A checklist was used to note the requirements monitored today. Upon arrival I was greeted by Elizabeth Groves, the Assistant Director. McKenzie Coon, the Director assisted me with today’s visit. There was a total of seventy-eight (78) children present, aging in range from infant to five years old. The children were observed playing in centers and with materials, participating in circle time, playing outside, completing routine tasks, and eating lunch. LICENSE STATUS Currently, this Child Care Center operates with a three (3) star license issued August 26, 2019, earning five (5) points in education, two (2) points in program standards and one (1) quality point. I reviewed the NC Secretary of State’s website and observed the owner of the facility as Rolesville Creek Learning Center, Inc. listed as current/active. INSPECTIONS *The last fire drill was conducted 1/13/26. *The last emergency drill was conducted 10/30/25. *The last playground inspection was completed 1/8/26. *The last sanitation was completed on 4/1/25 with a ‘Superior’ classification. *The last fire inspection was conducted on 1/5/2026. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. New staff CPR/FA, qualification letters and newly enrolled children files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. Transportation is not provided; therefore, these requirements were not monitored. The following violations were observed and documented during today’s visit. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. Medication authorization fora child in space #3 did not have valid dates for authorization. 10A NCAC 09 .0803(4)(6-9) 871 Center staff did not comply with the safe sleep policy. The crib sheet for an infant was not fitted for a child under 12 months of age in space #2. 10A NCAC 09 .0606(a) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before January 28, 2026, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. PRESCRIPTION MEDICATION/ NON-PRESCRIPTION It was observed that a diaper cream form for Aquaphor did not have dates for authorization. The administrator and teacher were unaware that the permission form did not have dates for authorization. I would recommend that all forms and medication are given to the administrators before being placed in the classrooms for review. SAFE SLEEP Licensed Centers are to ensure that the safe sleep policy is implemented and adopted to keep all infants 12 months and younger safe by not having loose bedding in a crib. In space #2 a sheet on a crib was not fitted that could be unsafe for a sleeping infant. I would recommend discussing with the parents to provide a fitted sheet. I would also recommend that you review your safe sleep policy and ITS-Sids training with all staff. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. QRIS Modernization A transitional plan was developed during the visit. The Administrators stated that they would like to complete Pathway 1 in September of 2026. HEALTH AND SAFETY TRAINING Health and Safety training is now available through Moodle on the DCDEE website. This training is provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the training in 2021, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. Upon arrival, I was greeted by the Director, Debra Boykin. You assisted me with the visit. I initially did observations in space #8. I then did observations in each classroom as lunch was taking place. I did the observations without the administration. While completing the classroom observations, I interviewed the teachers in space #6, # 7, #8 and #9. At the time of the visit, there were a total of 57 children present, ranging in age from infants to five (5) years old. The children were observed playing in centers, reading books, participating in outside water play, eating lunch, interacting with the teachers, and completing routine tasks. When I completed my observations, I discussed the nature of the complaint with Ms. Boykin. Ms. Boykin initially couldn’t recall any behaviors in space #8 that would have caused a child to be cared for in the office due to behavior. Ms. Boykin shared that most of the challenging behaviors occurred in space #5 and space #7. She recalled that she did have a child in space #8 with challenging behaviors about a year ago. Ms. Boykin discussed with me that she has had children spend some of their nap time in the office. I discussed with Ms. Boykin that the teachers all stated that they contact administration when they have a child with behaviors. Several teachers stated that when a child is removed from the classroom due to challenging behaviors, the child could stay for three minutes up to thirty minutes. A few teachers stated that some children have stayed in the office during the naptime and would return once naptime was over. During my observation in space #8 I observed two children having challenging behaviors. The teacher instantly called the front office to remove one child from the classroom. I asked the teacher what the policy is and procedure when a child is having challenging behaviors. The teacher discussed with me that she mostly does positive reinforcement and natural consequences before contacting the administrators to assist. The following violation was documented during today’s visit. Violation Number Comment Rule 209 Children used space that was not approved. Children are being cared for in the office during naptime when having challenging behaviors. GS 110-91(1)&(4-5) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. RESOLUTION Based on interviews, and the