The annual compliance monitoring checklist for child care centers was used to note requirements monitored today. Upon my arrival, I was greeted by staff member, F. Howard. Ms. Howard stated that the administrator, J. Brown, was outside supporting with a group of children. I was unaccompanied as I completed a general walk-through of the facility which consisted of the indoor and outdoor learning environments, and the kitchen. A total of eighteen (18) children were present during today’s visit. I observed the children during outdoor play, completing routine care tasks, and eating lunch. Today’s lunch was in compliance with the Meal Pattern requirements. LICENSE STATUS: This facility currently operates with a three-star rated license issued January 24, 2022. The NC Secretary of State website was reviewed on February 18, 2026, and J’s Play and Learn, LLC was listed as current and active. REQUIRED INSPECTIONS/DRILLS: The last annual compliance visit was completed March 6, 2025. The last annual fire inspection was completed on February 6, 2026. The last annual sanitation inspection was completed on December 16, 2025. The last documented monthly fire drill and the last documented monthly playground inspection were completed on February 6, 2026, and the last documented quarterly emergency drill and shelter-in-place drill was conducted on December 3, 2025. During today’s visit, a full assessment of the child care requirements was conducted. I observed all required postings, and attendance logs. There are no infants enrolled, so safe sleep logs were not monitored. I also monitored program requirements, equipment and furnishings, staff/child ratios & supervision, storage and/or administering of medication, and the outdoor area & equipment. I reviewed staff files and a sample of the children’s records. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. In space 1, an activity plan dated February 9-13, 2026, was posted. GS 110-91(12); .0508(a) 612 Each child's bed, cot, or 2" mat was not individually assigned and identified. In space 4, five (5) cribs and one (1) cot were not labeled with identifying information for the six (6) children present. 15A NCAC 18A .2821(b) & (c) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. An aerosol can of Lysol was stored in the bathroom. .2820(b) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Prescribed medication for enrolled child, I.I., was not stored in a locked storage. 15A NCAC 18A .2820(d) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. Written permission authorizing the caregiver to administer topical ointment for two (2) enrolled children was not on file. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) COMPLIANCE HISTORY: Prior to today’s visit, the 18-month compliance history for this facility was 76%. Additionally, written permission from the parent or guardian authorizing the caregiver to administer the medication must be on file and available to review. Please review section .0803 of the child care rules regarding administering medications in child care centers. Please note that a current activity plan must be posted in each space where children are present for easy reference. Activity plans serve as a guide for caregivers to use to deliver a rich, balanced curriculum that supports every child’s individual needs and promotes growth and development. During today’s visit, I observed that each crib and cot was not clearly labeled to identify which bed was assigned to each individual child. As a reminder, all cribs, cots, or mats must be clearly labeled with identifying information such as the child’s name and/or assigned number that corresponds with a classroom cot/mat chart. This helps caregivers easily determine which sleeping space belongs to a specific child. Additionally, these practices help support organization and ensure that children’s belongings remain separate, helping to reduce the spread of germs and communicable diseases. As a reminder, all child care programs are expected to achieve and maintain compliance at all times and are required by North Carolina General Statute 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Upon my arrival, I was greeted by the administrator, J. Brown. Limited monitoring of the child care requirements occurred during today’s visit. The license was posted. I observed all required postings and attendance logs. A total of seventeen (17) children were present during today’s visit. The children were observed during naptime. Ms. Brown submitted a compliance verification letter on December 12, 2025, stating that all violations have been corrected. The following violations were monitored for compliance during today’s visit: Item #315- During today’s visit, staff/child ratios were maintained for all groups. Therefore, no additional violation cited during today's visit. Item #1048 & Item #1049-On December 8, 2025, Ms. Brown submitted documentation verifying that a staff member with a hire date of July 21, 2025, had successfully completed first aid and CPR training. Ms. Brown accompanied me as I completed a general walk-through of the facility which consisted of three (3) classrooms, the outdoor play areas, and the kitchen. Due to the recent impacts of Hurricane Helene and other natural disasters, this deadline has been revised statewide, and facilities that have not yet enrolled should complete the online process by May 31, 2025. The Clean Classrooms for Carolina Kids Program provides free facility-wide testing and inspections without any out-of-pocket