An annual compliance monitoring checklist was used. You completed the FCCH and Centers in Residence Statements of Responsibility and the FCCH Verification Form. Your signature on these forms will serve as verification that the information is accurate and complete. During the visit you stated at this time you do not have an additional helper. This program currently operates with a four-star license issued August 2, 2023. During the visit, the inside area of your home was monitored. The outside was not monitored due to inclement weather during the time of the visit. Today there were two preschool children present observed playing with toys. The childcare room and the bathroom the children use was observed. Smoke detector, fire extinguisher, first aid kit, refrigerator thermometer, storage of medication and hazardous products, outlets, garbage storage, steps, toys, and materials were all monitored for compliance. The permit, emergency numbers, schedule and were posted. Qualifying letters, CPR / First Aid, health questionnaire, emergency procedures, and children's records were also monitored for compliance. Today I reminded you if any of your grandchildren turn sixteen, they will need a criminal background check because they are part of your household. You stated you were aware of this and did not have any questions at this time. During the visit you stated you had been working with the children on colors and shapes. The lunch met meal pattern requirements. Your last fire drill and monthly playground inspection were completed 6/11/26. The last EPR drill was a shelter-in-place completed 4/18/26. I reminded you this must be completed every three months and documentation kept on file for review. I recommend that you place your due dates on a calendar or a spreadsheet to help in maintaining compliance. The following violation was documented during the visit. Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. One child's completed health assessment was not on file for review during the visit. GS 110-91(1); 10A NCAC 09.1721(a)(1) The violation must be corrected immediately. A compliance letter is due to me within two (2) weeks of today’s visit. By July 1, 2026, please send me a letter addressing the violation, how the violation was corrected, and how compliance will be maintained in the future. In your letter, be sure to include your facility name, facility ID number and for the item number for the violation cited. You may submit the letter to me electronically at carla.hanchey-edwards@dhhs.nc.gov . If you are unable to correct the violation with the specified time frame, please submit a request for an extension. This request should include a specific plan to correct the violation and any timeframe needed. The request will need approval from management. Childcare licensing requirements are established to ensure a safe and healthy childcare environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. Today your additional caregiver who is your live-in daughter was present, and all the information was completed. This program currently operates with a four-star license, issued August 2, 2024. During the visit, the inside and outside areas of your home were monitored. During the visit there were three preschool and one school-aged child present. The children were observed playing with toys and later the additional caregiver was observed reading a story to the children. The childcare room and the bathroom the children use were observed. Smoke detector, fire extinguisher, first aid kit, refrigerator thermometer, storage of medication and hazardous products, outlets, garbage storage, steps, toys, and materials were all monitored for compliance. The permit, emergency numbers, schedule and NC Summary of the Child Care Law were posted. The lunch menu for today consisted of tuna, bread, lettuce, tomato, fruit, and milk. Your last fire inspection was completed 2/25/25. Your last fire drill was completed 6/10/25 and monthly outdoor playground inspection was dated 5/13/25 and a violation was documented. Your last EPR drill was a shelter-in-place completed 6/11/25. I reminded you this must be completed every three months and documentation kept on file for review. Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. The monthly playground check on file was dated 5/13/25. 10A NCAC .1721(e)(5)(A-F) Childcare licensing requirements are established to ensure a safe and healthy childcare environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. Technical assistance regarding the following items were reviewed with you today. All flexibilities expired, except for the flexibilities related to the star rated license. Please be reminded that a monthly outdoor inspection must be completed and documented and on file for review. Remember to check your playground daily prior to the children going outside to play. CBC Provider Portal Technical Assistance & Notification to the Division of New Hires or Residents Technical Assistance Guidance As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Today I reviewed your email address with you, and you stated nothing has changed. You stated your live-in daughter is your additional caregiver if needed. This program currently operates with a four-star license, issued August 2, 2023. The hold harmless has been extended until further notice. During the visit, the inside and outside of your home was monitored. Upon arrival you stated you have three children present, ranging from age 2 to 4 years of age. The children were observed playing with toys in the childcare room. The childcare room and the bathroom the children use was observed during today’s visit. Smoke detector, fire extinguisher, first aid kit, refrigerator thermometer, storage of medication and hazardous products, outlets, garbage storage, steps, toys, and materials were all monitored for compliance. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Aerosol can spray, bleach, and a container of disinfectant hand towels were not kept in locked storage in the bathroom the children use. .1719 (a)(7) 716 Electrical outlets not in use were not covered. One electrical outlet was not covered in the bathroom the children use. 10A NCAC .1719(a)(27) 907 Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. Two dogs rabie vaccinations expired 3/14/25. Both dogs received a new rabie's vaccination with an expiration date of 3/18/28. .1719(b)(1) 1804 Safe sleep policy and poster about safe sleep practices was not posted in a prominent place in the infant sleeping room or area where parents and caregivers are able to view daily.