admission from the Director and the teachers that children are being cared for during naptime in the office in an unlicensed space, the allegation is SUBSTANTIATED. TECHNICAL ASSISTANCE The following item was discussed during today’s visit to help you maintain quality within your program and to prevent further violations. During today’s visit I observed teachers contacting the administrators when challenging behavior occurred, as well as the teachers stated that is what they typically do. It is important to know that positive verbal interactions are essential to the emotional and social development of a child. It is important to ensure that your teachers are not overwhelmed to have a positive classroom environment. I would highly recommend viewing the Early Years website at www.earlyyears.nc.org and have your staff take training modules pertaining to positive interactions. I would highly recommend your staff complete the Flip it – Transforming challenging Behaviors trainings. One of the biggest challenges that early childhood educators and administrators face is children’s behavior. Often these behaviors are developmentally appropriate, however, some behaviors can be difficult for a teacher to manage. Providing technical assistance, coaching and professional development for staff to prevent and address these challenging behaviors, as well as promoting children’s social competence can be beneficial. I would also recommend that you view the website for Caring For Our Children, https://nrckids.org/CFOC regarding Chapter 2 Positive Behavior Support. Please take note of the resources such as Conscious Discipline, Redirecting Behavior, and Positive Guidance. It is best practice to allow children to remain in their learning environment instead of being removed from their environment. I would encourage giving all teachers tools and resources to help with challenging behaviors. CONSULTATION One of the best signs of quality child care is consistent staff with low turnover rates. Stress contributes to increased staff turnover and can negatively affect their interactions with children. Staff who are under stress may not be able to offer the praise, nurturing, and direction that children need for optimal development. Stress reduction procedures allow staff to identify risks of stress and the stressors specific to child caregiving, and specific ways to manage stress in the early care and education environment. It is also very important to ensure that all staff are properly onboarded. Proper training for new staff and continued training for old staff is essential for low turnover. I discussed with the director to work on team building activities during staff meetings. I shared ideas with Ms. Boykin that could possibly assist in building staff morale within the school. Please know that substantiation of a complaint allegation can possibly lead to an Administrative Action. Upon arrival, I was greeted by the new Assistant Director, Makayla Coon. The Director, Debra Boykins, assisted me with the visit. I discussed the nature of the visit. I then conducted a general walkthrough of the center. At the time of the visit, there were a total of 77 children present, ranging in age from infants to five (5) years old. The children were observed playing in centers, interacting with the teachers, playing outside and completing routine tasks. Ms. Boykins discussed with me that she was unaware of any ratio issues occurring on May 21st. Ms. Boykins discussed with me that she had had a number of teachers that left at one time. Staffing was very challenging, however, no one brought to her attention that a child staff ratio issue occurred. I interviewed the toddler teacher concerning May 21st ratio. The teacher discussed with me that on May 21st during drop off she had twelve children one year of age by herself. I asked if she tried to contact the administrators. The teacher stated that staffing was very challenging. The director was in the infant room as the teacher, and the assistant director was in ratio in another classroom. The Director discussed that she was not made aware of this until now. Ms. Boykins discussed with me that she has not been made aware of any concerns with diaper changes. I discussed that these concerns started once the main teacher was used in other classes. Ms. Boykins discussed with me that the main teacher was willing to assist in the two-year-old classroom. I interviewed the teacher that was working in the older toddler classroom. The teacher discussed with me that she changes diapers around 4:00 and she checks to make sure diapers are ok to send the child home. I viewed bright wheel documentation for diaper changing for three random children in the toddler classroom for May 22nd, May 27th and May 30th. The following violation was observed and documented during today’s visit: Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. On May 21, 2025 one teacher was caring for twelve children age one in space #1. GS 110-91(7);.0713(a-d) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before June 17, 2025, the letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. Please make sure to include: -Facility name -Facility ID number -Each item number The letter can be emailed to Shamequa.Wilkerson-Harris@dhhs.nc.gov RESOLUTION Based on interviews, and the teacher admitting that she was out of ratio on May 21st with twelve children one year of age alone, the allegation of child staff ratio is SUBSTANTIATED. Based on the interviews, and documentation viewed I am unable to