Name of Operation: NURTURING COVE HOME DAYCARE Facility ID: 73000254 Consultant: MARCIA HUMPHREY Operation Type: Family CC Home Case Number: Visit Date: 3/10/2025 Number Present: 2 Completed Date: 3/10/2025 Age: From 2 To 4 Total Minutes: 135 Time In: 10:15 AM Time Out: 12:30 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to complete the second temporary time period visit. The provider Mrs. Christy Stephens was present and assisted me during today's visit. I observed two children present upon our arrival involved in different free play and art activities with the provider and her daughter. The temporary license was issued to this family child care home on 10/17/24 and expires on 4/17/25. The Verification of Required Information for Operator and Additional Caregiver form was completed during today's visit. The last fire drill was completed on 2/25/25. The last outdoor play inspection was completed on 2/25/25. In addition, the quarterly shelter-n-place was completed on 2/25/25. The provider informed me during today's visit that she is in the process of signing up to complete EPR training with her local resource and referral agency. Once completed, I informed Mrs. Stephens that the EPR plan needs to be completed in the risk management portal within four months after the completion of the training. The provider does provide transportation and this was approved during the last visit. The license was posted along with the NC Summary of the Law, Emergency Number and Emergency Procedures. A first aid reference sheet was available. The daily schedule and activity plan were also posted and available for review. Attendance records have been maintained since opening. Keep this information on file for at least one year. COMPLIANCE HISTORY: Prior to today’s visit, the 18-month compliance history for this facility was 100%. The following violations were cited: Violation Number Comment Rule 729 Appliances with heating elements, such as bottle warmers, crock pots, curling irons, and electrical cords were accessible to children. A small space heater was accessible to the children in the child care space upon arrival. 10A NCAC .1719(a)(28) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. This was not available for review in two children's files during today's visit. GS 110-91(1); 10A NCAC 09.1721(a)(1) 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file. This was not available for review in one child's file during today's visit. GS 110-91(1); .1721(a)(2) Each violation cited was discussed with the Mrs. Stephens in detail. Mrs. Stephens was provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or 4/24/25. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All requested documentation must accompany the Corrective Action Letter. The letter can be emailed to marcia.humphrey@dhhs.nc.gov. TECHNICAL ASSISTANCE: I discussed with the provider the importance of making sure that all medical information has been received from the parent(s)/guardian(S) with-in the required thirty day timeframe. Mrs. Stephens stated that she will make sure to stay on top of this for future enrolling children. In addition, I informed Mrs. Stephens that she need to complete her health and safety trainings and professional development plan before 10/17/25. TECHNICAL ASSISTANCE: I discussed the Environmental Rating Scale (ERS) and the rated license process with Mrs. Stephens during today's visit and she informed us that she does not want to have the ERS completed at this time. I reviewed the education information that is currently in WORKS and verified that Mrs. Stephens has her AAS-Degree in Early Childhood Education. Mrs. Stephens did complete the 3 months reflective-self study process and it was reviewed and discussed with the provider. Based on the completion of this reflective self-reflective study this program will receive 2 points in Program Standards. In regards to the Quality Point, Ms. Sampson has decided to choose the option of serving NMT two infants under one year of age. I informed Mrs. Stephens that based on the following it appears that her family child care home will have the following: *Education Component-7 points (A.A.S in ECE + 21 years of experience) *Program Standards-2 points (completed reflective-self documentation has been received) *Quality-1 point (serving NMT two infants under one year of age) Total: 10 points and 4 Stars in the two component system If you have any additional questions or concerns I can be reached at (919) 259-5860 or by email at marcia.humphrey@dhhs.nc.gov.
