    Name of Operation: JUMPING JACKIE'S Facility ID: 73000259 Consultant: ANITA GRAHAM Operation Type: Center Case Number: Visit Date: 6/22/2026 Number Present: 14 Completed Date: 6/22/2026 Age: From 2 To 4 Total Minutes: 196 Time In: 10:48 AM Time Out: 02:04 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor for compliance with applicable childcare requirements during the facilities Annual Compliance visit. This facility operates with a Five (5) Star License which was initially issued on December 31, 2025. Upon arrival to the facility, I was greeted by the owner/center director, Mrs. Jackie Hamlett, who assisted me during today's visit. We conducted a walkthrough of the facility. Attendance was taken and observations made. The children were observed to participate in teacher-directed activities, outdoor free play, routines having lunch and resting. The last Fire Inspection was completed on August 27, 2025, and was approved for daytime care only. The last Sanitation Inspection was completed on June 22, 2026. The facility received a superior rating. A full assessment of child care requirements was conducted on this date. The permit is posted and restrictions are maintained. I monitored supervision and staff child ratio. I monitored program records. I monitored children’s records and staff files. Curriculum/Formative Assessment: -The facility is using the approved curriculum and formative assessment for Lillio Learning-Fun Shine Express accompanied by NC Foundations. -The Facility CQI and Individual CQI Plans for are maintained. -Family Engagement plans are ongoing. -Communication Options include a group me application as well as in-person. - A family resource area is accessible in the child care center that includes information regarding community resources and services that assist families with knowledge, support, and advocacy for their child in the languages of the families served by the child care center. -Engagement and Leadership Options: The child care center offers an enrollment orientation that includes an opportunity for families and children to visit the assigned classroom and lead teachers prior to the first day of enrollment. - The child care center offers at least two-family events annually. -Educational Opportunity Options: The center offers at least one family event annually that includes an educational opportunity for staff and families to learn together. The following violations were observed cited and technical assistance was provided. Violation Number Comment Rule 853 Incident logs were not completed and maintained as required. **Incident log was not maintained as required.** .0802(g)(1-6) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. **A medical record on file for a new staff member was older than 12 months.** 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. **A TB test/screening was on file for a new staff member was older than 12 months.** .0701(a) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. **All required information was not updated as changes as they occurred annually.** .0607(e) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. **The review of the Shaken Baby policy was not documented for one new staff member** .0608(d)(1-4) The above violations must be corrected immediately, and a corrective action letter shall be drafted, stating how, and when each of the items of noncompliance have been corrected. Please address each of the items of noncompliance in your corrective action plan. The letter should reach my office by July 6, 2026. You may email the letter to anita.c.graham@dhhs.nc.gov. If your letter does not reach my office by or before July 6, 2026, a return visit may be necessary to ensure compliance with childcare requirements. Additional violations may be cited during that follow-up visit. TECHNICAL ASSISTANCE: New staff records must include an updated TB test and medical report prior to the first day of caring for children. -The shaken baby policy must be reviewed with all new staff prior to employment. Emergency Preparedness plan should be updated annually to reflect all required information. RESOURCES: -The Health and Safety Trainings are now being offered on the DCDEE website https: Look for the ‘DCDEE Moodle’ tab under the ‘Services’ tab on the website. Health and Safety Trainings should be renewed every five (5) years and may be used as in-service training hours. Thank you for your assistance during today’s visit.
