Facility ID: 64000057 Consultant: APRIL FLEMING Operation Type: Center Case Number: Visit Date: 4/24/2025 Number Present: 40 Completed Date: 4/24/2025 Age: From 4 To 12 Total Minutes: 330 Time In: 10:00 AM Time Out: 03:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp w/Rated Lic Assess Announced/Unannounced: Unannounced The purpose of today’s unannounced annual compliance visit was to monitor this program for compliance with applicable child care requirements. Currently this facility operates with a Five-Star License, issued December 5, 2023. The last annual compliance visit was conducted on May 14, 2024. A checklist was completed to note the requirements monitored during today’s visit. The last sanitation inspection was completed on November 8, 2024, earning a superior classification and ten (7) demerits. The last satisfactory fire inspection was completed on October 9, 2024. C. Viverette, Director, was present and available to assist. Forty (40) children were present. Children were observed during outdoor play, free choice activities and during quiet time. Staff interacted with children in a positive manner. Fire drills, lockdown/shelter in place and playground inspections were current. The following violations of child care requirements were documented during the visit. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. A newly hired group leader did not have a medical report on file prior to employment date of April 14, 2025. The medical report was completed April 23, 2025. 10A NCAC 09 .0701(a) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. A newly hired group leader did not review the prevention of shaken baby syndrome and abusive head trauma policy prior to providing care with children. The policy was reviewed and dated April 24, 2025. .0608(d)(1-4) The following violations were corrected during the visit. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules always if they have been monitored or specifically discussed with you in the past. The best way for you to make sure that you meet all requirements is to periodically review the child care law and rules. Compliance History: Prior to today's visit, this facility maintained an 18-month compliance score of 98%. CONSULTATION/TECHNICAL ASSISTANCE CHILDREN AND STAFF RECORDS: Ten percent (10%) or more of children’s records were monitored. Children records reviewed, were in compliance. Other children’s files not reviewed should be monitored to ensure compliance for the same information. New staff and ten percent (10%) or more of existing staff files were monitored. Staff records reviewed were not in compliance. Other staff files not reviewed should be monitored to ensure compliance for the same information. I encourage you to continue to use the staff checklist and note key words such as "prior to" to ensure forms and documents are submitted in time. CRIMINIAL BACKGROUND APPLICATIONS: A New feature links criminal background applications to facilities. North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. This feature should be implemented to avoid violations. All staff should be linked to the facility. Instructions were left during the visit. CLEAN WATER FOR US KIDS: Results for lead in water, lead base paint and asbestos was verified completed in the Clean Water for Carolina Kids facility data. STAR RATED LICENSE (SRL): Hold harmless has been extended until the new QRIS is implemented (SB 425). However, you have requested to have the Environment Rating Scales completed. The following information pertains to the rated license reassessment. PROGRAM STANDARDS: The program currently meets minimum requirements and all enhanced requirements including enhanced ratios and space. Policies and procedures have been previously approved. The provider has requested to have the rated license assessment completed. An application for assessment and the rated license assessment request review forms were completed and received during the visit. EDUCATION STANDARDS: ADMINISTRATOR/PROGRAM COORDINATOR: The Administrator has earned a four-year degree, a Level III administrator and BSAC training certificate. The Program Coordinator has earned an NCECC certificate and BSAC training certificate with four additional school-age hours. GROUP LEADERS: Four to five group leaders are required for this facility. All group leaders have earned a BSAC training certificate with at least 600 hours of license school age experience. Any newly hired group leaders should ensure they have a DCDEE WORKS account with transcripts submitted, BSAC certificate uploaded and the group leader position applied for. Based on the information provided and observed points can not be determined at this time. QUALITY POINT: This program will continue to meet the one (1) quality point for having a staff benefits package and infrastructure for parent involvement. Based on this information, you may be eligible for one (1) quality point. Based on the information provided points can not be determined until the ERS assessment is completed and all staff are in DCDEE WORKS. Currently, this facility operates with a Five-Star license issued on December 5, 2023. The center's last annual compliance visit was completed on May 14, 2024. The last sanitation inspection was completed on November 8, 2024, earning a “Superior” classification and “7” demerits. The last satisfactory fire inspection was completed on October 9, 2024, and was approved for daytime care. C. Viverette, Director, was present and available to assist. Fifty-five (55) children were observed arriving at the center. Children were observed in free choice activities and preparing to eat snack. Staff interacted with children in a positive manner. The following violations of child care requirements were documented during the visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Provider Access to ABCMS allows administrators to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. To get started, complete the ABCMS Child Care Provider Portal Training in Moodle—a video tutorial followed by a few questions. CLEAN WATER FOR US KIDS: The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. In the Clean Water for Carolina Kids facility data, the facility status for lead in water, lead based paint and asbestos has been completed, and the results are available. STAR RATED LICENSE (SRL): Hold harmless has been extended until the new QRIS is implemented (SB 425). This means that no child care facilities in cohort one will have a rated license assessment unless they voluntarily want to. Also, cohort two do not need to start a preparation year. More information will be forthcoming. QRIS: GET READY FOR THE 3s (ECERS-3, ITERS-3, FCCERS-3): The DCDEE and the North Carolina Rated License Assessment Project (NCRLAP) are collaboratively working on preparations for the transition to the ECERS-3, ITERS-3, and FCCERS-3 that starts February 1, 2025. Anissa Adcock, Administrator, and Elaine Manning, Director, were present during the visit. Ms. Manning accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Adcock and Ms. Manning. Limited monitoring of childcare requirements occurred during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. A staff member was unaware that a child was using the microwave to warm food. The child was burned when removing food from the microwave. 10A NCAC 09 .0601(a) 830 A First Aid information sheet was not posted in a place for referral and/or the information sheet did not include all the required information.