determine if a lapse in diaper changing occurred. Therefore, the allegation is UNSUBSTANTIATED. TECHNICAL ASSISTANCE The following item was discussed during today’s visit to help you maintain quality within your program and to prevent further violations. -Technical assistance was given to Ms. Boykins regarding the arrival time of her staff based on the arrival time of the children. Arrival time is not always predictable for each child daily; therefore, I would recommend completing a time study over a week basis to determine staff schedules. Please be mindful that moving the oldest or youngest child each morning to the next classroom to meet staff/child ratios is not best practice. When children arrive at school they should be allowed to arrive in their classroom and not have to move to another classroom because of not having enough staff present on site. Required staff-child ratios for non-ambulatory children are essential for their safety and caregiving routines. Lower ratios are most critical for infants and young toddlers (birth to twenty-four months) as infant development and caregiving quality improves when group size and ratios are smaller. A follow-up visit will be made soon due to a child staff ratio occurring. Please know that substantiation of a complaint allegation can possibly lead to an Administrative Action. The expiration dates were 8/2024, 9/2024, 10/2024, and 12/2024. 10A NCAC 09 .0803(1)(d) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. A staff member that is scheduled for the infant/toddler classroom did not renew ITS-SIDS within three years. The monitoring also included License and permit restrictions, staff/child ratios, supervision, storage of hazardous items, medication, materials/equipment, and all space occupied by the children including outdoor play environment. This will also allow for parents to send emergency medications to the center before the expiration date. CPR/FA and SIDS TRAININGS During today’s visit I observed one teacher with expired CPR/FA training and one teacher with expired SIDS training. The administrators stated that they will be scheduling the training to be in compliance. I would recommend using a reminder to remind you of upcoming training courses to remain in compliance. EMERGENCY DRILLS During today’s visit I observed the emergency drill was last conducted on 3/6/24. Emergency drills should be conducted quarterly. I would recommend utilizing an outlook calendar or some form of reminder to ensure that all drills are completed on time to remain in compliance. STORAGE OF MEDICATION and HAZARDOUS ITEMS I observed diaper cream and sunscreen stored in an unlocked cabinet accessible to the children in care. The teacher stated that they had completed diaper changes and had not locked the cabinet. It is important to keep all medications out of the reach of children. I would recommend placing all diaper creams and sunscreen 5ft up in a locked device or storage. I would also recommend you visit the website Caring for Our Children pertaining to health and safety. I also observed in space #7 an unlocked cabinet under the sink that stored disinfectant Lysol wipes and Windex spray. The teacher stated that they did not know the cabinet was unlocked. I would recommend checking to make sure all cabinets under the sinks are locked and hazardous items are stored properly. INFANT BOTTLES I observed bottles in the infant room not properly labeled with a date. The teacher stated that the parents usually label with the name and date each morning. I would recommend that the teachers check to make sure all bottles are labeled properly daily. STAFF/CHILD RATIO During today’s visit I observed fifteen (15) children and one (1) teacher in space #5, which caused them to be out staff/child ratio. The classroom had two (2) three-year-olds, ten (10) four-year-olds and three (3) five-year-olds in attendance. The center has chosen to be licensed for enhanced ratios. I discussed with the administrators that they could choose to be licensed with enhanced space instead of ratios to assist with remaining in compliance with ratios. The administrators stated that they will remain with the enhanced ratios. Please know that I will be returning soon to follow up with ratios. Upon arrival I was greeted by Kelly Stallings, the Director. You and Debra Boykins, Co-Director assisted me with the visit. I completed a general walk-through of the center. At the time of the visit, there were a total of 89 children present, ranging in age from infant to five (5) years old. The children were observed playing outside, playing with materials, interacting with the teachers, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a three (3) star license issued August 26, 2019, earning five (5) points in education, two (2) points in program standards and one (1) quality point. INSPECTIONS *The last fire drill was conducted 6/26/24. *The last emergency drill was conducted 3/6/24. *The last playground inspection was completed 6/26/24. *The last sanitation was completed on 6/25/24 with a “Superior ‘’ classification. *The fire inspection was completed on 1/11/24 and the facility is approved for daytime care only. MONITORING During today’s visit, a partial assessment of Child Care Requirements was conducted for all classrooms. The monitoring included CPR/First Aid, criminal background checks and special training requirements of newly hired staff. The monitoring also included License and permit restrictions, staff/child ratios, supervision, storage of hazardous items, medication, materials/equipment, and all space occupied by the children including outdoor play environment. