This program is operating with a Three (3) Star Rated license, which was issued on January 16, 2018. The last annual compliance visit was conducted on June 5, 2025. Upon my arrival, to the facility, I was greeted by Mrs. Tylette Green-Cash, program administrator. I stated the purpose of my visit and Ms. Tylette assisted me during the visit. Attendance was taken and observation made. The children were observed participating in teacher-directed activities, routines, having lunch and resting. The last fire inspection was conducted on June 4, 2025, and was approved for day time care only. The last sanitation inspection was completed on June 24, 2025, with superior rating. A partial assessment of the childcare requirements was conducted on this date. The license was posted, restrictions maintained, I monitored supervision and staff/child ratios. I reviewed staff files for criminal background checks and emergency training. I reviewed program files. The following violation was observed, cited, and technical assistance was provided. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). **A review of the staff member's file showed that one staff member has a qualifying letter which expired on September 17, 2025. G.S. 110-90.2(b) & .2703(n)&(o) The above violation must be corrected immediately and a corrective action letter stating when and how the area of non-compliance was corrected. Please submit the letter by e-mailed to: anita.c.graham@dhhs.nc.gov. The letter should reach my office by or before, December 31, 2025. Please include your facility name, facility ID number, and an electronic signature in your e-mail. If I do not receive your letter by December 31, 2025, a return visit may be necessary to ensure that the violations have been adequately addressed and corrected. Additional violations may be documented if each violation has not been addressed sufficiently. You were provided with an opportunity to ask questions regarding child care requirements. TECHNICAL ASSISTANCE: All criminal background renewals must be conducted prior to the expiration of the qualifying letter. The staff member must complete the renewal of the letter and may not care for children until the qualifying letter is renewed. ADDITIONAL INFORMATION: We discussed the Pathways to the Star’s options. You have stated that you are still deciding how you want to proceed. We have discussed Pathway 1 and Pathway 2 and the different ways both may be suitable for your facility. This visit was conducted by Turkessha Casterlow, Childcare Consultant with Tylette Green-Cash, Director. Your program currently operates with a Three Star license. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 85% prior to today’s visit. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children during routine care, lunch, and rest time. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on June 27, 2024. A sanitation inspection was completed November 15, 2024, with a Superior classification. The last fire inspection was conducted on June 4, 2025. Program records and required postings were monitored. A fire drill was conducted on May 16, 2025. A shelter in place drill was documented on March 6, 2025. An outdoor inspection was documented on March 13, 2025. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Hazardous items were stored in a locked closet. Storage and administration of medication were monitored. Medication authorization was monitored. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The last monthly playground inspection was documented on March 13, 2025. .0605(q) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The last EPR plan was updated on August 10, 2017. There have been changes since the last review. .0607(e) Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before June 19, 2025. This program is operating with 3 star rated license. Upon arrival, I informed the owner/director, Tylette Green-Cash of the purpose of my visit. The last annual compliance visit was conducted on 6/27/2024. The operator completed the last fire inspection on 3/27/2024 and the last sanitation inspection on 12/12/2023 with superior rating. Ms. Green-Cash told me she has not had another sanitation inspection since December 2023. I conducted a partial assessment of the childcare requirements today. I reviewed one new staff file. CPR and First Aid training was up to date for the staff members. I observed the children finishing lunch, toileting, and resting. I observed the infants sleeping and crying infants being comforted. One two year old child with developmental needs was placed in space 1 with younger children as the operator and the parent agreed. I observed the following violations during today’s visit. Violation Number Comment Rule 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. "In space 2 and 4, there were small chalks, small manipulative toys, and foam/rubber brushes and tracing shapes in the art area, accessible to children. There were 2 year old children present in the rooms." .0604(q) 866 A caregiver administered medication to a child without parental permission or authorization from a bona fide medical care provider during the event of an emergency medical condition when the child's parent was not available. "There were two inhalers in space 4 without parental permission. Permission for one had expired in March 2024. The other medication did not accompany a written parental permission." 