The program currently operates with a Two-Star Child-Care License that was issued on July 30, 2024. You, Ms. R. Oakley, assisted me with the visit. The last Annual Compliance Visit was conducted on June 11th, 2024. The home's compliance history was reviewed with the operator. The program’s compliance history was ninety seven percent (987%) as of 6/4/25. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed four (4) children in the outdoor learning environment and found supervision and staff/child ratios to be in compliance. The caregiver was interacting and meeting the developmental needs of the children. The following items were observed in compliance during today's visit: license posted, and restrictions followed, summary of law posted, emergency medical care plan, emergency numbers posted, EPR Plan, first aid poster posted, daily schedule, storage of hazardous materials and substances, CPR/First Aid and SIDS training completed as required, and Criminal Record Checks completed as required. The last fire drill was conducted on 6/2/25. The last shelter-in-place and lock down drills were conducted: 4/3/25. The last Outdoor inspection was completed on 5/2025. All staff and children’s files were monitored during today’s visit. A checklist was used to note the requirements I monitored today. One violation was observed. The provider did ask questions pertaining to staying in compliance and technical assistance and consultation information was given. Violation Number Comment Rule 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file. Two children L. Strader, and J. Harris did not have immunization records on file during today's visit. GS 110-91(1); .1721(a)(2) The violation(s) documented must be corrected immediately. On or before 6-18-25, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Each center shall establish procedures for pick-up and delivery of children to ensure children are protected from danger and not exposed to risk of harm. These procedures shall be communicated to parents, and a copy shall be posted in the center where they can be seen by the parents. A First Aid kit and fire extinguisher shall be located in each vehicle used to transport children. The First Aid kit and fire extinguisher shall be mounted or secured if kept in the passenger compartment. For each child being transported, identifying information, including the child's name, photograph, emergency contact information, and a copy of the emergency medical care information form required by Rule .0802(c) of this Chapter, shall be in the vehicle. The driver shall: (1) be 21 years old or a licensed bus driver; (2) have a valid driver's license of the type required under North Carolina Motor Vehicle Law for the vehicle being driven or comparable license from the state in which the driver resides; and (3) have no convictions of Driving While Impaired (DWI) or any other impaired driving offense within the previous three years. Each person in the vehicle shall be seated in the manufacturer's designated areas. No child shall ride in the load carrying area or floor of a vehicle. Children shall not be left in a vehicle unattended by an adult. Children shall be loaded and unloaded from curbside or in a safe, off-street area, out of the flow of traffic, so that they are protected from all traffic hazards. Before children are transported, written permission from a parent shall be obtained that shall include when and where the child is to be transported, expected time of departure and arrival, and the transportation provider. Parents may give standing permission, valid for up to 12 months, for transport of children to and from the center not including off premise activities as described in Rule .1005 of this Section. When children are transported, staff in each vehicle shall have a functioning cellular telephone or other functioning two-way voice communication device. Staff shall not use cellular telephones or other functioning two- way voice communication devices except in the case of an emergency and only when the vehicle is parked in a safe location. For routine transport of children to and from the center, staff shall have a list of the children being transported. Staff members shall use this list to document attendance as children board the vehicle and as they depart the vehicle. The plan shall: (1) document the individual's professional development goals; (2) be appropriate for the ages of children in their care; (3) include the continuing education, coursework or training needed to meet the individual's planned goals; (4) be completed by the operator and staff member in a collaborative manner; and (5) be maintained in their personnel file. Another form may be used other than the sample templates provided by the Division as long as the form includes the information set forth in this Rule. The program currently operates with a Two-Star Child-Care License that was issued on July 30, 2024. You, Ms. R. Oakley, assisted me with the visit. The last Annual Compliance Visit was conducted on June 11th, 2024. The home's compliance history was reviewed with the operator. The program’s compliance history was ninety seven percent (987%) as of 6/4/25. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed four (4) children in the outdoor learning environment and found supervision and staff/child ratios to be in compliance. The caregiver was interacting and meeting the developmental needs of the children. The following items were observed in compliance during today's visit: license posted, and restrictions followed, summary of law posted, emergency medical care plan, emergency numbers posted, EPR Plan, first aid poster posted, daily schedule, storage of hazardous materials and substances, CPR/First Aid and SIDS training completed as required, and Criminal Record Checks completed as required. The last fire drill was conducted on 6/2/25. The last shelter-in-place and lock down drills were conducted: 4/3/25. The last Outdoor inspection was completed on 5/2025. All staff and children’s files were monitored during today’s visit. A checklist was used to note the requirements I monitored today. One violation was observed. The provider did ask questions pertaining to staying in compliance and technical assistance and consultation information was given. Each center shall establish procedures for pick-up and delivery of children to ensure children are protected from danger and not exposed to risk of harm. These procedures shall be communicated to parents, and a copy shall be posted in the center where they can be seen by the parents. A First Aid kit and fire extinguisher shall be located in each vehicle used to transport children. The First Aid kit and fire extinguisher shall be mounted or secured if kept in the passenger compartment. For each child being transported, identifying information, including the child's name, photograph, emergency contact information, and a copy of the emergency medical care information form required by Rule .0802(c) of this Chapter, shall be in the vehicle. The driver shall: (1) be 21 years old or a licensed bus driver; (2) have a valid driver's license of the type required under North Carolina Motor Vehicle Law for the vehicle being driven or comparable license from the state in which the driver resides; and (3) have no convictions of Driving While Impaired (DWI) or any other impaired driving offense within the previous three years. Each person in the vehicle shall be seated in the manufacturer's designated areas. No child shall ride in the load carrying area or floor of a vehicle. Children shall not be left in a vehicle unattended by an adult. Children shall be loaded and unloaded from curbside or in a safe, off-street area, out of the flow of traffic, so that they are protected from all traffic hazards. Before children are transported, written permission from a parent shall be obtained that shall include when and where the child is to be transported, expected time of departure and arrival, and the transportation provider. Parents may give standing permission, valid for up to 12 months, for transport of children to and from the center not including off premise activities as described in Rule .1005 of this Section. When children are transported, staff in each vehicle shall have a functioning cellular telephone or other functioning two-way voice communication device. Staff shall not use cellular telephones or other functioning two- way voice communication devices except in the case of an emergency and only when the vehicle is parked in a safe location. For routine transport of children to and from the center, staff shall have a list of the children being transported. Staff members shall use this list to document attendance as children board the vehicle and as they depart the vehicle. The family child care home operator and staff members shall complete a professional development plan within one year of employment and at least thereafter. The plan shall: (1) document the individual's professional development goals; (2) be appropriate for the ages of children in their care; (3) include the continuing education, coursework or training needed to meet the individual's planned goals; (4) be completed by the operator and staff member in a collaborative manner; and (5) be maintained in their personnel file.
