This visit was conducted with Tosha Holt, Lead Teacher. Currently this center operates with a Five Star License. The restrictions include daytime care only and meets all enhanced requirements and reduced ratios. This facility is owned and operated by Rockingham County Schools. Sixteen (16) children were present during today’s visit. I observed the children playing indoors, playing outdoors, completing routine tasks and preparing for lunch. The facility had a fire inspection completed on October 6, 2025. I received a copy today. The sanitation inspection was completed on September 9, 2025. The facility received a superior rating with two (2) demerits. The emergency drill log was monitored during today's visit. The last fire drill recorded was completed on October 24, 2025. The evacuation took three (3) minutes. The last emergency drill, a shelter in place drill, was conducted on October 23, 2025. There are two (2) new staff. The staff have current qualifying letters but they are not listed on the staff roster in ABCMS system. They do not provide routine transportation for children enrolled this school year. The following violation was cited today. Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The two (2) new staff are not listed on the staff roster in the ABCMS system. G.S. 110-90.2 & .2703(r) Rated License: I spoke with Felicia Jumper, Head Start Director, today on the telephone to review the QRIS modernization pathways. I completed a template documenting the pathway you plan to pursue, identifying needs, documenting a timeline for completion, resources, next steps, etc. Over the next few months, I’ll be checking in with you, ahead of your next visit to check on progress and determine if additional assistance is needed with WORKS, NCRLAP/ERS, accreditation, etc. Technical Assistance: Child Care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. Provider Access to ABCMS allows administrators to: •See the real-time background check status of staff members •Run a printable report of the staff roster to assist with compliance visits •See new background check applicants and add to staff roster To get started, complete the ABCMS Child Care Provider Portal Training in Moodle. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Should you need assistance please contact the Criminal Background Check Unit at (919) 814-8401 and someone will assist you. Compliance: You must correct the violation found during today's visit immediately. Please send me a two (2) letters verifying compliance. The letters must be received by November 27, 2025. Please include in your letter each violation number and how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. Please include in your letter the name of your center and ID number, date, and the title of the person who signs the letter. The visit was conducted with Tosha Holt, Lead Teacher, by Sally Allen, Child Care Consultant. Currently this center operates with a Temporary License, issued on August 26, 2024, following a change of location. The facility is a head start classroom that has NC Pre-K children enrolled. A sampling of classrooms, staff, and children’s files were monitored during the visit. All program records and documents required to be posted were monitored. The outdoor learning environment was monitored today. The facility does not provide transportation. Meals and snacks are prepared at the facility, served, and eaten in the classrooms. Fourteen (14) children were present today. The children were engaged in outdoor play, routine tasks, group time and playing in activity areas. During today’s visit we discussed the two-component rated license. Due to this facility receiving a Temporary License after a change of location and the hold harmless still in effect, the facility will receive a Five (5) Star based on the points earned on the previous license, following the Temporary license. Your compliance history also effects the star rating. It is imperative that staff maintain compliance with child care rules and requirements at all times. Violations of child care requirements effect the facility’s compliance history. You must maintain a minimum compliance history of seventy-five (75) percent. The following violations were cited today. Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu has not been printed to post. 10A NCAC 09 .0901(b) 847 Parent's medication authorization did not include required information. There was no authorization on file for an epi-pen. 10A NCAC 09 .0803(4)(6-9) Technical Assistance and/or Consultation was provided during today’s visit on the following: You must have permission to administer any medication. The permission for medications prescribed for chronic conditions can be for six (6) months. The permission must contain all the required information. Ms. Holt was not aware that they did not have a permission form for a child’s prescribed epinephrine. She stated they may have a copy at the main head start office. We discussed the process that forms are submitted to the facility from the main head start office. The teacher asked about the staff/child ratio during tasks that take the teachers out of the classroom, such as getting the lunch trays. I informed her that the staff/child ratio must be met at all times except in emergency situations. Getting the lunch trays or clerical reasons for being out of the classroom are not emergencies. Compliance: The cited violations must be corrected immediately. A signed letter stating how the violations were corrected must be received by me by October 16, 2024. The letter may be emailed to me at sally.allen@dhhs.nc.gov or a hard copy may be sent to PO Box 1305, Eden, NC 27289. If a copy has not been sent by the 10th day an email will be sent reminding you to submit the letter. Failure to correct the violations may result in an administrative action.
