The visit was conducted with you, Patrcia Deluca, NC Pre K Lead Teacher. Ashley Twitty, Administrator assisted during the visit with completion of the Program Assessment Pathway #3 paperwork. This program currently operates with a Five-star license issued on August 8, 2025. The permit restrictions include 1st shift, enhanced ratios, enhanced space, and enhanced ratios minus one. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year -old children are enrolled. Your program uses Creative Curriculum Teaching Strategies Gold. The last annual compliance visit was conducted April 9, 2025. The most recent sanitation inspection for your facility was conducted on October 8, 2025 A superior sanitation classification was issued with no demerits noted on the grade card. The most recent approved fire inspection for your facility was conducted on August 26, 2025 for daytime care only. The most recent monthly fire drill was conducted on March 2, 2026. The most recent quarterly shelter-in-place drill was conducted on March 5, 2026. The most recent monthly playground inspection was completed on March 2, 2026. The center's compliance history was reviewed with the operator. The program’s compliance history was 97 percent as of March 6, 2026. The analysis date for the most recent lead water test was January 16, 2025. Lead testing must be completed every three years. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in free play in activity center, indoor gross motor activities, and handwashing routines. The caregivers were interacting and meeting the developmental needs for each of the children. A checklist was used to note the requirements I monitored today. You provided me with applicable program, staff, and children’s records for review. Files for one new staff, one existing staff, and applicable records for all staff were reviewed. A sample of two children’s records were reviewed. NC Pre-K requirements were monitored today. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. A selection of files were monitored for completed health assessments and developmental screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. Parent conferences are held three times per year. You stated that you use the Class DoJo app to communicate with parents. In addition, a daily report is sent home to the parents which includes any information about the child’s day and any reminders. Rated License Information: We completed and Ms. Twitty, Adminisrator signed the application for re-assessment during today’s visit, choosing the Accreditation and Head Start Pathway #3 option. Your new Five-Star License is based on the following: The license is based on the Head Start educational and programmatic requirements that are comparable or exceed NC QRIS requirements. Your program meets the Head Start Ratio. You completed a Classroom Staff to Child Ratio for Accreditation or Head Start form during today’s visit. This form is to be posted on each classroom space. The Request to use Accreditation and Head Start Licensure Pathway to Earn a Star Rated License form and the Pathway 3 Accreditation and Head Start Star Level Assessment for Child Care Centers form was completed during today’s visit. An award letter for the current budget period of August 1, 2025 through July 31, 2026 was provided. The program met the requirements for a Five-Star License. A new license will be mailed directly to you and must be posted upon receipt. At that time, return the old license to me as it remains the property of the State of North Carolina. The following violations were observed: Violation Number Comment Rule 1767 The health assessment did not include a vision screening. One child, five years of age that enrolled in NC Pre K on 8/25/25 had a vision screening on file dated 12/9/25. .3005 (a)(3) 1769 The health assessment did not include a dental screening. One child, five years of age that enrolled in NC Pre K on 8/25/25 had a dental screening on file dated 11/25/25. .3005 (a)(5) Comments: The violations observed were corrected during today’s visit. No compliance letter is required. Technical Assistance was provided on the following: A health assessment must be on file at the NC Pre-K site within 30 days after a child enters the NC Pre-K program and the assessment may be no more than 12 months old at the time of program entry. The health assessment must include the following: (1) physical examination; (2) updated immunizations; (3) vision screening; (4) hearing screening; and (5) dental screening. I suggested you review all medical exams for NC Pre-K students as soon as they are turned into the facility to ensure all required assessments, including hearing screening, vision screening, and dental screening have been completed. The violations were corrected during the visit as the vision screening dated December 9, 2025 was one file and the dental screening dated November 25, 2025 was on file. Consultation: We discussed that the family advocate and substitute staff members should be reflected on all ABCMS provide portal facility rosters for the programs they work in. The ABCMS provider portal facility roster should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Upon arrival you, Darlina Patterson, Lead Teacher, assisted me with the visit. A checklist was used to note the requirements I monitored today. Your program currently operates with a five star license effective January 11, 2019. The permit restrictions were in compliance including 1st shift, meets enhanced ratios, meets enhanced space and meets enhanced ratios minus one. