The family child care home operates with a four-star license that was issued on November 18, 2019. The last annual compliance visit was completed on June 5, 2025. Upon arrival, I was greeted by Mrs. Wilkerson, I stated the purpose of my visit, and she invited me in and assisted me during the visit. There were four children present. Mrs. Wilkerson provided me with the staff, children, and program records for my review. A full assessment of child care requirements was conducted on this date. I reviewed program records and children’s files. The provider and the additional caregiver’s file were also reviewed. The permit was posted, restrictions maintained. The summary of laws, emergency information and menus were all posted. I monitored the outdoor learning space, and information on required inspections. The following violations were observed, cited, and technical assistance. Violation Number Comment Rule 711 Outdoor stationary play equipment was not anchored and/or equipment was installed over concrete or asphalt. **A wooden climbing structure with a swing was observed not sturdy and lifting from the ground.** 10 A NCAC .1719(a)(22) 920 Records were not made available for review. The following records were not available for review: Asbestos and lead testing results, current attendance records G.S. 110-91(9) 1301 Operator did not complete the required number of on-going training hours as specified in rule. ** The provider nor the additional care givers have completed all of the required hours of training.** GS 110-91(11); 10A NCAC 09 .1705(b)(5) 1916 A shaded area was not available to children while using the outdoor learning environment. ** There was not a shaded area observed on the playground during today's visit.** .1719(25) 1933 Operator was not positioned in a manner that would maximize his/her ability to hear and see the preschool age children in care, at all times to render immediate assistance. **Three school-ag children were allowed to play outside when the provider nor the additional care givers were outside to supervise them.*** .1711(a)(1) The above violations must be corrected immediately. You must be able to see or hear school-age children and be available to render assistance as needed. They may not be inside or outside without yours or a qualified additional caregiver’s supervision. These include but are not limited to monthly fire drills (April), playground inspections (March) were not documented, Asbestos and Lead testing results must be up-to-date and available for review. Daily attendance must be maintained. Be sure to complete daily outdoor observations to ensure that the area is clean and free of hazards. You must ensure all outdoor area and equipment requirements are maintained, including but not limited to shaded areas available, equipment which must be stationary is in good repair, sturdy and safely secured. Upon arrival, I was greeted by the provider’s husband, Michael Wilkerson, who is the additional caregiver. I told him the purpose of my visit and he invited me in and assisted me during the visit. There were no children present. Mr. Wilkerson provided me with the staff, children, and program records for my review. I conducted a walk-through of the indoor and outdoor learning areas. I reviewed program records and children’s files. The provider and the additional caregiver file were also reviewed. Transportation requirements were monitored. I was not able to verify whether the provider’s auto insurance is current. You may not transport children until I receive the proof of insurance. The following violations were observed. Upon arrival, I was invited in by the provider’s husband, Michael Wilkerson. There were no children present. Mr. Wilkerson informed me Mrs. Wilkerson was out of town this week. I told him of the purpose of my visit. He provided me with the staff, children, and program records for my review. Mr. Wilkerson is the additional caregiver. Throughout the visit, Mr. Wilkerson and I communicated with Mrs. Wilkerson over the telephone. I conducted a walk-through of the indoor and outdoor learning areas. I reviewed program records and children’s files. The provider and the additional caregiver file were also reviewed. I left a copy of the File Checklist for Additional Caregivers for Mrs. Wilkerson. Transportation requirements were monitored. I was not able to verify whether the provider’s auto insurance is current. You may not transport children until I receive a proof of insurance. I observed the following violations. Violation Number Comment Rule 801 Bacteriological analysis indicating the water is free of bacteria had not been performed on a sample of water from each well used for the children's water supply every two years. Results were not kept on file in the home. The last bacteriological analysis of the well water was completed on 3/30/2022. It expired on 3/30/2024. 10A NCAC 09 .1725(a)(1) 920 Records were not made available for review. The current auto insurance information and the additional caregiver's information was not available for review other than the Criminal Background Check, ITS-SIDS, and CPR/First Aid Training. G.S. 110-91(9) 1720 Operator did not give written notice of the written plan of care (WPOC) amendment to parents of all children enrolled, at least 30 days before the change was implemented. Each parent did not sign a statement acknowledging the receipt and explanation of the amendment. The operator did not retain the acknowledgment in the child's record for as long as the child was enrolled in the home and a copy was not maintained on file for review by Division representatives. Upon arrival, I was invited in by the provider’s husband, Michael Wilkerson. There were no children present. Mr. Wilkerson informed me Mrs. Wilkerson was out of town this week. I told him of the purpose of my visit. He provided me with the staff, children, and program records for my review. Mr. Wilkerson is the additional caregiver. Throughout the visit, Mr. Wilkerson and I communicated with Mrs. Wilkerson over the telephone. I conducted a walk-through of the indoor and outdoor learning areas. I reviewed program records and children’s files. The provider and the additional caregiver file were also reviewed. I left a copy of the File Checklist for Additional Caregivers for Mrs. Wilkerson. Transportation requirements were monitored. I was not able to verify whether the provider’s auto insurance is current. You may not transport children until I receive a proof of insurance. I observed the following violations. Violation Number Comment Rule 801 Bacteriological analysis indicating the water is free of bacteria had not been performed on a sample of water from each well used for the children's water supply every two years. Results were not kept on file in the home. The last bacteriological analysis of the well water was completed on 3/30/2022. It expired on 3/30/2024. 10A NCAC 09 .1725(a)(1) 920 Records were not made available for review. The current auto insurance information and the additional caregiver's information was not available for review other than the Criminal Background Check, ITS-SIDS, and CPR/First Aid Training. G.S. 110-91(9) 1720 Operator did not give written notice of the written plan of care (WPOC) amendment to parents of all children enrolled, at least 30 days before the change was implemented. Each parent did not sign a statement acknowledging the receipt and explanation of the amendment. The operator did not retain the acknowledgment in the child's record for as long as the child was enrolled in the home and a copy was not maintained on file for review by Division representatives.
