    Name of Operation: NORTH END ELEMENTARY SCHOOL AGE CARE Facility ID: 73000186 Consultant: BOON TROTT Operation Type: Center Case Number: Visit Date: 10/3/2023 Number Present: 15 Completed Date: 10/3/2023 Age: From 5 To 9 Total Minutes: 110 Time In: 02:40 PM Time Out: 04:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements during a Routine Unannounced Visit at this DPI afterschool program. I checked in at the school office when I arrived. I informed the site director, Rhonda Blackwell, of the purpose of my visit. Ms. Blackwell assisted me with the records. The last annual compliance visit was completed on 3/6/2022. The last fire inspection was completed on 9/27/2022 and the sanitation inspection on 5/30/2023. I conducted a partial assessment of the child care requirements today. I completed a walkthrough of the indoor and outdoor learning environment. I observed the children arriving, washing hands, having a snack, working on an art project, and departing with parents. There were no new staff files to review. I observed one violation during today’s visit. Violation Number Comment Rule 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. "Since the school started at the end of August, the operator has not completed any playground inspection." .0605(q) The above violation must be corrected immediately. I must receive a compliance statement/ letter by email or mail by 10/17/2023. In the letter, include the following information: 1. Your facility ID# and Name 2. Visit date 3. Violation item number(s) 4. How and when the violation was corrected (Be specific) 5. Reminder >Conduct a quarterly emergency drill by November 28. >Playground inspection must be completed monthly. Technical Assistance >Reach out to the fire marshal for a fire inspection. It’s time for an annual inspection. >Email me the fire inspection report within 7 days of receiving it per child care rule 10A NCAC 09 .0304. >Rated License process was discussed. Your facility is in Cohort 2, meaning year 2024-2025 is a preparation year and the assessment of the rated license begins in year 2025. >If applicable, submit any new education information to WORKS in a timely manner. >If you plan to have the School Age Environment Rating Scale, SACERS, completed, visit www.ncrlap.org to access the training videos, frequently asked questions, additional notes, and more.