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios in compliance. Children were participating in play in activity areas, group story time, handwashing protocols and lunchtime. The center's compliance history was at 96% prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A sample of two children’s files were reviewed. No new staff members have been hired since the last monitoring visit. A sample of one existing staff file was reviewed. The last monthly fire drill was practiced on 3/28/24. The last shelter-in-place drill was practiced on 3/21/24. The last monthly playground inspection was completed on 3/27/24. The most recent lead water testing results were completed on July 27, 2021. Lead testing must be completed every three years. Upon arrival you, Darlina Patterson, Lead Teacher, assisted me with the visit. A checklist was used to note the requirements I monitored today. Your program currently operates with a five star license effective January 11, 2019. The permit restrictions were in compliance including 1st shift, meets enhanced ratios, meets enhanced space and meets enhanced ratios minus one. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios in compliance. Children were participating in play in activity areas, group story time, handwashing protocols and lunchtime. The center's compliance history was at 96% prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A sample of two children’s files were reviewed. No new staff members have been hired since the last monitoring visit. A sample of one existing staff file was reviewed. The last monthly fire drill was practiced on 3/28/24. The last shelter-in-place drill was practiced on 3/21/24. The last monthly playground inspection was completed on 3/27/24. The most recent lead water testing results were completed on July 27, 2021. Lead testing must be completed every three years. The most recent sanitation inspection for your facility was conducted on April 10, 2024. A superior sanitation classification was issued with no demerits. The most recent fire inspection for your facility was conducted on March 11, 2024. The facility was approved for day time care. The following violation(s) were observed and discussed with you today: Violation Number Comment Rule 1768 The health assessment did not include a hearing screening. One child enrolled 1/4/24, did not have a hearing screening on file. .3005 (a)(4) 1769 The health assessment did not include a dental screening. One child enrolled 10/13/23, did not have a dental screening on file. .3005 (a)(5) The violation(s) documented must be corrected immediately. On or before May 7, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. If you email the compliance letter, it must be sent from the email address on file with DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Areas discussed included: Items # 1768 and #1769: 10A NCAC 09 .3005 CHILD HEALTH ASSESSMENTS (a) A health assessment shall be on file at the NC Pre-K site within 30 days after a child enters the NC Pre-K program. The assessment must include: (1) physical examination (2) updated immunizations (3) vision screening (4) hearing screening (5) dental screening. You shared that the Carver Center, who is the NC Pre K contractor, is responsible for completion of the hearing, vision and dentals screenings. The needed information was not in the children’s files. You reported that you think the screenings may have been completed and the paperwork was not sent to the school. I encouraged you to talk with the Carver Center to make sure that the needed screening paperwork is placed in the children’s files. The screenings will need to be on file within the next 2 weeks. Consultation or other topics discussed today: Per 10A NCAC 09.0304, The operator shall submit the original of the approved annual fire inspection report to the Division within one week of the inspection visit on the form provided by the Division. If the fire inspection report is not submitted to your Child Care Consultant within one week after the fire inspection is conducted, a violation will be cited during the facilities next monitoring visit. Resuming Star Rated License Reassessment Information: As a reminder your facility is in cohort 2 for resuming the star rated license reassessment. Your child care program’s prep year will begin on July 1, 2024, through June 30, 2025. Your child care program’s reassessment year will begin on July 1, 2025, through June 30, 2026. This is in addition to the existing requirement that they be free of lead poisoning hazards. In addition to the updates to this rule, 10A NCAC 41C .1001 through .1007 and 15A NCAC 18A .2816, require all child care facilities (homes and centers) to test for lead in drinking, water, identify lead-based paint hazard, and mitigate or restrict access to identified hazards. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements.
