The visit was conducted with you, Mihaya Pitts, Lead Teacher. This program currently operates with a Five star license issued on March 21, 2025. The permit restrictions include 1st shift, enhanced ratios, enhanced space, and enhanced ratios minus one. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year -old children are enrolled. The last annual compliance visit was conducted January 6, 2025. The most recent sanitation inspection for your facility was conducted on September 11, 2025. A superior sanitation classification was issued with no demerits noted on the grade card. The most recent approved fire inspection for your facility was conducted on August 21, 2025 for daytime care only. The most recent monthly fire drill was conducted on November 3, 2025. The most recent quarterly lockdown drill was conducted on September 22, 2025. The facilities EPR Plan was reviewed August 4, 2025. The program uses the Rutherford County Schools Critical Response Plan. The most recent monthly playground inspection was completed on November 5, 2025. The center's compliance history was reviewed with the operator. The program’s compliance history was 99 percent as of November 26, 2025. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group story time, free play activities indoors, handwashing routines, lunch, and rest time. The caregivers were interacting and meeting the developmental needs for each of the children. A checklist was used to note the requirements I monitored today. The analysis date for the most recent lead water test was January 16, 2025. Lead testing must be completed every three years. You provided me with applicable program, staff, and children’s records for review. Files for one existing staff, one substitute staff member present today, and applicable records for all staff were reviewed. All staff were listed on the ABCMS Provider Portal roster for this facility. A sample of two children’s records were reviewed. NC Pre-K requirements were monitored today. Violations observed today were discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. The following violations were observed: Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Michelle Clement had a qualification letter that expired August 4, 2025. The new valid qualification letter on file was dated September 14, 2025. G.S. 110-90.2(b) & .2703(n)&(o) 1769 The health assessment did not include a dental screening. One child four years of age that enrolled on September 11, 2025, did not have a dental screening on file for review. .3005 (a)(5) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before December 15, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. Ms. Clement’s valid qualification letter dated September 14, 2025 was on file for review during today’s visit. This violation was corrected during the visit. I suggested you create a working document to track the expiration dates of the qualifying letters for all staff members to ensure all staff members maintain a current and valid qualifying letter. I also suggested you remind staff two (2) months prior to the expiration date of their qualifying letter to complete an application for an updated qualifying letter to begin the process. A health assessment must be on file at the NC Pre-K site within 30 days after a child enters the NC Pre-K program and the assessment may be no more than 12 months old at the time of program entry. The health assessment must include the following: (1) physical examination; (2) updated immunizations; (3) vision screening; (4) hearing screening; and (5) dental screening. A dental screening may be completed on a separate form and must also be on file within 30 days after a child enters the NC Pre-K program. I suggested you obtain the dental screening from the parent during the enrollment process to ensure it is on file within the required timeframe. Consultation: One staff member employed on September 20, 2010 had verification of four completed annual on-going training hours on file. Verification of a total of twenty completed on-going training hours must be on file by January 6, 2026. The ABCMS provider portal facility roster should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The permit restrictions were in compliance including 1st shift, meets enhanced ratios, meets enhanced space and meets ratios reduced by one in each group. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios in compliance. Children were participating in outdoor gross motor time, transitions/handwashing protocols, and play in activity centers. The center's compliance history prior to today’s visit was reviewed with the operator. The program’s compliance history was 99% as of August 28, 2024. Upon arrival, the purpose of today’s visit was reviewed with you, Mihaya Pitts, NC Pre K Lead Teacher. Your program currently operates with a five star license effective June 7, 2019. The permit restrictions were in compliance including 1st shift, meets enhanced ratios, meets enhanced space and meets ratios reduced by one in each group. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios in compliance. Children were participating in outdoor gross motor time, transitions/handwashing protocols, and play in activity centers. The center's compliance history prior to today’s visit was reviewed with the operator. The program’s compliance history was 99% as of August 28, 2024. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. One new staff member has been hired since the annual compliance visit on January 18, 2024. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. The most recent lead water testing results were completed on July 30, 2021. Lead testing must be completed every three years. A supervisor sanitation classification was issued with no demerits. The most recent fire inspection was conducted on August 19, 2024. The facility was approved for daytime care. The following violation was observed today: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The current approved fire inspection was completed on August 19, 2024. The previous fire inspection was completed on August 13, 2023. 10A NCAC 09 .0304(a) The violation observed today was corrected during the visit. No violation correction letter is required. Technical assistance on violations was reviewed with you. Areas discussed included: Per 10A NCAC 09.0304, Each operator shall schedule and obtain a fire inspection within 12 months of the center's previous fire inspection. The operator shall notify the local fire inspector when it is time for the center's annual fire inspection. The operator shall submit the original of the approved annual fire inspection report to the Division within one week of the inspection visit on the form provided by the Division. The previous inspection was completed on August 13, 2023. This year’s inspection was completed on August 19, 2024. There is now an approved inspection on file and a copy was given to me for the Division during the visit. This violation was corrected during the visit. Consultation or other topics discussed today: We discussed that activity plans and menus must be posted in an area that parents and service providers are able to view. Hold harmless has been extended until the new QRIS system is implemented. Programs still have the option to choose to voluntarily complete the star rated license reassessment if they so desire. Lead & Asbestos Testing The Clean Classrooms for Carolina Kids Program is providing free facility-wide testing and inspections without any out-of-pocket cost to child care centers and public schools.