costs. A total of seventeen (17) children were present during today’s visit. I observed the children participating in free choice play and preparing for lunch. Today’s lunch was in compliance with the Meal Pattern requirements. LICENSE STATUS: This facility currently operates with a three-star rated license issued January 24, 2022. The NC Secretary of State website was reviewed on March 6, 2025, and J’s Play and Learn, LLC was listed as current and active. REQUIRED INSPECTIONS/DRILLS: The last annual compliance visit was conducted on April 4, 2024. The most recent annual fire inspection was completed on May 1, 2024, and the most recent annual sanitation inspection was completed on November 6, 2024. The most recent documented monthly fire drill was conducted on February 4, 2025, the most recent documented quarterly lockdown drill/shelter-in-place drill was conducted on December 6, 2024, and the most recent documented monthly playground inspection was completed on March 4, 2025. During today’s visit, a full assessment of the Child Care Requirements was conducted. I observed all required postings, attendance logs, and safe sleep logs. I also monitored program requirements, equipment and furnishings, staff/child ratios & supervision, storage and/or administering of medication, and the outdoor area & equipment. I also reviewed a sample of the children’s records. The director did not complete the staff/training worksheets; therefore, staff files were not monitored during today’s visit. This facility does not provide transportation. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 303 Children were not adequately supervised at all times. Three (3) children were left unsupervised in the classroom while the caregiver went to greet the consultant. .1801(a)(1-5) 316 Children under one year of age were not kept separate from children two years and older. A six-month-old infant and a two-year-old child were grouped together. 10A NCAC 09 .0713(a)(5) 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The posted menus were dated for February 2025. 10A NCAC 09 .0901(b) 544 Screen time was offered to children under three years of age. A group of four(4) two-year-old children were watching television. .0510(f) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space 1, a can of Lysol, Clorox bleach cleaner, and Grease Lightening were stored in an unlocked closet. .2820(b) 860 Balloons were accessible to children. Helium balloons located at the front of the facility were accessible to children. .0604(q) 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A two-year-old was not attended to in a nurturing and appropriate manner when she began to cry after breaking a toy. A caregiver could be heard telling the child to hush up and stop crying, because it would not have broken if you did not have it in your mouth. Hush before you will be standing somewhere by yourself which is not keeping with the child’s developmental needs. G.S. 110-91(10) COMPLIANCE HISTORY: Prior to today’s visit, the 18-month compliance history for this facility was 95%. COMPLIANCE LETTER: All violations must be corrected immediately. A compliance letter is due to me within two (2) weeks of today’s visit. By March 20, 2025, please send me a letter addressing each violation, how the violations were corrected, and how compliance will be maintained in the future. TECHNICAL ASSISTANCE/CONSULTATION: During today’s visit, Ms. Baldwin left a total of three (3) children unsupervised so that she could assist me with the visit. I stated that children must be adequately supervised at all times. Child Care Rule .1801 states that adequate supervision shall mean that: (1) staff must be positioned in the indoor and outdoor environment to maximize their ability to hear or see the children at all times and render assistance; (2) staff must interact with the children while moving about the indoor or outdoor area; (3) staff must know where each child is located and be aware of the children's activities at all times; (4) staff must provide supervision appropriate to the individual age, needs, and capabilities of each child; and (5) staff must be able to see and hear children aged birth to five years old while the children are eating. All of the conditions in this Paragraph shall apply except when emergencies necessitate that adequate supervision is impossible. In addition, an infant under one year of age was grouped with a two-year-old child. Violations pertaining to staff/child ratios and supervision are particularly serious as these types of violations have a direct impact on the safety of the children while in your care and must be corrected immediately. As a reminder, staff files are reviewed during an annual compliance visit and subject to additional review during other compliance monitoring visits. Therefore, it is important to update staff and training worksheets as you hire/dismiss employees, complete ongoing training, and meet other child care requirements. Thank you for your time today. I observed fourteen (14) children present during today’s visit. The children were observed eating lunch and preparing for naptime. Today’s meal was in compliance with Meal Pattern requirements. CRIMINAL BACKGROUND CHECK SYSTEM: North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. ABCMS allows administrators to see the real-time background check status of staff members, run printable report of the staff roster to assist with compliance visits, and see new background check applicants and add to the staff roster.
