    Name of Operation: STORIES CREEK ELEMENTARY SCHOOL AGE CARE Facility ID: 73000188 Consultant: ANITA GRAHAM Operation Type: Center Case Number: Visit Date: 1/8/2026 Number Present: 26 Completed Date: 1/8/2026 Age: From 5 To 10 Total Minutes: 78 Time In: 02:52 PM Time Out: 04:10 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance with applicable requirements during the Annual Compliance visit. The last Annual Compliance visit was conducted on January 17, 2025. This facility operates with a Four (4) Star License which was issued on October 31, 2019. Upon my arrival to the facility, I signed in at the front office and proceeded to the after-school classroom, which is the MP/cafeteria. I was greeted by Ms. Lynn Bowes, the assistant Group Leader. I stated the purpose of my visit. Ms. Bowes informed me that the Group Leader, Ms. Humphries, was on her way to the classroom. The Group Leader did arrive within fifteen (15) minutes of my arrival. Both teachers assisted me during the visit. Attendance was taken and observations made. The children were observed having snacks participating in routines gross motor activities in the gym. I conducted a walkthrough of the classroom. The last Fre Inspection was conducted on April 21, 2025, and was approved for day time care only. The last Sanitation Inspection was conducted on December 17, 2025. The program achieved a ‘Superior’ classification. A full assessment of child care requirements was conducted on this date. This facility has one School-Age classroom. The permit was posted. I monitored supervision, staff/child ratio, and general safety. I monitored program records, a sampling of children’s records and the group leaders’ files. The following violation was observed, cited, and technical assistance was provided. Violation Number Comment Rule 107 The center did not comply with the permit restrictions. **During today’s visit one group leader was observed caring for twenty-six (26) school-agers ranging in age from five-years-old up to age ten-years old. The group leader did arrive within fifteen minutes of my arrival to the classroom. GS 110-91; GS 110-106 The above violation must be corrected immediately, and corrective action letter must be sent to my office stating how and when compliance with this rule was achieved. The letter should be email on/or before January 28, 2026. The letter may be emailed to my supervisor Kaye Adkins at kaye.adkins@dhhs.nc.gov and please ‘cc’ me at anita.c.graham@dhhs.nc.gov. If your letter is not received by the due date, a follow-up visit may be necessary to collect the corrective action letter and to ensure compliance with childcare requirements. Please be advised that additional violations may be cited, if observed during that visit. TECHNICAL ASSISTANCE: During the visit today, I showed the group leader the restriction for their permit which indicates that the facility has chosen to meet enhanced ratios and space requirements. During today’s visit one group leader was observed caring for twenty-six (26) school-agers ranging in age from a five-year old up to age ten-years old. Your staff/ratio must be maintained at one staff member to fifteen (15) children when a five-year-old is present and one staff member to nineteen (19) children when the children are six years old and older. Please remember that it is your responsibility to be abreast of all child care rules and requirements and to always maintain compliance with those rules.
