Shameka Jordan, Owner/Operator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Jordan. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed during today’s visit. Violation Number Comment Rule 920 Records were not made available for review. On May 31, 2024, the operator failed to provide one child's file for review. G.S. 110-91(9) 1885 Operator released a child to an individual not listed on the application. On May 29, 2024, the operator released four children to an individual based on verbal consent given by the parent. The individual was not listed on any of the children's applications. .1721 (b) All violations documented above must be corrected immediately. A written, dated, and signed letter of compliance must be submitted to me within one week, by June 7, 2024, describing how and when the violations were corrected and how compliance will be maintained in the future. A checklist was used to note the requirements I monitored today. The facility operates with a Four-Star license issued May 31, 2019, earning 4 points in Education, 7 points in Program, and 1 quality point for choosing a Programmatic Option. Restrictions on the license include a 1st, 2nd and 3rd shift capacity of 8 children ages 0-12; maximum of 5 preschool children at any time; fireplace/woodstove not used during operating hours; no more than 2 children under the age of one. The license was posted, and restrictions were in compliance. The last annual compliance visit was conducted at this facility on February 2, 2023. The facility’s 18-month compliance history was 100% as of January 22, 2024. Owner/Operator, Shameka Jordan, was present and available for consultation. There are twenty one children enrolled between first, second, and third shift care and four children ages 2 – 6 were present today. All approved indoor and outdoor spaces were monitored today. Daily schedules were available for children. Activity plans were observed posted in the classroom. A variety of age-appropriate learning materials was observed in the classroom. Children were seated at the table participating in free choice activities upon arrival. Children were observed completing bathroom and handwashing routines and prepared to eat lunch. Lunch consisted of hotdogs, French fries, orange slices, and milk. The caregiver spoke with the children, and they all engaged in conversation during their meal. After lunch, children washed hands and lay down on individual cots with individual linens as soft music played, and the lights were dimmed. Outdoors there were balls, bikes, ride on toys, a basketball goal and action figures for children to engage with. Supervision and interactions between children and caregiver were appropriate. Reminder: Capacity - You currently have a total of twenty one children enrolled at your facility on first, second, or third shift, including drop ins. You cannot have more than eight children, five preschool children and three school age children, present at any time, no exceptions. If you have preschool children of your own, they count in your total number of preschool children allowed at one time. There is no shift overlap and it is vital that you keep accurate sign in/out sheets and attendance records to verify times when children are in attendance. There was one violation documented today. Violation must be corrected immediately. Continued compliance with applicable child care requirements is required at all times. Review the applicable child care requirements often. It is your responsibility to be knowledgeable of the child care laws and rules. Take time daily to review the applicable child care requirements. Violation Number Comment Rule 9995 A two year old child was fed hot dogs and it was not properly cut up to prevent choking. Chapter 10 of the NC General Statues or 10A NCAC Chapter 09 In order for your facility to remain in compliance with NC Child Care Requirements, violations must be corrected immediately. A signed and dated letter of compliance must be received by January 30, 2024, describing how each violation was corrected and how compliance will be maintained in the future. A time frame is established to allow you time for submitting your compliance letter. This timeframe is not intended to be used as a guide for correcting violations, as they should be corrected immediately. Box 278, Jacksonville, NC 28541 Technical assistance regarding violation cited: Safety – A two-year-old child was served a hot dog that was not properly cut up. Hot dogs that are not cut up properly are a choking hazard for children under the age of three. Hot dogs are long and round and when (young children) bite off a piece of it, it really looks like a thick quarter and that is the perfect size to get lodged into a child's throat. Choking is a leading cause of injury among children, and can sometimes be fatal. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times whether or not they have been monitored or specifically discussed with you in the past. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Other Information Rated License Reassessment - Your facility’s three-year rated license reassessment was due to be completed in July 2021. As discussed today, reassessments will not become mandatory again until July 1, 2024, and your facility has been placed into “Cohort 2.” Placement in this cohort means you will have from July 1, 2024, to June 30, 2025, to plan and prepare for your reassessment, with your reassessment being due between July 1, 2025, and June 30, 2026. In your planning year, you may review resources available at www.ncrlap.org and contact Smart Start of Pender County for assistance. I am also available to assist you upon request. Ms. Jordan advised that would like to complete her reassessment in the summer – she will notify me of her decision. Rules - Child Care Rules were revised July 1, 2023, and a new rule book was posted to the DCDEE website. As discussed, you should discard any old versions of the rules and ensure you are referring to the most current rule book when reviewing or researching requirements. Sanitation rules were also readopted July 1, 2023.
