This family child care home is operating with a Three (3) Star Rated License, which was issued May 8, 2024. The last annual compliance visit occurred May 5, 2025. Upon arrival to the home Ms. Springfield, the owner, greeted me and invited me in. I stated the purpose of my visit. Together, we conducted a walkthrough of the child care home. There is one preschool child present. The child was observed watching television, having snack, participating in routine activities, such as toileting and handwashing. A partial assessment of the child care requirements was completed today. I monitored program and space requirements, the provider’s training records, capacity and supervision, and general safety. The following violations were observed, cited and technical assistance was provided. Violation Number Comment Rule 705 All medicines, including refrigerated and unrefrigerated, were not kept in locked storage. **The providers personal medications were observed in an opened door bedroom and accessible to the child in care. .1719(a)(5) 716 Electrical outlets not in use were not covered. An electrical outlet in the room used to care for the child was observed uncovered. 10A NCAC .1719(a)(27) 729 Appliances with heating elements, such as bottle warmers, crock pots, curling irons, and electrical cords were accessible to children. A crock-pot was observed in the floor of the kitchen. 10A NCAC .1719(a)(28) 813 House was not free of rodents (untamed mice, rats, squirrels, etc.). **Rodent droppings were observed on the dinning table, on the television table for which the television viewed by the child. Rodent droppings were also observed in the EPR binder the consultant requested to view. The provider was observed brushing the "droppings" on to the floor of the same room the child was being cared for. 10A NCAC 09 .1725(a)(11) 929 Developmentally appropriate activities for the ages of children in care, were not provided as planned on the written schedule and activity plan. Activity plans observed were documented to be from another child care facility and a school-age activity plan was also present. Neither, of the plans were age-appropriate for the child enrolled and in care during today's visit. G.S.110-91(12) & .1718(a)(6) 1853 The operator did not conduct a monthly fire drill. The last fire drill documented was August 2025. .1719(a)(15) & .1721( e)(2) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. Documentation posted showed the last lock-down drill was conducted June 2025. .1719(a )(16) & .1721(e )(7) 1917 Children were not cared for in space designated as the caregiving area on the floor plan provided by the operator. The child was being cared for the personal living space of the home. .1707(7) 1919 A First Aid Information sheet, complete with required information, was not posted for quick referral by staff. .1719(a )(14) 1932 A clean and open area that allows freedom of movement was not provided for children, both indoors and outdoors. The classroom designated as the care space was filled with clutter and appeared to be used for storage. The provider stated she is not currently using the space for care. .1718(a)(9) 1968 Screen time was not limited to a maximum of thirty minutes per day, and no more than two and a half hours per week, per child. Upon my arrival the child in care was observed watching television from 10:05am and periodically throughout my visit. If the letter has not been received by my office by 5:00pm, on November 7, 2025, a return visit may be necessary to ensure compliance with childcare requirements. Therefore, it is important that you are knowledgeable of all the licensing requirements which apply to your program and to always maintain compliance with child care laws. The best practice is to periodically review the childcare laws. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/, on the home page under the ‘Services’ tab. TECHNICAL ASSISTANCE: Several of the violations cited today are repeat records violations. Please be advised that repeat violations may result in administrative actions which may result in civil penalties and, up to revocation of your child care license. The provider must address sanitation and pest control concerns immediately by removing rodent droppings from the facility and you must store rodent bait spaces inaccessible to children. The provider must secure all hazardous materials including aerosol spray cans and cleaning products in to locked storage. Protective covers shall be on all electrical outlets within child accessible areas. The providers own medications must also be kept in locked storage and inaccessible to the children. The provider must maintain an organized clean and hazard free facility. All clutter should be removed from all spaces were some level of care is provided for the children. The rugs must be vacuumed, cleaned and maintained for a safe sanitary environment. The provider should ensure that fire and shelter in place drills are conducted and documented as required by child care law. Provider must maintain daily attendance records and age-appropriate activity plans based on childcare requirements. The activity plans should be appropriate for the age of the children enrolled in your program. Currently there is one four-year-old child enrolled in your program. Therefore, school age activity plans are not age appropriate. The provider should limit screen time to 30 minutes per day and document accordingly for children who are three years old and up. Screen time should be used to meet developmental domains and should be documented on the activity log. The provider must use the designated space identified in the floor plan. The space should be cleaned and organized and have free walking space for safe navigation throughout the learning space. Designated centers should be identifiable. The visit summary was reviewed with the provider, Ms. Elizabeth Springfield and a copy will be emailed to her. Upon arrival, I was greeted by Ms. Springfield. I completed a general walk through of the indoor and outdoor environment. There was one (1) preschool child present during the visit. The child was observed sitting on the couch with the provider upon arrival. INSPECTIONS *The last annual compliance visit was conducted on May 8, 2024. *The last shelter-in-place drill was last completed on 2/2/25. DESIGNATED CHILDREN’S SPACE During today's walk-through it was very hard to determine where the child care space was due to the amount of items observed in the living room and in the three bedrooms located towards the back of the home. The bathroom is located down the hallway to the left and the kitchen is located next to the dinning room. During my observation today the kitchen was very cluttered and I had concerns regarding if the stove was operating correctly to cook food. It appears that activities take place with the one child present in care in the living room. *The outdoor play area is in the front of the home. *The provider informed me of the following: -There are no infants enrolled. -Medication is not being administered to any of the children. -Transportation is not provided at this time. -There have not been any incident/accident reports written. