You, Bridget Franklin, Operator, assisted me with the visit today. This facility was issued a temporary license on February 9, 2026 with a restriction of daytime care only, fireplace/woodstove not used during operating hours, and children must play in fenced in area only. The Secretary of State website was checked on March 20, 2026, and your business, Ms. Bridget’s Bright Beginnings, LLC, was active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify your consultant, at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. All designated indoor and outdoor spaces used for child care were monitored as well as the materials, equipment and required posted items for meeting all minimum licensing requirements. The children were observed during free play activities indoors, diapering changing routines, handwashing protocols, lunch, and rest time. The daily schedule and activity plans were posted. The most recent monthly fire drill was completed on February 26, 2026, at 1:45 pm. The most recent monthly outdoor inspection was completed on February 25, 2026. You completed the lead water test on December 3, 2025. You completed the lead-based paint and asbestos survey, and was determined exempt. You have a total of four (4) children enrolled during your first shift hours. There were four (4) children’s files available for review. Three children were in attendance during the visit. You do not provide transportation. We discussed the Pathways to the Stars. I suggested that you continue to review the information regarding QRIS Modernization on DCDEE’s website under “What’s New.” You reported a plan to apply for the rated license and the requirements for the Classroom and Instructional Quality Pathway. I provided you with the rated license application and applicable documents. We completed a QRIS conversation template during today’s visit. Continue to review child care rule .3206 for information regarding the requirements for the Program Assessment and the Classroom and Instructional Quality Pathways. You must complete and submit these documents to your consultant no later than May 9, 2026. You have established a DCDEE WORKS account including education evaluations completed. Documentation from Workforce on the results of the education evaluation must be on file for review. I reminded you that programs with less than a three-star license will not be eligible to receive subsidy funding. The following violations were observed today: Violation Number Comment Rule 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. On 3/18/26 the position for sleep was not completed at 8:02am, 8:10am, and 8:20am on the visually sleep check records for a five month old child that enrolled on 2/18/26. .1724(a)(8)&(f) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The ABCMS provider portal training had not been completed to establish a provider portal facility roster. G.S. 110-90.2 & .2703(r) 1893 Application did not include all required information including, but not limited to: child's full name, named to be called, child’s date of birth, any allergies, including symptoms and the type of response required, and any fears or behavior characteristics a child has. Operators shall notify the Division of all new childcare providers or household members, as defined in G.S. 110-90.2(a)(2), who are hired within five business days. During the visit, you stated you had not completed the required Moodle training to obtain a provider code needed to move forward with this requirement. To correct this violation, you must complete the Moodle training, obtain a provider code and connect all household members to the ABCMS Provider Portal roster for this facility. I suggested that you add family members to the roster within five days of their entering/leaving the household. I provided you with a copy of the ABCMS Provider Technical Assistance document during the visit. Your facility’s safe sleep policy must be followed at all times. Per rule and your facility’s safe sleep policy, sleeping infants must be visually checked at least once every fifteen (15) minutes and the check documented on a safe sleep log including the position in which the child is sleeping. I suggested you keep the sleep logs near the crib where infants sleep. A child’s application for enrollment must include the following: (A) the child's full name and the name the child is to be called; (B) the child's date of birth; (C) any allergies and the symptoms and type of response required for allergic reactions; (D) any health care needs or concerns, symptoms of and the type of response required for these health care needs or concerns; (E) fears or behavior characteristics that the child has; (F) the names of individuals to whom the operator may release the child as authorized by the person who signs the application; (G) the names and phone numbers of persons to be contacted in an emergency situation; (H) the name and phone number of the child's physician; and (I) authorization for the operator to seek emergency medical care in the parent's absence I suggested that you review all applications to ensure that all questions are answered. If a question is non-applicable, please write N/A to verify that the information was discussed with parents. Consultation was provided on the following: We discussed that the Recognizing and Responding to Suspicions of Child Maltreatment training must be completed by May 9, 2026. You can access this training at https://www.preventchildabusenc.org/services/trainings-and-professional-development/rrcourse We reviewed that you are required to follow one of the following capacity options at all times: Option 1: A maximum of eight children, with no more than five children who are from birth to 5 years of age, plus three school age children; or Option 2: A maximum of three children from birth to 24 months of age, plus three children from 2 to 5 years of age and three school age children up to 13 years of age, for a total of nine children; or Option 3: A maximum of 10 children if all children are older than 24 months of age. Health and Safety Training must be completed within one year of employment for new staff and every five years for staff that have already completed the training.
