Name of Operation: PRECIOUS ANGELS HOME DAY CARE LLC Facility ID: 73000229 Consultant: ANITA GRAHAM Operation Type: Family CC Home Case Number: Visit Date: 4/1/2026 Number Present: 5 Completed Date: 4/1/2026 Age: From 1 To 3 Total Minutes: 192 Time In: 11:09 AM Time Out: 02:21 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp w/Rated Lic Assess Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to monitor child care requirements applicable to this Four Star (4) Family Child Care home and to conduct an Annual Compliance/Rated License visit. The last Annual Compliance Visit was on April 9, 2025. Upon my arrival I was greeted by the owner/ operator, Mrs. Angel Clay, who assisted me during today’s visit. I stated the purpose of my visit. Attendance was taken, and observations were made. I observed the children engaged in indoor free play, routine care and having lunch and preparing for resting. A full assessment of child care requirements during the visit. I monitored program requirements. I observed the license posted and restrictions maintained. I observed the written plan of care, emergency numbers, emergency drills, and playground inspections completed. The summary of laws was posted as were the first aid posters. I monitored the provider’s file and a sample of children’s records. The following violations were observed cited, and technical assistance was provided. Violation Number Comment Rule 508 Operator did not successfully complete a first aid course as referenced in Rule.1702(b)(2) First aid training was not renewed on or before the expiration of the certification. **The training certificates reviewed had an expiration dated of 2/2026.** .1703(a)(2) 511 Operator did not successfully complete a CPR course as referenced in Rule.1702(b)(2) CPR training was not renewed on or before the expiration of the certification. **The training certificates reviewed had an expiration dated of 2/2026.** .1703(a)(3) 701 All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. *Various chemicals used for outdoor maintenance were observed stored on the ground outside of the storage building and not in locked storage containers.** .1719(a)(1)&(17) 716 Electrical outlets not in use were not covered. **Several electrical outlets which were accessible to the children were observed uncovered. Two of these were observed in the area where the children were resting.** 10A NCAC .1719(a)(27) 801 Bacteriological analysis indicating the water is free of bacteria had not been performed on a sample of water from each well used for the children's water supply every two years. Results were not kept on file in the home. *Documentation on file indicated the two-year water testing was due March 24, 2026.** 10A NCAC 09 .1725(a)(1) The above items of non-compliance must be corrected immediately, and a corrective action letter must be mailed to me by April 15, 2026. The letter must address each violation; how and when they were corrected. If the letter does not reach my office by April 15th, a follow-up visit may be required to ensure that compliance has been attained or to collect the compliance letter. TECHNICAL ASSISTANCE: 10A NCAC 09 .1725 SANITATION REQUIREMENTS FOR FAMILY CHILD CARE HOMES (a) To assure the health of children through proper sanitation, the family child care home operator must collect, and submit samples of water from each well used for the children's water supply for bacteriological analysis to the local health department or a laboratory certified to analyze drinking water for public water supplies by the North Carolina State Laboratory of Public Health every two years. Results of the analysis shall be on file in the home. Please be reminded that CPR/First Aid training must be renewed every two years prior to the end of the expiration month. It’s important to keep all electrical sockets which are accessible to children, equipped with safety plugs. All chemicals with two or more warnings shall be maintained in locked storage. This is a requirement for indoors as well as, outdoor learning spaces. RATED LICENSE: An overview of QRIS section .3200 of the child care rules was provided. The pathway options discussed, and the education requirements to maintain the current Four (4) Star status. You have stated that your goal during the Rated License year will be to obtain a Five (5) Star License. Based on today’s conversation/discussion and the explanation of pathways, the provider has chosen to pursue Pathway #2, RESOURCES: Using NCRLAP website is just one of the ways to ensure that your approach to a quality program and to document your three (3) month Self-Assessment monitoring. See the attached link, https://www.ncrlap.org/. There are training modules on the Division of Child Development website under Provider-Professional development.
