Upon arrival I was greeted by Vanessa Cami, the Director/Owner. You assisted with today’s visit. There was a total of sixty-five (65) children present, aging in range from infants to five (5) years old. The children were observed playing in centers, having water play, interacting with the teachers, playing outside, eating lunch, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a Three (3) star license issued July 22, 2025, earning one (1) point in education, six (6) point in program standards and one (1) quality point. I reviewed the NC Secretary of State’s website and observed the owner of the facility Little Ones Academy listed as current/active. INSPECTIONS *The last fire drill was conducted on 5/9/2026. *The last emergency drill was conducted on 11/18/25. *The last playground inspection was completed 5/19/26. *The last sanitation was completed on 3/9/26 with a ‘Superior’ classification. *The last fire inspection was conducted on 3/10/26. MONITORING During today’s visit, a partial assessment of Child Care Requirements was conducted for all classrooms. The monitoring included CPR/First Aid, criminal background checks and special training requirements of staff. Four (4) newly hired staff files were reviewed. The monitoring also included License and permit restrictions, staff/child ratios, supervision, storage of hazardous items, medication, materials/equipment, and all space occupied by the children including outdoor play environment. Transportation is not provided; therefore, these requirements were not monitored. This facility does have children under 12 months enrolled; therefore, safe sleep requirements were monitored and found in compliance. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. It was observed in space #4 solar buddies sunscreen and after bite ointment did not have written authorization from parents to be used. 10A NCAC 09 .0803(1)(a & b) 847 Parent's medication authorization did not include required information. In space #2, #4 and #7 medication authorization information was not completed for sunscreens, diaper creams and lotions. 10A NCAC 09 .0803(4)(6-9) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space #2 sunscreen was expired with an expiration date 11/2025 and 3/2025. In space #4 Sunscreen was expired with expiration dates of 3/2025 and 2/2026. .0803(12) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Plastic bags and ziplock bags were observed in the art center, science center and in the children's cubbies in space #4 that care for two-year old children. .0604(q) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last documented shelter in place or lockdown drill was conducted on 11/18/25. .0604(u);.0302(d)(8) 1856 Center was using an inflatable and it was not during a special event. Inflatables with water and pools were observed being used during today's visit. .0605(s) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before July 10, 2026, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. Please make sure to include: -Facility name -Facility ID number -Each item number The letter can be emailed to Shamequa.Wilkerson-Harris@dhhs.nc.gov COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 94%. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. PRESCRIPTION AND NON-PRESCRIPTION MEDICATION Sunscreen and diaper cream in space #4 were observed without having written authorization from parents. Sunscreen was not stored in a locked container or five feet up in space #1. Sunscreen was stored in an unlocked drawer and on the counter. In space #2 two sunscreen lotions had expiration dates of 3/2025 and 11/2025. In space #4 three sunscreens had an expiration date of 3/2025, and 2/2026. In space #4 after bite ointment and solar buddies’ sunscreen did not have parent authorization forms. In spaces #2, #4, and #7 parent authorization forms were not completed with all information. The administrator stated that the parents were given the form to sign but was unaware the parents did not complete the forms. The administrator stated she will fix all forms today in each classroom. was not aware that diaper cream needed written authorization from parents. I would recommend having all forms and medications reviewed by the administrator before placing them in the classrooms. I would also suggest sending a memo out to parents informing them that authorizations forms must be completed before leaving any medication. PLASTIC BAGS It was observed in space #4 that cares for children two-year-old plastic bags in the science area, art area, and in the children’s cubbies. Plastic bags are a suffocation and choking hazard for children two years and younger. I would recommend checking for all plastic bags, zip lock bags in all classrooms that care for children under the age of three. I would also review with staff your safety policy regarding plastic bags. INFLATABLES/WATER PLAY During today’s visit water slide inflatables with pools attached were being used. I discussed with the administrator the safety and sanitation risk of using inflatables and water pools. We discussed alternatives to having water play with the inflatables and wading pools. The administrator stated she was unaware that inflatables and wading pools were not allowed. I would recommend viewing information from Caring for our children regarding inflatables and water play at https://nrckids.org/CFOC. EMERGENCY DRILLS The last documented shelter and place or lockdown drill was documented on 11/18/25. It is important to conduct quarterly emergency drills for the safety of your children and staff. I would recommend you use your outlook calendar or a reminder tool to remind you quarterly to complete the emergency drills. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. Please make sure you place a copy of the criminal background qualification letter in the file of each staff member when hired. AGENCY UPDATES Please remember to check the Division of Child Development and Early Education website (ncchildcare.gov) on a weekly basis to stay up to date on all new policies and rules. A checklist was used to note the requirements monitored today. Upon arrival I was greeted by Vanessa Cami, the Director/Owner. You assisted with today’s visit. There was a total of fifty-two (52) children present, aging in range from infants to five (5) years old. The children were observed playing in centers, interacting with the teachers, playing outside, eating lunch, and completing routine tasks. The center must provide a permit for the club house on the playground that is being built. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. New staff CPR/FA, qualification letters, a percentage of old staff and children’s files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. Transportation is not provided; therefore, these requirements were not monitored. This facility does have children under 12 months enrolled; therefore safe sleep requirements were monitored and found in compliance. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Air Freshener, clorox wipes, modge podge, and shaving cream was not stored in a looked room or cabinet in the Pre-K4, Pre-k3, and infant classrooms. .2820(b) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. The forms were printed out during the visit in preparation to have parents complete at pick-up. I would recommend having all forms and medications reviewed by the administrator before placing them in the classrooms. I would also suggest sending a memo out to parents informing them that authorizations forms must be completed before leaving any medication. STORAGE OF HAZARDOUS MATERIALS Clorox wipes, air freshener in an aerosol can, modge podge, and shaving cream were not stored in a locked cabinet in Pre-K4, Pre-K3, and Infant classrooms. To ensure the safety of all children it is important that all cleaning supplies and aerosol can items be stored in a lock cabinet or container out of the reach of children. I would recommend going over your safety practices that include storage of hazardous materials with all staff during your staff meeting. I would also recommend that you view the website Caring For Our Children, https://nrckids.org/CFOC in regard to Chapter 3 Health Promotion and Protection. STAFF FILES Medical and TB reports were older than 12 months for two staff members hired. The administrator was unaware that the staff were to have a medical or tb renewed if they are older than 12 months when hired. I would recommend reviewing all staff files to ensure that all staff are in compliance. I discussed with the Administrator that I am available to assist with questions concerning files.
