The last annual compliance visit was conducted on August 20, 2025. The facility operates with a GS-110 Notice of Compliance issued August 14, 2020. Upon arrival to the facility, I was greeted by a staff member who escorted me to the classroom. The center director, Deborah West, was present and assisted me during today’s. I conducted a walk-through of the classroom. There are two preschool classrooms at this site. Only one of them is in use currently. In K-4 classroom, attendance was taken and observations made. The children were observed participating in teacher directed activities, routines, as well as an indoor gross motor activity. The last Fire Inspection was conducted on April 10, 2026. The last Sanitation Inspection was completed April 14, 2026, with a ‘Superior’ classification and 0 demerits. A partial assessment of childcare requirements was conducted today. This includes, but is not limited to, observing the license posted and restrictions maintained. Also posted were the summary of laws, emergency contact number, and the emergency care plan. I monitored staff training to include CPR/FA, Emergency Preparedness, and Health and Safety training. The following violation was documented and technical assistance was offered. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair.**A pole securing a canvas used for shading, is leaning and the concrete footing is exposed.** G.S. 110-91(6); .0601(b) You were provided with an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. The violation cited during the visit was discussed with Ms. West and Ms. McDowell, the school’s Principal. This must be corrected immediately, and a signed and dated letter of compliance must be received on or before May 12, 2026. The letter must include the *Facility Name and ID number, the violation item number, and how and when the violation was corrected. Also, please attached documentation need to demonstrate compliance. Please submit the letter to my email at anita.c.graham@dhhs.nc.gov. TECHNICAL ASSISTANCE: All playground structures must be made secure. A pole holding up the canvas shade structure appears leaning. This may pose a safety issue for the children while in outdoor play. The last annual compliance visit was conducted 9/10/24. Currently this program operates with a GS110 Notice of Compliance issued 8/14/20. The center director, Deborah West, assisted me today. I conducted an unassisted walk-through of the classrooms. The last sanitation inspection was completed 10/18/24 with a ‘Superior’ classification and 0 demerits. The last fire inspection was conducted on 3/5/24. Children were observed playing in activity areas, large group activities, and routine care activities. There were no new staff members. A partial assessment of childcare requirements was conducted today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was completed 3/5/24. 10A NCAC 09 .0304(a) 805 Fire drills were not practiced monthly and/or the drill record was incomplete. A fire drill was not conducted in September 2025. .0604(t); .0302(d)(5) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Disinfecting wipes and an aerosol disinfectant were not in locked storage in room 9. The cabinet containing cleaners was unlocked in room 9. The cabinet in room 7 containing disinfecting wipes and aerosol disinfectant was unlocked. .2820(b) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. A lockdown drill was conducted in September 2024 and February 2025. These drills were not recorded on the drill log. A drill was not conducted every three months. .0604(u);.0302(d)(8) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Violations cited during the visit were discussed with Ms. West. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 4/7/25. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All items must be included, or the corrective action letter will be returned for corrections. All requested documentation must accompany the Corrective Action Letter. The letter can be emailed to lori.rierson@dhhs.nc.gov. CONSULTATION AND RESOURCES: An incident report for a child requiring additional medical treatment was completed on 3/21/25. Once the parent signs the form today, please email to me. In the email, include the type of treatment the child received. A fire inspection must be completed within 364 days of the previous year’s inspection. For example, if an inspection is completed March 15 then the next year's inspection must be completed by March 14. We reviewed items that are required to be placed in locked storage. An item that only have the warning ‘Keep out of reach of children’ and no other warnings can be stored up five feet and out of reach of children. If the item has any additional warnings, it must be in locked storage. An item that is in an aerosol can, like disinfectant or sunscreen, must be in locked storage. If the school does not practice shelter-in-place or lockdown drill every three months, you can practice one independently for your two preschool classrooms. Name of Operation: ROXBORO CHRISTIAN ACADEMY Facility ID: 73000214 Consultant: BOON TROTT Operation Type: Center Case Number: Visit Date: 9/19/2023 Number Present: 17 Completed Date: 9/19/2023 Age: From 3 To 5 Total Minutes: 160 Time In: 09:40 AM Time Out: 12:20 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance with applicable childcare requirements during an Annual Compliance Visit. Upon arrival, I signed in at the school office. I was greeted by the director/ lead teacher, Deborah West. This program is operating with a Notice of Compliance. The operator completed the last fire inspection on 10/18/2022. The last sanitation inspection was completed on 5/18/2023 with a superior rating. I conducted a walkthrough of the indoor and outdoor of the facility. I observed the children playing, lacing, making, drawing, playing in the outdoor learning environment, and being read to during a group time. Ms. West assisted me by gathering the documents and files I needed to review. Some staff records were maintained in the school office. The school office staff member assisted me with the records. I observed the following violations today. I reviewed the items with Ms. West and provided technical assistance. Violation Number Comment Rule 319 Staff/child ratios applicable to a classroom, were not posted in each classroom. No staff/child ratio sheet was posted in the 3 year-old classroom. .0713(a)(10), (c) & (f)(3); .2818(e) 811 Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. In the playground, I observed a child holding a bottle of bubbles and blowing bubbles. In the 4 and 5 year old classroom, I observed bottles of soapy water, sanitizing solution, and disinfectant wipes within reach of children. This was corrected by storing them out of reach when it was brought to the teacher's attention. .0604(a) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. One staff member did not have the medical record and TB test or screening questionnaire on file. .0701(d) The above violations must be corrected immediately. I must receive a compliance statement/ letter by email or mail by 10/3/2023. Include the following in the letter: 1. Your facility name and ID# 2. Visit date 3. Violation item number 4. NC GS 110-90(4)(d) requires all childcare facilities to maintain compliance history score of at least 75% for the past 18 months or during the length of time the facility operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Technical Assistance >Potentially hazardous items must be inaccessible to children. Do a quick check of the playground daily and remove any potentially hazardous items before the children play in it. Inaccessible to children means storing at least 5 feet off the floor. >TB result may be obtained either by a health care provider completing a skin test or screening. The form containing a series of screening questions is on the DCDEE website, https://ncchildcare.ncdhhs.gov/. >Medical record and TB test or screening questionnaire must be on file prior to the first day of employment. >Sanitation Rules update: Children’s Environmental Health has set up virtual training for childcare center operators on the newly readopted childcare sanitation rules. The dates for the training are September 20, 2023 and September 26, 2023; both scheduled from 9:00 am – 11:30 am. These training courses are identical, so only one will need to be attended. > It is the childcare operator’s responsibility to be familiar and up to date on the most current child are rules. Child Care Rulebook as well as the Provider Forms and Documents can be accessed by going to the DCDEE Website, https://ncchildcare.ncdhhs.gov/. For childcare Rules, go to the Provider tab and hover over it. In the drop-down box Click on Child Care Rules. Scroll down to the North Carolina Child Care Rules and Law section. Click on Chapter 9- Child Care Rules. >Be sure the forms/documents you are using are the most current versions which align with the current childcare rules. Visit the DCDEE website above to access the Provider Forms and Documents. > Fire inspection report- Child Care Rule .0304 (a) states: Each operator shall schedule and obtain a fire inspection within 12 months of the center's previous fire inspection. The operator shall notify the local fire inspector when it is time for the center's annual fire inspection. The operator shall submit the original of the approved annual fire inspection report to the Division within one week of the inspection visit on the form provided by the Division.
