This facility operates with a Four (4) Star License permit, which was issued on January 30, 2019. The facility earned seven (7) points in education, two (2) points in program standards and the quality point for program options met. The last annual compliance visit was conducted on September 26, 2024. The provider, Ms. Amelia Holloway, was present and assisted me during today's visit. There are two (2) preschool age children present at my arrival. The children were observed participating in a screen time activity, free play, routines, having lunch and resting. A full assessment of child care requirements was monitored during the visit. The program and space requirements, the license was posted, and restrictions maintained. Staff supervision, interactions with children and capacity. Emergency Numbers, Emergency Procedures and the First Aid postings were available for viewing, and daily schedule was posted. Indoor and outdoor equipment and Furnishings were also monitored. Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five (75) percent. Any violation(s) documented may impact the compliance history score. Your current compliance history is ninety-eight 98%. The following violations were observed, cited, and technical assistance was provided. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. The last annual compliance visit was conducted on September 26, 2024. The provider, Ms. Amelia Holloway, was present and assisted me during today's visit. There are two (2) preschool age children present at my arrival. The children were observed participating in a screen time activity, free play, routines, having lunch and resting. A full assessment of child care requirements was monitored during the visit. The program and space requirements, the license was posted, and restrictions maintained. Staff supervision, interactions with children and capacity. Emergency Numbers, Emergency Procedures and the First Aid postings were available for viewing, and daily schedule was posted. Indoor and outdoor equipment and Furnishings were also monitored. Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five (75) percent. Any violation(s) documented may impact the compliance history score. Your current compliance history is ninety-eight 98%. The following violations were observed, cited, and technical assistance was provided. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Clorox bleach, Lysol wipes, Lysol sprays and other aerosol products were observe unmaintained in proper storage. .1719 (a)(7) 918 Prior to employment, a Criminal Background Check was not completed. The additional caregiver providing care for the children could not provide the CBC letter. A check of the ABCMS system for early child care does not document a CBC for the provider present during the visit. G.S. 110-90.2; .2703(a) 920 Records were not made available for review. The provided did not have/maintain all of the required records required for the Division Representative to review, including information on the additional care giver who was present and assisting in providing care this date. G.S. 110-91(9) 2035 Operator did not document screen time on a cumulative log or activity plan and/or it was not available for review. Operator did not document screen time log, activity nor did they maintain the required fifteen-(15) minute restriction to amount of time children should participate in screen time. Children were allowed unlimited time viewing videos. .1718(b)(3) 9995 All other rules must be followed even if there is not a specific violation number listed on this form. Menu observed posted had a date of March 2025 and substitutions were not marked on the menu. Chapter 10 of the NC General Statues or 10A NCAC Chapter 09 The above violations must be corrected immediately and a compliance letter sent to my office stating how and when the items of non-compliance were addressed and corrected. The letter should reach my office by or before 5:00 pm, September 17, 2025. Please include the name and ID number of your facility, on your facility letterhead. Please close the letter with your electronic signature. If the letter has not been received by my office by 5:00pm, on September 17, 2025, a return visit may be necessary to ensure compliance with childcare requirements. You were provided with an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Childcare licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. It is your responsibility to always maintain compliance with all the applicable laws and rules. The best way for you to make sure that you meet all requirements is to periodically review the childcare laws and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/, on the home page under the ‘Services’ tab. TECHNICAL ASSISTANCE: SCREENTIME REQUIREMENT: screentime must be limited to fifteen (15) minutes per day and no more than 2/12 hours per week per child aged three (3) years and up. The screen should be used for developmental purposes and children who are under three (3) years of age should be given a separate activity, as they are not allowed screentime. GENERAL SAFETY: Electrical sockets, including power strips, must be equipped with safety covers when not in use. Fire drills must be conducted and documented/recording within thirty (30) days of the previous month’s drill. The lockdown/shelter-in-space drills must also be conducted and documented quarterly or every three (3) months. Products with two or more warnings must be maintained in locked storage, these include but are not limited to Clorox, (bleach and wipes), aerosol spray cans, such as Lysol, Febreze, or any products dispensed under pressure. Please ensure that Any items with even one (1) warning are stored at least five (5) feet up and out of reach of children. Nutrition Requirements: Menu items must be planned a week in advance and available for Division Representative Review. See the rule below. The menus posted today has a March date. GENERAL NUTRITION REQUIREMENTS 10A NCAC 09 .0901 (b) When food is prepared by or provided by the center, menus for nutritious meals and snacks shall be planned at least one week in advance. At least one dated copy of the current week's menu shall be posted where it can be seen by parents and food preparation staff when food is prepared or provided by the center. RECORDS: All program records must be available to review by the Division Representative upon request. These include inspections, children’s records/application which must include all the required information such as, copies of immunization; must be on file on the first day of enrollment, the health assessments, signed by a physician, must be on file for each child enrolled in the program within thirty-days (30) of enrollment, hospital/physician preferences, additional information: Parents must provide information such as possible allergies, fears or behaviors a child may or may not have. You have indicated that you have two “additional care givers. All required records for both of these caregivers must be available for review by the Division Representative. All additional care givers, must complete a Criminal Background check prior to providing care for children. Please also remember additional care givers left alone with children must be at least twenty-one (21) years of age. (see the DCDEE website) ncchildcare@dhhs.nc.gov. Click on the Provider Tab, hover over Provider Documents/Forms, click on the Family Child Care and search for all forms records requirements. A follow-up visit will be conducted to ensure all records are available, up-to-date and accurate. Further violations may be cited if compliance is not achieved. ADDITIONAL INFORMATION: Preparing for North Carolina’s New QRIS System As you prepare to participate in Pathways to the Stars; North Carolina’s Quality Rating Improvement System (QRIS) is being modernized to provide more flexible and equitable ways for programs to demonstrate quality. Beginning later this year, providers will be able to choose from three star-rating pathways: Program Assessment, Classroom & Instructional Quality Accreditation. This new approach allows programs to align their strengths with the pathway that best fits their operations. Now is the time to prepare. Programs should begin evaluating staff qualifications including updating staff education in NC Works, curriculum, and classroom interactions to determine the most suitable pathway. Updates to staff qualification standards and assessment tools are expected, and while current star ratings remain valid for now, all new ratings will be issued under the revised QRIS system. Your program’s quality should reflect not only compliance, but also a strong commitment to best practices, child development, and continuous improvement. Providers are encouraged to take advantage of available technical assistance and training opportunities. I conducted a walk-through of the indoor and outdoor learning areas. Ms. Holloway assisted by gathering the records for me to review. During today’s visit a partial assessment of applicable child care requirements was conducted. I observed the following violations. Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). The childcare provider had an expired criminal background check and had not began the process to renew the letter. G.S. 110-90.2 & .2703(n) & (o) 1831 A valid qualification letter was not on file and available for review at the facility. The provider's qualification letter had expired.
