the NC Pre-K Program were monitored. Prior to today's visit, the Program Consultant Site Visit Information Report was reviewed in the NC Pre-K Plan system. Staff listed on the report are staff that were in the classroom. DIAL-4 is used for the developmental screening tool, Teaching Strategies Gold is the on-going assessment used and Creative Curriculum is the curriculum used. Parent involvement activities include home visits, parent-teacher conferences, Ready Rosie, classroom events/festivals, and volunteer opportunities. ParentSquare is used as a communication tool. You stated the NC Pre-K Program hours for the children are 8:00 to 2:45 and the staff hours are 7:00-3:00. The following violations were observed during today’s visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last satisfactory fire inspection was completed on August 28, 2024. 10A NCAC 09 .0304(a) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The Division was not notified through the ABCMS Provider Portal of child care providers working at the child care facility. G.S. 110-90.2 & .2703(r) Technical Assistance: • Child care rule 10A NCAC 09 .0304 requires a child care center operator to: schedule and obtain a fire inspection within 12 months of the center's previous fire inspection, to notify the local fire inspector when it is time for the center's annual fire inspection, and to submit the original of the approved annual fire inspection report to the Division within one week of the inspection visit on the form provided by the Division. A fire inspection was conducted on August 19, 2025, but indicated that the fire safety conditions in the facility were unsatisfactory. You stated that all issues have been corrected, but a new fire inspection has not been completed to your knowledge. Please contact the fire inspector to complete an updated fire inspection. • The Division must be notified within five days when new employees have been hired by your facility. The process of notifying the Division changed in February 2024 and is now captured in ABCMS. Today we reviewed the ABCMS Provider Portal. Consultation: • Today you asked for clarification regarding staff members providing coverage in the classroom. All staff members must have a valid Criminal Background Check qualification letter on file before caring for children and stepping into the classroom to hold ratio. • During today’s visit, we discussed the three QRIS Pathways to the Stars effective July 1, 2025. We covered staff education (including ensuring all staff have WORKS accounts, understanding the 50% requirement for lead teachers and for other educators, and how work experience may count), enhanced ratios and space expectations, the Family and Community Engagement Foundational Practices, and Continuous Quality Improvement (CQI) Plans. The following steps are how you can begin preparing for the Rated License process: Next Steps by Pathway- If you choose Pathway 1: - Confirm all staff have active WORKS accounts. - Complete the QRIS Staff Information and Education Worksheet electronically. - Begin planning for your three-month self-study using the applicable ITERS-3, ECERS-3, SACERS-U, OR FCCERS-3 books. - Prepare for Environmental Rating Scale (ERS) assessments. - Access resources, trainings, and outreach via ncrlap.org. - Work with CCR&R or Smart Start for support in preparation. - Identify CQI goals and begin planning for implementation. - Develop strategies for Family and Community Engagement. - If four-year-olds are enrolled, select and implement an approved curriculum. Your last Annual Compliance Visit was conducted on November 7, 2024. Your facility’s compliance history score prior to today’s visit was 98%. You, Allison Stirewalt, Lead NC Pre-K Teacher, assisted me with the visit. I observed your Five-Star License, NC Child Care Summary of the Law, menu, Emergency Medical Care Plan, Sanitation placard, staff-child ratio worksheets, First Aid poster, tobacco free policy signage and evacuation plans posted as required. There have been no new staff hired since the last visit. Criminal Background Checks, CPR/First Aid, and Recognizing and Responding to Suspicions of Child Maltreatment were monitored in staff files. Your signature on all forms served and will serve as verification that the information provided was accurate and complete. The following violations were observed during today’s visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In Space 207, an electrical outlet was not covered with a safety plug. 10A NCAC 09 .0604(c) 847 Parent's medication authorization did not include required information. In Space 207, the Medication Administration Permission Form for an EpiPen and Benadryl was last updated on 9/5/2024. 10A NCAC 09 .0803(4)(6-9) Technical Assistance: • To ensure a safe environment for children, I suggest that NC Pre-K staff complete a daily walkthrough of their classrooms each morning before children arrive to ensure the area is free of safety hazards, such as uncovered outlets. Today the violation was corrected by placing a safety cover in the uncovered outlet. • Today we discussed that written permission to administer medication for chronic medical conditions or allergies can be given for up to 6 months. I suggested that you have families complete the Medication Administration Permission Form found on the DCDEE website and place a note on your calendar of when these forms need to be updated. I printed a copy of this form and left it with you today. Consultation: • NEW QRIS MODERNIZATION PLAN: The new QRIS Modernization Plan known as “Pathways to Success” is being