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. Minimum staff/child ratio was not maintained in space #5 during today's visit. Space #5 consisted of two (3-year-olds), ten (4-year-olds), and three (5-year-olds) with one teacher. GS 110-91(7);.0713(a-d) 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Bottles in space #2 were not labeled with the date during today's visit. 15A NCAC 18A .2804(d) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Disinfectant Lysol wipes and Windex spray was stored in an unlocked cabinet under the sink in space #7 during today's visit. .2820(b) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Diaper cream and sunscreen was stored in an unlocked cabinet under the sink in space #1. 15A NCAC 18A .2820(d) 843 A drug or medicine was administered after its expiration date. EpiPen was observed with an expiration date of June 2024 in space #6. 10A NCAC 09 .0803(1)(d) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. A staff member First Aid training expired on 12/2023. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. A staff member CPR training expired on 12/2023. .1102(d) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last documented shelter-in-place/lockdown drill was conducted on 3/6/24. .0604(u);.0302(d)(8) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before August 13, 2024, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. EPI-PENS I observed the Epi-Pen expired. The director stated that the parent has ordered a new Epi-Pen. I would recommend checking all medications weekly to ensure that medications are returned to the parents. This will also allow for parents to send emergency medications to the center before the expiration date. CPR/FA and SIDS TRAININGS During today’s visit I observed one teacher with expired CPR/FA training and one teacher with expired SIDS training. The administrators stated that they will be scheduling the training to be in compliance. I would recommend using a reminder to remind you of upcoming training courses to remain in compliance. EMERGENCY DRILLS During today’s visit I observed the emergency drill was last conducted on 3/6/24. Emergency drills should be conducted quarterly. I would recommend utilizing an outlook calendar or some form of reminder to ensure that all drills are completed on time to remain in compliance. STORAGE OF MEDICATION and HAZARDOUS ITEMS I observed diaper cream and sunscreen stored in an unlocked cabinet accessible to the children in care. The teacher stated that they had completed diaper changes and had not locked the cabinet. It is important to keep all medications out of the reach of children. I would recommend placing all diaper creams and sunscreen 5ft up in a locked device or storage. I would also recommend you visit the website Caring for Our Children pertaining to health and safety. I also observed in space #7 an unlocked cabinet under the sink that stored disinfectant Lysol wipes and Windex spray. The teacher stated that they did not know the cabinet was unlocked. I would recommend checking to make sure all cabinets under the sinks are locked and hazardous items are stored properly. INFANT BOTTLES I observed bottles in the infant room not properly labeled with a date. The teacher stated that the parents usually label with the name and date each morning. I would recommend that the teachers check to make sure all bottles are labeled properly daily. STAFF/CHILD RATIO During today’s visit I observed fifteen (15) children and one (1) teacher in space #5, which caused them to be out staff/child ratio. The classroom had two (2) three-year-olds, ten (10) four-year-olds and three (3) five-year-olds in attendance. The center has chosen to be licensed for enhanced ratios. I discussed with the administrators that they could choose to be licensed with enhanced space instead of ratios to assist with remaining in compliance with ratios. The administrators stated that they will remain with the enhanced ratios. Please know that I will be returning soon to follow up with ratios. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the trainings in 2019, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. AGENCY UPDATES Please remember to check the Division of Child Development and Early Education website (ncchildcare.gov) on a weekly basis to stay up to date on all new policies and rules. The center currently operates with a 3-star license, operating at ratios in rule .2818 (b). My visit was conducted with you, Heather Director. A checklist was used to note the requirements I monitored. The children were observed preparing for nap. The facility does not transport children; therefore, these requirements were not monitored. VERIFICATION OF RECORDS: *Last fire inspection= 1-11-24 *Sanitation inspection=11-29-23 (send me your 2024 inspections within 5 days) *The EPR was updated on 1-3-24; Heather completed EPR training on 2-21-19 & Jennifer Cua, Regional Director completed EPR training on 5-20-16. *The last fire drill was on 1-29-24 and the last playground safety check was checked on 1-29-24. Heather completed playground safety training on 8-20-19. *The last water test completed by Clean Water for Carolina Kids was on 7-12-23 and will expire on 7-12-26. *The Guinea Pigs last veterinarian exam was on 12-20-23 and will expire 12-20-24. To maintain compliance with NC laws and rules regarding childcare operation, the following violations must be corrected immediately.
