Shirley Ball, administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Ball and two additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. A staff member failed to document the accurate arrival and departure time for a 9-month-old on 5/21/25. 10A NCAC 09 .0302(d)(4) Violations must be corrected immediately. Within one week, 6/4/25, you must submit documentation of the corrections you made and your plan to maintain compliance with the identified child care requirements to me at tiffany.d.moore@dhhs.nc.gov. Limited monitoring was conducted today. I informed the administrator, Shirley Ball, of the purpose of my visit. The allegation is as follows: There are concerns regarding unsafe outdoor learning environment. Upon arrival, I observed a group of toddler children going out to the playground. The caregiver led them by supervising them from behind the children. The preschool age children were going for a walk around the neighborhood. There were 6 children with 1 caregiver. They walked on the paved street and took two breaks on the grassy area. I followed them during the entire walk. The following violation was cited. Violation Number Comment Rule 303 Children were not adequately supervised at all times. "I followed the preschool class of 6 children and 1 adult going for a walk around the neighborhood. It was a paved loop street. Children and the caregiver walked in the middle of the road for the most part. I observed two cars go by while they walked. Both times, children happened to be taking a break or exploring on the grassy area, away from the street. When they were walking, I observed the older children running/walking 10 to 40 feet ahead of the caregiver. The caregiver did not use rope or any device/system to assist with appropriate supervision. She had to rely on verbal commands which children did not follow well. The route they took was a residential neighborhood except for the first one third of the way." .1801(a)(1-5) The above violation must be corrected immediately. I must receive a compliance statement/letter by email or mail by 6/20/2024. In the letter, include the following information: 1. Your facility ID# and Name 2. Visit date 3. Findings Two staff members stated they have not had any incidents where any child/children left the group while outside. They stated they did not have to go after any children or see anyone bring the children to them after she/he found them in/near the parking lot. No staff member remembered hearing or seeing anyone have a child take off from a group of children. The administrator told me she did have an incident where the caregiver reported to her that a child took off and that she was able to catch the child right away. I followed the preschool class of 6 children and 1 adult going for a walk around the neighborhood. It was a paved loop street. Based on my observation and the information gathered, the allegation regarding the violation of unsafe outdoor environment was unconfirmed. Technical Assistance I discussed with Ms. Ball regarding supervision and safe practices when children are taken for a walk off premise, including but not limited to having a method to keep the children safe such as rope to hold onto, having an extra adult accompany, reminding children of acceptable and unacceptable behavior while on a walk, etc. Please refer to Child Care Rule 10A NCAC 09 .1801 SUPERVISION IN CHILD CARE CENTERS. At the completion of the visit, I reviewed the visit with Ms. Ball and provided an opportunity to ask questions regarding the interpretation and implementation of child care requirements. I was greeted by the director, Shirley Ball, and informed her the purpose of today’s visit. This program is operating with a Notice of Compliance. I observed the children resting, getting up, and infants being diapered and having a tummy time.Ms. Ball assisted me by gathering the documents and files I needed to review. I completed a walkthrough of the child care facility. The operator completed the last fire inspection on 10/25/2023 and the last sanitation inspection on 12/22/2023. All inspections and drills were up to date. I observed the following violation and reviewed the item with Ms. Ball. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. "In the playground toward the parking lot, there was a black and white 'Tykes' riding toy. One of the wheels has come off the toy leaving the metal part of the petal sticking out." 10A NCAC 09 .0601(a) The above violation must be corrected immediately. This child care center is operating with a Notice of Compliance. Upon arrival, I was invited in by a staff member. The administrator, Shirley Ball, was working in the infant room. I informed Ms. Ball the purpose of my visit. She assisted with the records for review. The last annual compliance visit was conducted on 3/22/2023. Last fire inspection was completed on 7/26/2022, and the sanitation inspection was last completed on 8/3/2023 with superior rating. Ms. Ball informed me the fire marshal conducted a preliminary part of the inspection on August 21 to verify the pertinent facility information and that he will return to conduct a full fire inspection. She did not know the exact date. I conducted a partial assessment of the child care requirements today. I completed a walkthrough of the indoor and outdoor learning environment. There were no new staff files to review. I observed the children resting and waking up. I observed the following violation during today’s visit and provided technical assistance on the items. Violation Number Comment Rule 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. "In space 2 where two year old children were present, there were baskets containing cotton balls, Googly Eyes, and foam materials in the art area." This was corrected by Ms. Ball when I brought it to her attention. She removed the items and stored them out of reach. This is a repeat violation. .0604(q) Compliance letter is not required since the violation was corrected during the visit. Compliance: Child care operators are required to maintain compliance with all applicable child care requirements at all times. GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Reminder: >Email the fire inspection report to me within 7 days of receipt. Technical Assistance provided: >Quiet activities for children not sleeping or waking up before the rest period is over. >Choking hazards for children under the age three. The operator shall submit the original of the approved annual fire inspection report to the Division within one week of the inspection visit on the form provided by the Division.
