Upon arrival I was greeted by Ms. L. Ambrose Family Services who assisted with today’s visit. Today Ms. S. Lisane Eligibility Recruitment Selection Enrollment and Attendance Manager from Duplin County Schools was also present during the visit. Ms. S. Lisane stated she works with all of the Prekindergarten sites in Duplin County. During the visit the principal was not on site at the facility. The group size, supervision, ratio, capacity and approved space were in compliance. The restrictions, license and emergency care plan were posted. First Aid/CPR, storage of hazardous materials and the NC Child Care Summary of the Law were posted. I monitored the program requirements, furnishings and equipment, outdoor area, and medications. The preschool children in spaces #116 and #117 were observed playing in activity areas inside their classrooms. The children in space #118 were observed during morning circle time. The theme in the lesson plans were on the five senses and pumpkins. The lunch menu met the meal pattern requirements. This program currently operates with a five-star license issued June 6, 2019. Your last fire drill was completed on 10/31/25. The last monthly outdoor playground inspection was dated 11/4/25. Your last EPR drill was a shelter-in-place completed 11/10/25. I reminded you that an EPR drill must be completed every three months. Please keep all documentation on file for review. The following violation was documented and corrected during the visit. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance).The qualification letter had not been renewed for two employees prior to beginning work at this license facility. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. Technical assistance regarding the following items were reviewed with you today. I reminded you that the health and safety training courses have been relocated from Pro-Solutions to Moodle on the DCDEE website and they are free of charge. Remember continuing professional development training provides you and your staff with the knowledge and skills needed to provide a quality child care program. Please be reminded that the health and safety training courses are required to be completed every five years. The hold harmless has been extended until further notice at this time. I monitored the indoor environments for compliance with child care requirements. The outdoor environment was not monitored due to inclement weather during the time of the visit. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival, and departure procedures, first aid chart, menus, activity plans, daily schedules, emergency care plan, emergency telephone numbers, staff records, and summary of NC Child Care Law, were posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. A random selection of staff and children’s records were also reviewed today. Prior to the visit I received your staff and training worksheets. Your signature on these forms will serve as verification that the information provided is accurate. The children’s record worksheet was completed. NC Pre-K During today’s visit, I monitored the three NC Pre-K classrooms for requirements in section .3000 of the child care rules. The center uses Creative Curriculum and completes the Teaching Strategies Gold checklist to document the evidence of children’s ongoing process. The checkpoints are completed three times a year during the fall, winter, and spring. A parent teacher conference is held three times a year or more if needed. The classrooms operate from 7:50am-3pm. Newsletters are sent home monthly which include information on current themes, activities, and upcoming events. The last rating scale assessment overall score received was 5.38. Upon arrival the children in space #116 were playing in activity areas. The children in spaces #117 and #118 were in morning group time. The theme in the lesson plans were ocean life and pets. The lunch menu consisted of chicken patty with bun, sweet potato fries, green beans, orange slices and milk. While reviewing children's files, one child was missing an immunization record. During today’s visit I completed a walk-through of the facility with PreK Family Specialist Ms. Ambrose who also assisted with today’s visit. Upon arrival the children in spaces #116 and #118 were observed playing in activity areas inside their classrooms. The children in space #117 were observed in morning group time. The group size, supervision, ratio, capacity and approved space were in compliance. The restrictions, license and emergency care plan were posted. First Aid/CPR, storage of hazardous materials and the NC Child Care Summary of the Law was posted. I monitored the program requirements, furnishings and equipment, outdoor area, and medications. The theme of the lesson plans was love, valentines, and animals. The lunch menu consisted of spaghetti, green peas, bread sticks, bananas, and milk. The program currently operates with a five-star license issued June 6, 2019. During the visit I reminded you to make sure all staff have sent their education to WORKS to be evaluated. The harmless hold has been extended until further notice at this time. Your last fire drill was completed 1/14/25, and your last monthly outdoor playground inspection was dated 2/4/25. Your last EPR drill was a shelter-in-place dated 9/26/24. A violation was documented for the EPR drill. Please keep all documentation on file for review. Please be reminded your current fire inspection expires 3/6/25. One violation was documented during today’s visit. Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last shelter-in-place drill on file was dated 9/26/24. .0604(u);.0302(d)(8) The violation must be corrected immediately. Please send me a compliance letter (written statement) describing how and when the above violation was corrected. The compliance letter must be received no later than February 25,2025. Contact me if you have questions regarding correction of the violation documented during today’s visit. If you are unable to correct the violation within the specified timeframe, send a compliance letter regarding the violation that was corrected. An extension will need to be requested from management. Childcare licensing requirements are established to ensure a safe and healthy childcare environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. Technical assistance regarding the following items were reviewed with you today. All flexibilities expired, except for the flexibilities related to the star rated license. During today’s visit I explained the importance of completing the EPR drills within the timeframe to meet compliance with childcare requirements. Please be reminded that an EPR drills shelter-in-place or lockdown must be completed every three months. You stated the EPR drill will be completed and kept on file. I reminded you the health and safety training courses have been relocated from Pro-Solutions to Moodle on the DCDEE website and they are free of charge. I discussed various other training resources, including those online and in-person. Remember continuing professional development training provides you and your staff with the knowledge and skills needed to provide a quality child care program. Please be reminded the health and safety training courses are required to be completed every five years. CBC Provider Portal Technical Assistance & Notification to the Division of New Hires or Residents Technical Assistance Guidance As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Clean Classrooms for Carolina Kids is a program designed to identify and eliminate exposure to lead and asbestos hazards in buildings.
