Ms. L. Stokes Owner /Administrator accompanied me on the walk- through of the facility and assisted with today’s visit. The group size, supervision, ratio, capacity and approved space were in compliance. The restrictions, license and emergency care plan were posted. First Aid/CPR, storage of hazardous materials and the NC Child Care Summary of the Law were posted. I monitored the program requirements, furnishings and equipment, outdoor area, and medications. Since my last visit you stated you have employed three new staff members and the staff and training worksheet were completed during the visit. Today two infants were observed sleeping in their cribs while one toddler was observed playing with toys. The preschool children were observed playing in activity areas and completing an art activity. The theme in the lesson plans was Thanksgiving. The lunch menu met the meal pattern requirements . This program currently operates with a four -star license issued October 2, 2023. Your last fire drill was completed 11/10/25. The monthly outdoor playground inspection was completed 10/15/25. Your last EPR drill was a shelter-in-place completed 10/2/25. I reminded you that an EPR drill must be completed every three months. Please keep all documentation on file for review. The following violation was documented during the visit. Violation Number Comment Rule 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Documentation with visually checking on sleeping infants aged 12 months or younger was not completed daily for 2 infants in care. .0606(g) The violation must be corrected immediately. A compliance letter is due to me within two (2) weeks of today’s visit. By December 8, 2025, please send me a letter addressing the violation, how the violation was corrected, and how compliance will be maintained in the future. In your letter, be sure to include your facility name, facility ID number and each item number for the violation cited. You may submit the letter to me electronically at carla.hanchey-edwards@dhhs.nc.gov Childcare licensing requirements are established to ensure a safe and healthy childcare environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. Technical assistance regarding the following items were reviewed with you today. I reminded you that the health and safety training courses have been relocated from Pro-Solutions to Moodle on the DCDEE website and they are free of charge. I discussed various other training resources, including those online and in-person. Remember continuing professional development training provides you and your staff with the knowledge and skills needed to provide a quality child care program. Please be reminded that the health and safety training courses are required to be completed every five years. LICENSING FEES: Annual Licensing Fee: Pursuant to GS 110-90(1a) the Division establishes an annual fee for licensed child care facilities. The invoices for the fees are assessed based on your licensed capacity and emailed after the 1st of November every year. You will only receive one invoice, and payment is due in FULL within 30 days. Please closely monitor your email and be on the lookout for your invoice. Please don’t delay making your payment and be aware that Payment Plans are NOT available. In addition, please be aware that failure to pay a license fee within 30 days of receipt will result in late charges. If payments are made late and late charges are applied, the fee as well as the late charge must be resolved. Please be reminded that failure to pay a license fee could also result in the facility's child care license being revoked. A checklist was used to note the requirements I monitored today. Today L.Stokes Owner/ Administrator was not present during the time of the visit. J.Carroll and T. Stokes both assisted with today’s visit and stated the Administrator was out of town. During the visit the Administrator was called to let her know I was present completing a visit. Prior to today’s visit I reviewed your corporation status was current-active on the Secretary of State website. This program currently operates with a four-star license issued October 2, 2023. Please be reminded to have all the staff’s education sent to WORKS to be evaluated. The hold harmless has been extended until further notice. I monitored the indoor and outdoor environments for compliance with childcare requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival, and departure procedures, first aid chart, menus, activity plans, daily schedules, emergency care plan, emergency telephone numbers, staff records, and summary of NC Child Care Law, were posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. Prior to the visit I received your staff and training worksheets. Your signature on these forms will serve as verification that the information provided is accurate. A random selection of children’s files and staff files were reviewed, and the children’s record worksheet was completed. The infants and toddlers were observed playing with toys. The two-year-old children and the group of preschool children were observed playing in activity areas inside their classrooms. The school-aged children were not present during the time of the visit. The theme in the lesson plans were artists and community helpers. The lunch menu for today consisted of sliced hotdogs, green beans, wheat bread, pineapple, and milk. Your last fire drill was completed 2/25/25 and your monthly playground inspection was dated 2/14/25. Your last EPR drill was a lockdown completed 1/13/25. I reminded you an EPR drill must be completed every three months and to keep all documentation on file for review. Your last fire inspection was dated 10/21/24. Your last sanitation inspection was completed 12/12/24 with a superior rating. Please contact Environmental Health and your local fire inspector within 30 days of the due date to request an inspection to be completed. One staff member with a hire date of 9/22/24 did not have a completed Emergency form on file for review. CBC Provider Portal Technical Assistance & Notification to the Division