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. The following violations were cited: Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. DESIGNATED CHILDREN’S SPACE During today's walk-through it was very hard to determine where the child care space was due to the amount of items observed in the living room and in the three bedrooms located towards the back of the home. The bathroom is located down the hallway to the left and the kitchen is located next to the dinning room. During my observation today the kitchen was very cluttered and I had concerns regarding if the stove was operating correctly to cook food. It appears that activities take place with the one child present in care in the living room. *The outdoor play area is in the front of the home. *The provider informed me of the following: -There are no infants enrolled. -Medication is not being administered to any of the children. -Transportation is not provided at this time. -There have not been any incident/accident reports written. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Upon arrival, I was in by the provider, Elizabeth Springfield. She was caring for one child. No infant child was enrolled. I completed a walkthrough of the indoor and outdoor learning environment. I observed the child playing with the toys and materials, being held and comforted by the provider, having the diaper changed, and going outside. I observed the following violations today. "The provider's five year criminal back ground check expired 4/25/2024." G.S. 110-90.2 & .2703(n) & (o) 1301 Operator did not complete the required number of on-going training hours as specified in rule. "The provider completed three hours out of eight required annual on-going training hours." GS 110-91(11); 10A NCAC 09 .1705(b)(5) 1306 Operator has not completed ITS-SIDS training every three years from the completion date of the previous ITS-SIDS training. "The provider did not complete the ITS-SIDS training every three years." .1703(a)(4) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. "There was no record of conducting a shelter-in or lockdown drills for over a year." .1719(a )(16) & .1721(e )(7) 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. "The provider has not completed a Health and Safety Training since 2017." .1703(d)(2) During today's visit, Ms. Springfield called the Person Co. Partnership to verify she took the ITS-SIDS training and the Health and Safety training. The person on the phone did not see a record where the provider completed the two training. She told us the specialist who could verify was not in and won’t return until the end of this week. The above violations must be corrected immediately. Technical Assistance >I discussed with Ms. Springfield a possible administrative action as a consequence for repeat violations. >Criminal Background Check (CBC) is good for five years. Your CBC expired 4/25/2024. Rule # The process for the Criminal Background Check has changed. The system you’d access is now called ABCMS. Applicants will log into ABCMS using their NCID and password, make application, pay the $26.50 fee for the background check and print the fingerprint release form to take to their local law enforcement office to be fingerprinted. You can also learn more about the background check by accessing the website, https://ncabcms.nc.gov/DCDEE/Home. Providers are encouraged to complete the Moodle training first regarding the check. This is child care rule .2703. Once results have been received and the fingerprints processed by the Division of Child Development and Early Education Criminal Background Check Unit, the applicant will be notified by email that they can then log back into ABCMS using their NCID and password to see the results. Approved applicants can print their qualification letter directly from ABCMS. Health and Safety Training hours count toward the required annual on-going training hours. You may access the HS Training on the Division’s website, https://www.dcdee.moodle.nc.gov/. This is child care rule .1703 (b) (d). >Emergency lock down or shelter in drill must be completed every 3 months and recorded. This is child care rule .1719 and .1721. >On-going training hours you are required to take annually is 8 hours. Prorated monthly, you should be taking 0.67 hours each month. This is chilld care rule .1703(d). Other Information Weighted blankets or vests: 1.Blankets of any type are prohibited for children aged 0 to 12 months, per .0606(a)(2) Safe Sleep Policy. The current child care rules specifically state no blankets or other items can be used/placed with a sleeping infant aged 12 months or younger. 2.Weighted blankets or vests can potentially restrict a child’s movement; therefore, they are prohibited for use unless a therapist requests and approves the use. A therapist can only approve the use for children 13 months or older. 3.If the use of a weighted blanket or vest is something a facility wants to consider using, you’d need to contact me prior to use to ensure compliance with applicable requirements. >It is the childcare operator’s responsibility to be familiar and up to date on the most current child are rules. Child Care Rulebook as well as the Provider Forms and Documents can be accessed by going to the DCDEE Website, https://ncchildcare.ncdhhs.gov/. Childcare Rules may be accessed by going to the Provider tab and hover over it. In the drop-down box Click on Child Care Rules. Scroll down to North Carolina Child Care Rules and Law section. Click on Chapter 9- Child Care Rules. >Be sure the forms/documents you are using are the most current versions which align with the current childcare rules. Visit the DCDEE website above to access the Provider Forms and Documents.
