Name of Operation: EARLY INTERVENTION & FAMILY SERVICES Facility ID: 73000179 Consultant: ANITA GRAHAM Operation Type: Center Case Number: Visit Date: 8/21/2025 Number Present: 23 Completed Date: 8/21/2025 Age: From 3 To 4 Total Minutes: 82 Time In: 10:30 AM Time Out: 11:52 AM Time In: Time Out: List to Use: Center Type Of Visit: Unannounced Visit Follow-Up Announced/Unannounced: Unannounced The purpose of today’s visit was to conduct an unannounced follow-up visit for this Five (5)-Star program. Upon arrival at the facility, I was greeted by a staff member who escorted me to the program administrator’s office. I explained that the purpose of my visit was to conduct the follow-up to the unannounced routine visit conducted on August 8, 2025. At which time, no children were present at the facility. During today’s visit, twenty-eight (28) children are present, ranging in age from three (3) to four (4) years old. I conducted a partial assessment of child care requirements today. The license was posted restrictions maintained. I conducted classroom observations in three classrooms. The children were observed participating in circle time, indoor gross motor activity, outdoor gross motor activities, routines, and preparing for lunch. The following violations were observed, cited and technical assistance was provided. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. ***In all three classrooms electrical outlets and power strips were observed not covered with safety plugs. 10A NCAC 09 .0604(c) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. ***I observed Clorox and Lysol wipes were observed on a shelf and not in locked storage in the restroom in a classroom serving children who are four(4) and five(5) years old.*** .2820(b) The above items must be corrected immediately. Please prepare and submit a formal letter detailing how each item of noncompliance has been addressed and/or corrected. The letter must: Be submitted on your facility’s official letterhead, list and respond to each cited item of noncompliance, including an electronic signature, must be submitted to my office no later than September 4, 2025. Failure to submit the corrective action letter by the stated deadline may result in a return visit to verify compliance, which may lead to additional regulatory requirements. During the visit, you were provided with the opportunity to ask questions related to child care regulations, and we discussed each area of noncompliance in detail to ensure clarity and understanding moving forward. TECHNICAL ASSISTANCE: Please ensure that all cleaning products with two or more warnings are in lock storage. Products which have one warning will be allowed stored five (5) feet up. It is important that all electrical outlets have safety plugs when not in use. RESOURCES: For information about rule changes, visit the Division’s website at ncchildcare.ncdhhs.gov and look under the “What’s” Tab. Also, under the “SERVICES” tab look for “DCDEE Moodle”. From the Moodle home screen go to “My Courses”; and then click on the modules. Health and Safety Trainings are offered on the DCDEE Moodle format. Name of Operation: EARLY INTERVENTION & FAMILY SERVICES Facility ID: 73000179 Consultant: BOON TROTT Operation Type: Center Case Number: Visit Date: 11/7/2024 Number Present: 45 Completed Date: 11/7/2024 Age: From 3 To 5 Total Minutes: 115 Time In: 09:15 AM Time Out: 11:10 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements during a routine unannounced visit. This program is operating with five star rated license. Upon arrival, I was invited in by the administrative assistant, Lisa Anders. She told me the director, Dana Faulkner, was not in yet. I informed Ms. Anders of the purpose of my visit. A short time later, Ms. Faulkner arrived. The last annual compliance visit was conducted on 1/22/2024. The operator completed the last fire inspection on 3/14/2023 and the last sanitation inspection on 5/28/2024. I conducted a partial assessment of the childcare requirements today. I completed a walk through of the facility unescorted. I observed the children eating breakfast, toileting, and having group time. There were no new staff files to review. Ms. Faulkner stated she did not have any new staff members. CPR and First Aid training was up to date for the staff members. I observed the following violation during today’s visit. Violation Number Comment Rule 1739 All records required were not available for review by a representative of the Division. "Fire inspection report was not available for review during today's visit." .2318(1-8) The above violation must be corrected immediately. I must receive a compliance statement/letter by email or mail by 11/21/2024. In the letter, include the following information: 1. Your facility ID# and Name 2. Visit date 3. Violation item number(s) 4. How and when the violation was corrected (Be specific) 5. If it’s near the time for a reinspection and the fire marshal has not come by to conduct a full inspection, you’d need to reach out to ensure the inspection is completed annually. The inspection report should be sent to your child care consultant within 7 days of receipt. This is Child Care Rule 10A NCAC 09 .0304. > Lead in water and asbestos testing should be completed and on file by January 1, 2025.
