I am conducting the visit today for April Fleming. Ms. Fleming remains your assigned Child Care Consultant. Her contact information is: (252)450-9796 and her email is: april.fleming@dhhs.nc.gov. The entire home is licensed for child care and must meet the applicable child care requirements. Although you designated space to be used for child care in a specified section of your home, the license is issued for the entire location. Any monitoring visit conducted may include spaces other than those you designated for child care. All areas of your home and the outdoor area were monitored today. Your program currently operates with a Three-Star license issued June 21, 2018, earning 6 points in the education component, 2 points in the program standards component and 1 quality point for infant capacity restricted to no more than two (2) children under one year of age. The last annual compliance visit was conducted on February 4, 2025. The facility’s compliance history was reviewed with you today. Five (5) children ranging in ages from infancy through three (3) years old were present. The children were observed playing with developmentally appropriate materials in the area designated for child care. The children played with Legos, riding toys, pretend food, blocks and pretend animals. You and the children also sang to music during the visit. The indoor and outdoor learning environment were observed during today’s visit. You interacted with the children in a positive and nurturing manner. TRANSPORTATION: When asked, you are not providing any transportation. The following violation was documented during today’s visit: Violation Number Comment Rule 2058 The family child care home operator did not collect and submit samples of water from each outlet used for drinking or food preparation for lead analysis to the local health department or a laboratory certified to analyze for lead in drinking water by the North Carolina State Laboratory of Public Health. The operator has not collected nor submitted water samples from each outlet used for drinking or food preparation for lead analysis. 10A NCAC 09 .1725(a)(2) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The program’s compliance history was 98% percent as of January 28, 2026. The violation documented must be corrected immediately. Verify compliance on or before February 11, 2026. I must receive a compliance letter that describes accurately and in detail how and when the violation was corrected. Please be aware that any written information submitted by you regarding correction of violations documented during today's visit is legal documentation. Therefore, it is important that all information submitted be accurate and truthful. Failure to comply may affect your Family Child Care Home License. You verified the mailing address, phone number, and email address listed on the facility profile I shared with you are correct. TECHNICAL ASSISTANCE/CONSULTATION: LEAD WATER, LEAD PAINT, AND ASBESTOS TESTING: According to the Clean Water for US Kids website, you have not yet completed the webinar nor completed the testing of your water for potential lead hazard. We discussed logging onto the Clean Water for Us Kids website: www.cleanwaterforuskids.org to complete the enrollment and webinar for lead water, lead paint, and asbestos testing. If you need assistance, please let me know. You stated you have someone that can assist you with the computer/technology. ONGOING TRAINING: According to the information on file, you did not have any documentation of completing on-going training. You stated you are working on registering to complete the required trainings for the year. You are required to complete at least eight (8) hours of on-going training per year. Ensure you complete at least six (6) hours of training on or before February 4, 2026. You had two (2) carry forward hours from last year. Your last annual compliance visit was conducted February 4, 2025. Ensure you maintain your training certificates on file for review. CHILDREN’S RECORDS: We discussed ensuring a medical exam and immunization records are on file for the infant enrolled. The infant was enrolled on January 20, 2026 therefore a medical exam and immunization records need to be completed and on file on or before February 20, 2026. CPR AND FIRST AID TRAINING: You are due to renew CPR and First Aid training on or before August 2, 2026. Ensure you schedule and complete the training before the expiration date. QRIS CONVERSATION TEMPLATE: We briefly reviewed section .3200-Standards for Two through Five Star License. I explained that April Fleming, assigned Child Care Consultant, will further share information with you pertaining to the QRIS. You were encouraged to visit DCDEE’s website and review the “QRIS Modernization” tab for more detailed information pertaining to QRIS. I showed you how to access the QRIS Modernization page today. We reviewed the requirements for you to potentially transition using Pathway Two, which is the Classroom and Instructional Quality Pathway. We reviewed each required form for Pathway Two. You can also review the Moodle training from DCDEE’s website to help you better understand the rule changes in section .3200 of the Child Care rules. If you are interested in Pathway Two, begin working on collecting documentation for Family and Community Engagement and working on using an approved formative assessment and approved curriculum. The transition time for existing programs for QRIS is around September 2026. This facility operates with a Three Star License issued June 21, 2018. The last annual compliance visit was completed April 12, 2023. A checklist was used to note the Family Child Care Home requirements monitored today. J. Hammiel, Owner/Operator was present and available to assist. Three (3) children, zero (0) to one (1) year of age were present during the visit. The children were observed during free choice play and bottle feeding. The caregiver interacted with the children in care in a positive manner. This family child care home is licensed to serve five (5) children. Fire drills, shelter in place/lockdown drills, and playground inspections were all current. You stated you only provide transportation in emergencies. This program is owned and operated by Joyce Hammiel, Sole Proprietor. HOUSEHOLD MEMBERS: You stated you are the only household member. If there are changes in your household, please contact me immediately. The following violations of child care requirements were documented. Violation Number Comment Rule 919 Accurate records were not maintained for all staff and children. Accurate children records were not maintained for four children. G.S. 110-91(9) The above violations must be corrected immediately. The letter must describe, in detail, specifically how each violation has been corrected and any supporting documents. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules always if they have been monitored or specifically discussed with you in the past. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Compliance History: Prior to today's visit, this facility has maintained an 18-month compliance score of 87%. TECHNICAL ASSISTANCE AND CONSULTATION: RECORDS: Children records were monitored and not in compliance. To ensure compliance, you should continue to use the children’s checklist and complete a file review quarterly. Caregiver verification of required information, and statement of responsibility form was completed and verified. STAR RATED LICENSE (SRL): The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024, and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each includes a preparation year and a reassessment year. Your program is in cohort one and this is your preparation year. You should ensure your three month self-assessment is completed to avoid delay of process of rated license when reassessment year arrives. More information will be provided as the cohort approaches. PROVIDER DOCUMENTS/FORMS: You should visit the website often to ensure you’re using all updated forms.