10A NCAC 09 .0803(10) 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. Upon arrival, I was greeted by the owner/administrator Tylette Cash-Green. Ms. Green was working as the lead teacher in the preschool classroom. This program is operating with a three-star license earned with 5 points in Staff Education, 2 points in Program Standards, and 1 Quality Point. The operator completed the last fire inspection on 3/27/2024 and the last sanitation inspection on 12/12/2023. I completed a walkthrough of the indoor and outdoor learning environment. I observed the children playing with the toys and materials accessible to them, getting ready to go outside, playing in the playground, being diapered, eating lunch, and resting. I observed the following violations today. I reviewed the item with Ms. Green and provided technical assistance. Violation Number Comment Rule 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. The refrigerator in the toddler room where children's milk is stored was 50 degrees even after changing to the lowest setting. 15A NCAC 18A .2806(j)(2) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. An outlet in space 4 did not have a safety plug cover. 10A NCAC 09 .0604(c) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A container of Disinfectant Wipes were on a shelf lower than 5 feet off the floor, within child's reach, in space 4. .2820(b) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. On the desk of the director, there was a bottle of "Pepcid" and a bottle of beauty product. The desk was in an open area near the classrooms. This was corrected during the visit. 15A NCAC 18A .2820(d) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. The new employee did not complete the first aid training. She was hired on 5/29/2024. .1102(c) 1871 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with parents of currently enrolled children 0-5 within 30 days of adopting the policy. One child did not have a documentation of a review and receipt of Shaken Baby Syndrome policy on file. .0608(b) The above violations must be corrected immediately. The window month for the assessment is in September, 2024. When the ERS assessment is completed, you may keep the score if you are satisfied. If you are not, you don’t have to keep the score. You can request another ERS during the rated license reassessment year which begins in July 2024. I provided consultation related to the ERS and the child care environment. We discussed the room arrangement possibilities, block areas, displaying more of children’s work on the wall at the child’s level, sanitation and health related practices, and supervision related items. >Visit www.ncrlap.org to access the Environment Rating Scale (ERS) training videos, frequently asked questions, additional notes, and more. >If applicable, submit any new education information to WORKS in a timely manner. Complete and record the orientation received for Ms. Harris withinn6 weeks of the hire date, 5/29/2024. 4. Ms. Harris needs to complete the health and safety training within 12 month of the hire date. After the first year, she has to complete the required hours of annual on-going training. Click on Chapter 9- Child Care Rules. Other Information Weighted blankets or vests: 1.Blankets of any type are prohibited for children aged 0 to 12 months, per .0606(a)(2) Safe Sleep Policy. The current child care rules specifically state no blankets or other items can be used/placed with a sleeping infant aged 12 months or younger. 2.Weighted blankets or vests can potentially restrict a child’s movement; therefore, they are prohibited for use unless a therapist requests and approves the use. A therapist can only approve the use for children 13 months or older. 3.If the use of a weighted blanket or vest is something a facility wants to consider using, you’d need to contact me prior to use to ensure compliance with applicable requirements. I provided Ms. Green an opportunity to ask questions regarding today’s visit and interpretation and the implementation of childcare requirements applicable to the facility. I was greeted by the owner/director, Tylette Cash, who was supervising the resting children in space 4. This program is operating with a three star rated license, meeting enhanced ratios. I completed a walkthrough of the indoor and outdoor of the facility. I observed the children resting. Ms. Cash assisted by gathering the documents and files I needed to review. The operator completed the last fire inspection on 1/10/2023 and the last sanitation inspection on 6/29/2023. I observed the following violation today. I reviewed the items with Ms. Cash and provided technical assistance. Violation Number Comment Rule 892 The center's safe sleep policy was not posted in a prominent place in the infant room where parents and caregivers were able to view daily. .0606(b) The above violation was corrected during the visit; therefore, compliance letter is not required of you. Compliance: Child care providers are required to maintain compliance with all applicable child care rules and regulations at all times. NC GS 110-90(4)(d) requires all child care facilities to maintain compliance history score of at least 75% for the past 18 months or during the length of time the facility operated, whichever is less. Failure to do so may result in an issuance of provisional license or other administrative action. Technical Assistance provided: >If you are using Zono sanitizing solution, it is okay to use on surfaces, but it should not be used on food and mouthed toys. >Remind your infant room staff to keep the safe sleep policy posted for them to refer to and for the parents and the visitors to view. >Until the stair landing in front of the playground gate near space 4 is repaired, continue to use the other gate to enter and leave the playground. >Rated license assessment process was discussed. If applicable, submit any new education information or changes in WORKS in a timely manner. When hiring, try to maintain the same education level or higher for the staff member you are replacing. >Health and Safety Training: After the initial training (within 12 months of the hire date) it’s to be retaken every 5 years.