added to law which includes moving to the use of the updated Environmental Rating Scale (ERS-3 versions) for ITERS, ECERS, and FCCERS, effective February 1, 2025. DCDEE and the NC Child Care Commission will continue the rulemaking process to add the rule language to child care requirements. This process with take several months. Hold Harmless provisions have been extended. Star rated license reassessments are postponed until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. A pathway for child care programs currently accredited through the approved accrediting bodies may now move to a 3- or 5-star license if they are not already at this star level. Providers need to submit a request to their licensing consultant and provide a copy of their accreditation award/certificate for processing: Accreditation Licensure Request Form. Your last Annual Compliance visit was conducted on December 12, 2023. The current 18-month compliance history is 99%. Ashley Joyner, NC Pre-K Teacher, and Jessica Moscardini, NC Pre-K Teacher, assisted me with the visit today. This facility operates with a five-star license and is licensed for 36 children on first shift, ages 3-5 with an effective date June 27, 2019. The following restrictions are listed: daytime care only, meets enhanced space, and meets highest enhanced ratios. I used the "Annual Compliance Monitoring Visit Checklist for Child Care Centers" during today's visit. Child to staff ratios, group size, supervision, permit restrictions, and adequate and approved space were monitored. A walk-through of the indoor and outdoor spaces was completed. The children were observed during free choice activities, group time, and lunch. All medications and accompanying documentation were monitored. This program does not provide transportation. Program records were monitored, including emergency drills, fire drills, monthly outdoor inspections, and incident logs. A sanitation inspection was conducted on May 21, 2024 with two (2) demerits and a superior classification. A fire inspection was conducted on August 28, 2024. There has been one new staff member hired since the last visit. I monitored their file and a portion of the staff files. One staff member's qualification letter expired June 6, 2024. This violation must be corrected within fifteen (15) days and a copy of the qualification letter must be sent to me with the required compliance letter. I monitored a portion of the children’s records. The developmental screenings, health assessments, vision screenings, hearing screenings and dental screenings required for the NC Pre-K Program were monitored. Prior to today's visit, the Program Consultant Site Visit Information Report was reviewed in the NC Pre-K Plan system. Staff listed on the report are staff that were in the classroom. DIAL-4 is used for the developmental screening tool, Teaching Strategies Gold is the on-going assessment used and Creative Curriculum is the curriculum used. The last annual compliance visit was conducted on January 9, 2023. The Environmental Rating Scale (ERS) assessment was conducted in April 2019. The 18-month compliance history prior to today’s visit was at 97%. The license was posted, and the following restrictions were listed: daytime care only, meets highest enhanced ratios and meets enhanced space. Upon arrival, I was greeted by office staff, and I stated the reason for my visit. I observed the children during free choice activities indoors and outdoors, hand-washing routines and transitioning to use the restroom that is down the hall from classroom 207. I observed children in classroom 500 during group time and lunch. I observed positive interactions between staff and children. I also observed the children during free choice activies outdoors and the two staff sitting in folding chairs supervising the children playing. I provided technical assistance and discussed the importance of walking around to supervise and engage in play with the children. After our discussion the staff were observed moving and engage with play with the children. The following items were monitored: supervision, staff/child ratios, CPR and First Aid, special training, criminal background checks completed, general safety, discipline, Emergency Medical Care Plan, storage of hazardous products, adequate/approved space, program records, posted license and restrictions met. The emergency drills and outdoor inspections were monitored and are occurring as required. A fire inspection was completed on February 15, 2023. The sanitation inspection was completed on November 9, 2023, with no demerits. There have been two new staff members hired since my last visit and their files and a portion of the existing staff files were monitored. There were 21 children present today and 31 children are enrolled. I monitored a portion of the children’s records. The Program Consultant Site Visit Information Report was reviewed in the NC Pre-K Plan system prior to the visit. The staff listed in the report are the staff that are working in the NC Pre-K classroom. DIAL-4 is the developmental screening tool used. Make sure to monitor the dates when receive the report. This is also the case when a child transfers from one NC Pre-K program at one facility to another NC Pre-K at another facility. CONSULTATION: -NC PRE-K teacher assistants are required to complete a minimum of 15 hours of annual in- service professional development. A combination of college coursework, Continuing Education Units (CEUS), or clock hours may be used to complete the requirements of this Rule. -The summary of the law has been updated.