of New Hires or Residents Technical Assistance Guidance As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Name of Operation: TAILORED LEARNING CENTER TOO Facility ID: 31000279 Consultant: MICHELE REMINGTON Operation Type: Center Case Number: Visit Date: 8/16/2023 Number Present: 19 Completed Date: 8/16/2023 Age: From 0 To 6 Total Minutes: 180 Time In: 09:35 AM Time Out: 12:35 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of my unannounced visit today was to monitor your center for meeting applicable child care requirements during your Temporary License period. Ms. Hanchey-Edwards, Child Care Consultant, accompanied me today. She will be you contact moving forward. During today’s visit the following items were monitored for compliance: supervision, ratios, CPR/FA training, Criminal Background Checks, storage of hazardous substances, and medications, use of approved/adequate space, children’s and program records, license, and permit restrictions. The last fire drill was on 07/172023 with an evacuation time of one (1) minute. The last outdoor inspection was completed by SH on 07/19/2023. Daily schedules and current weekly activity plans were posted in each classroom. A current weekly menu was posted, today’s meal met the USDA requirements. When we arrived, the children were participating in classroom activities. Staff engaged in activities and conversations with in three of the classrooms, when we entered the classroom for school age children the staff member was sitting at a desk and not engaging with the children. We discussed this with the classroom teacher and the administrative staff. One violations of child care requirements were cited today. It was corrected during the visit and a compliance letter is not required. Violation Number Comment Rule 1115 First aid kit and/or fire extinguisher was not located in vehicle; kit and/or extinguisher was not mounted or secured if in passenger area. A fire extinguisher was not on the center van when we checked the van. A fire extinguisher was purchased and placed in the van during the visit. 10A NCAC 09 .1003(c) RATED LICENSE ASSESSMENT: a rated license application and quality point form were completed today. A rated license will be processed by 09/15/2023, the temporary license expires on 10/01/2023. Details were reviewed last week during today’s visit. Ms. Hanchey-Edwards will need to verify staff information and WORKS prior to processing the rated license. Program Points- 2 - did not choose to have an ERS assessment. You will meet enhanced space. Please update the posted ratio charts on 10/01/2023. Education Points – TBD based on current staff information. Quality point – 1 – staff benefits and parent involvement. The current law allows you to use the previous owner’s rated license as this was a change of ownership, the previous owner had a three-star license. If you will be eligible for a higher star rated license, a rated license will be processed using your current staff. Two of the current staff working in a classroom worked for the previous owner, all other classroom staff are new. TECHNICAL ASSISTANCE & CONSULTATION: Interactions and conversations with children should occur all day in every classroom, age-appropriate activities should be offered to children throughout the day. When staff engage and encourage play children benefit from the interaction. One staff member was sitting at a desk quietly rather than playing and talking with the children. In another classroom children were given black and red crayons and a ladybug sheet to color. Limit use of premade “art” and allow children to color, paint, create what they want to create. If you are talking about ladybugs, you can read stories, show pictures, and then if children choose to draw/color a ladybug they can use a blank sheet of paper. In the classrooms group like materials together, i.e., manipulatives together, dramatic play materials in the dramatic play area, block, and accessories in the block areas. Duplo are not considered blocks; they are considered manipulatives. If it stacks together, it is a block if it hooks together, it is a manipulative. Ensure areas are large enough to for at least three children to use materials at the same time. In all classrooms (or in the break room) have a place for staff to lock their belongings. Today a staff purse was on the floor. She moved the purse to a high shelf in the classroom. ANNUAL COMPLIANCE VISIT: Your center will be due for an annual compliance visit prior to 04/01/2024. Additional visits may be conducted by the Child Care Consultant prior to then. In mid-January complete the staff and training worksheet. You will need to have an annual fire inspection by 08/03/2024. An annual sanitation inspection will be due by 06/29/2024. The sanitation and fire inspections must be mailed to your consultant within 7 days. You may access these forms on the DCDEE website at www.ncchildcare.nc.gov. ON-GOING TRAINING: Training hours will be monitored for compliance during your annual compliance visit. Training hours are required to be recorded on the on-going training log with certificates attached to the log for verification. This information will be reviewed by the child care consultant during the annual compline visit. HEALTH & SAFETY TRAINING: All staff must complete Health & Safety training topics within one (1) year of hire. After initial completion all topic areas must be completed every five years. A training log is available on the DCDEE website. This training information will be reviewed by a DCDEE consultant during a routine unannounced/technical assistance or courtesy visit. Annually you must review the EPR plan online and print the review form for your records. You must review the EPR plan and the EMC plan with all staff on an annual basis. I recommend doing this in the fall each year prior to the annual compliance visit. GENERAL INFORMATION: The facility email and phone number were verified during today’s visit. If you have any changes to your phone number, email or mailing address, please update me so I can make changes in Regulatory.
