Lori Locklear, Administrator, accompanied me on a walk-through of the premises. The program currently operates with a three-star license, issued July 14, 2022, earning four points in education standards, two points in program standards, and one quality point by choosing to meet the programmatic option. The last annual compliance visit was conducted May 9, 2023. The sanitation inspection was completed March 7, 2023, with a “Superior” classification. The last fire inspection was conducted December 20, 2023, and your facility meets the satisfactory fire safety conditions. The program’s compliance history was ninety-two percent as of March 27, 2024. You stated Quality Learning Center is still own by Sandra Locklear, and the business structure has not changed. Upon my arrival, children were observed during breakfast and indoor free play lead activities. Teachers were observed interacting with children while they played. Age-appropriate materials and equipment were accessible to the children. Children were adequately supervised. Breakfast consisted of biscuits, sausage, hash browns, apples, and milk. Lunch consisted of barbecue, coleslaw, french fries, oranges, apple sauce, and milk Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. The last sanitation inspection received is dated March 7, 2023. The Administrator has requested a sanitation inspection from Environmental Health. 10A NCAC 09 .0304(b) 428 A current activity plan was not posted for each group of children for reference. Spaces #1 & 2, the activity plan was dated for March. GS 110-91(12); .0508(a) 522 Multi-use articles, including highchair feeding trays, were not washed, rinsed and sanitized in the center's kitchen after each use. Three highchair feeding trays were not washed, rinsed and sanitized in the center's kitchen after breakfast and lunch. 15A NCAC 18A .2812(e) 871 Center staff did not comply with the safe sleep policy. Space #2, one infant was in a crib with a bib around their neck. The teacher removed the bib and placed the infant back on their back. 10A NCAC 09 .0606(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. On or before April 24, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when was corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information.
