Name of Operation: SOUTH ELEMENTARY SCHOOL AGE CARE Facility ID: 73000184 Consultant: MONA NDIAYE Operation Type: Center Case Number: Visit Date: 2/18/2025 Number Present: 16 Completed Date: 2/18/2025 Age: From 5 To 11 Total Minutes: 60 Time In: 03:10 PM Time Out: 04:10 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor for applicable child care requirements. The facility currently operates with a Five Star License that was issued on January 27,2020. The last annual compliance visit was conducted on August 29, 2024. The Group Leaders, were present and assisted me during the visit. Children were observed participating in free play in the cafeteria, the Group Leaders were observed moving about the space interacting with the children and supervising the children. The children were observed doing bathroom routines and eating snack. The last sanitation inspection was completed December 12, 2024 and earned a “Superior” classification. During today’s visit, a partial assessment of applicable child care requirements was conducted. I observed the following violation. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). The Program Coordinator's criminal background check expired 8/330/2024. G.S. 110-90.2(b) & .2703(n)&(o) The above violations must be corrected immediately, and a letter must be sent to me no later than March 4, 2025 stating how each item was corrected and a plan to maintain compliance with the requirement. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation and your plan to keep each item in compliance. My email address is mona.ndiaye@dhhs.nc.gov. If I do not receive your letter by November 18, 2024 and/or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the violations have not been corrected or addressed. You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/ , on the home page under the ‘Services’ tab. Technical Assistance The Program Coordinator was not present during today’s visit. I emailed her during today’s visit and she responded stating she did not have a valid/current criminal background check, but was in the process of getting one. Her criminal background check expired 8/30/2024. RESOURCES The Health and Safety Trainings are offered on the DCDEE Moodle format. On the Division’s website look under the “SERVICES” tab for “DCDEE Moodle”. These trainings are free, and participants will receive training certificates when completed. Child care programs can order free posters from the NC Child Care Health and Safety Resource Center at https://healthychildcare.unc.edu/ .
