Name of Operation: WILLIFORD ELEMENTARY SCHOOL Facility ID: 64000467 Consultant: APRIL FLEMING Operation Type: Center Case Number: Visit Date: 9/4/2025 Number Present: 85 Completed Date: 9/4/2025 Age: From 3 To 5 Total Minutes: 300 Time In: 10:00 AM Time Out: 03:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of my routine unannounced visit was to monitor for compliance with applicable child care requirements. Currently, this facility operates with a Five-Star license, issued on July 15, 2024. The last annual compliance visit was conducted on February 5, 2025. The last sanitation inspection was completed on August 13, 2025, earning a Superior classification and “9” demerits. The last satisfactory fire inspection was completed October 10, 2024, and was approved for day time care only. Natasha Williams, NC PreK Coordinator, was onsite during the visit. Mary Wright and Taniqua Rackley, Teachers, were present and available to assist. Eighty-five children were present during the visit. Children were observed during outdoor play, free choice activities, and preparing for lunch. This facility operates as Development Day and participates in the NC PreK program. The facility is owned and operated by Nash County Public Schools. The following violations were documented during the visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Six electrical outlets not in use, located on the prek hallway did not have safety plugs. 10A NCAC 09 .0604(c) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Two staff members did not renew certification in First Aid prior to expiration date. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Two staff members did not renew certification in CPR prior to expiration date. .1102(d) The above violations must be corrected immediately. The letter must describe, in detail, specifically how each violation has been corrected and any supporting documents. You can mail or email me with verification of compliance regarding the violations that were documented. If you choose to mail, two signed and dated copies must be sent and received by September 18, 2025. Compliance History: Prior to today's visit, this facility maintained an 18-month compliance score of 92%. TECHNICAL ASSISTANCE/CONSULTATION: NCPREK TEACHER REQUIREMENTS/DCDEE WORKS: During the visit, I discussed teacher and teacher assistant requirements to Mrs. Wright and Mrs. Rackley. All requirements should be met to ensure compliance. Also, staff should have an education qualification letter on file. To have education evaluated, staff should create a DCDEE WORKS account. Technical assistance was given to Mrs. Rackley and Mrs. Wright on how to create an account. CPR/FIRST AID: CPR & FA should be completed within the first 90 days of employment and prior to expiration date. Two staff members, CPR & FA, were observed expired. To ensure compliance, I suggest a training expiration list be created as a reminder for staff when trainings are due. ELECTRICAL OUTLETS: Electrical outlets not in use should be covered with safety plugs. Six (6) electrical outlets in the hallway were not covered with safety plugs. Staff stated custodial staff use outlets and do not return the plug. To ensure compliance, I encourage you to have a discussion with custodial staff to inform them of the importance of keeping the outlets covered. Also, designated staff should complete a morning and afternoon walkthrough checking outlets, ensuring they are covered in the event custodial staff does not. This was corrected during the visit when staff covered the outlets. FIRE INSPECTION: The fire inspection was observed in compliance. However, an inspection will need to be completed by the end of October 2025 to ensure compliance. CRIMINAL BACKGROUND APPLICATIONS: A New feature links criminal background applications to facilities. This was observed; however, staff are still linking their criminal background checks to the facility. Please remember you must notify the Division of any new child care providers who were hired within five business days. This should be reflected in the ABCMS. CLEAN WATER FOR US KIDS: In the Clean Water for Carolina Kids facility data, status for lead in water status is “section complete”. The lead-based paint and asbestos status is “enrollment started”. To ensure compliance, you should log onto or contact Clean Water for US Kids to know what needs to be completed to have a “section complete” status. STAR RATED LICENSE (SRL)/QRIS Pathway to Stars: The Pathways to the Stars transition will begin soon. A rule roll out is coming soon to our DCDEE e-learning Moodle platform. An in-person meeting discussing the Pathway to Stars will be held September 18, 2025. Please be on the lookout for an email invitation. A checklist was used to note the requirements I monitored during the visit. The last sanitation inspection was completed on November 1, 2024, earning a “Superior” classification and “11” demerits. The last satisfactory fire inspection was completed on October 10, 2024. M. Muhammad, Lead Teacher, was present and available to assist. There was Ninety-seven (97) children present during my visit. Children were observed in free choice play, outdoor activities, teacher directed activities, toileting, and handwashing. All staff interacted with children in a positive manner. Public school off-site forms were observed completed. Fire drills, lockdown/shelter in place, and playground inspections were current. This facility participates in the NC PreK program and is owned and operated by the NC Public Schols. NC PreK requirements were monitored during today’s visit. The monitoring tool was onsite. The following violations of child care requirements were documented during the visit. Provider Access to ABCMS allows administrators to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. To get started, complete the ABCMS Child Care Provider Portal Training in Moodle—a video tutorial followed by a few questions. CLEAN WATER FOR US KIDS: The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. In the Clean Water for Carolina Kids facility data, the facility status for lead in water has been completed. However, lead based paint and asbestos status is enrollment has started but not completed. To maintain compliance, you should log into the clean water for kids to ensure all steps are completed to determine if your facility is exempted or need testing. STAR RATED LICENSE (SRL): Hold harmless has been extended until the new QRIS is implemented (SB 425). This means that no child care facilities in cohort one will have a rated license assessment unless they voluntarily want to. Also, cohort two do not need to start a preparation year. The last sanitation inspection was completed on February 19, 2024, earning a “Superior” classification and “9” demerits. The last satisfactory fire inspection was completed on April 8, 2024. N. Williams, Director of Early Learning and M. Muhammad, Lead Teacher, was present and available to assist. Eighty-eight (88) children ages three (3) to five (5) years old were present.
