Currently, this facility operates with a Four-Star license issued on November 29, 2017. The center's last annual compliance visit was completed on November 7, 2025. The last sanitation inspection was completed on August 27, 2025, earning a superior classification and fourteen (14) demerits. The last satisfactory fire inspection was completed on August 13, 2025. Carolyn Slade, Owner and Tiffany Baker, Administrator was present and available to assist. One hundred and eight (108) children were present during the visit. Children were observed in daily routine activities, teacher directed activities, outdoor play, and eating lunch. All staff interacted with the children in a positive manner. The center is owned and operated by Greater St. Steven Church of Christ. This corporation was researched in the North Carolina Secretary of State Corporation Database and found to have a Current/Active Status. The following violations of child care requirements were documented during the visit. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Prior to the expiration date of the qualification letter, four staff members did not complete and submit required forms to update the criminal background check. G.S. 110-90.2(b) & .2703(n)&(o) 1757 A valid qualification letter was not on file and available to review at the facility. A valid qualification letter was not on file for four staff members. G.S. 110-90.2(b) & (d) & .2703(e) The above violations must be corrected immediately. The letter must describe, in detail, specifically how each violation has been corrected and any supporting documents. You can mail or email me with verification of compliance regarding the violations that were documented. If you choose to mail, two signed and dated copies must be sent and received by May 6, 2026. Please be aware that any written information submitted by you regarding correction of the violations documented during today's visit is legal documentation. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules always if they have been monitored or specifically discussed with you in the past. The best way for you to make sure that you meet all requirements is to periodically review the child care law and rules. Compliance History: Prior to today's visit, this facility maintained an 18-month compliance score of 100%. CONSULTATION/TECHNICAL ASSISTANCE CRIMINAL BACKGROUND APPLICATIONS: A New feature links criminal background applications to facilities. The training has been completed and a roster was generated. However, all staff should be linked to the facility. Please remember, this information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Should you need assistance please contact the Criminal Background Check Unit at 1-800-859-0829 and someone will assist you. Four (4) staff members did not complete the process to update and obtain their criminal background check. All four staff qualification letters expired in March 2026. The criminal background check is valid for a maximum of five (5) years. C. Slade, D. Slade, J. Bullock, and T. Baker should complete the process immediately. Child care staff may remain at the facility as long as the violation is corrected within two weeks. Once qualification letters are obtained, you should notify me. CLEAN WATER FOR US KIDS: In the Clean Water for Carolina Kids facility data, status for lead in water status is “analysis complete”. The lead-based paint and asbestos status is “survey review by RTI”. To ensure compliance, you should log onto or contact Clean Water for US Kids for assistance to complete all steps to have a “section complete” status. STAR RATED LICENSE (SRL)/QRIS: An overview of section .3200 of the child care rules was provided on November 7, 2025. During today’s visit, we reviewed and discussed the QRIS Pathway to the Stars and your plan for transitioning to the new rated license. Your program is licensed for NCPREK, and you are considering pathway two (2). You stated you are working on your coaching and training for your staff and possibly will be finished by the end of May 2026. Please be reminded your application should be submitted by September 16, 2026. You should contact me when you are ready for your rated license assessment. Currently, this facility operates with a Four-Star license issued on November 29, 2017. The center's last annual compliance visit was completed on January 5, 2025. The last sanitation inspection was completed on May 7, 2025, earning a “Approved” classification and “16” demerits. The last satisfactory fire inspection was completed on March 8, 2024, and approved for day time and night care. T. Baker, Administrator and C. Slade, Owner/Operator, were present and available to assist. Seventy-two (72) children were present during the visit. Children were observed in free choice play and teacher directed activities. All staff interacted with children in a positive manner. The following violations of child care requirements were documented during the visit. TECHNICAL ASSISTANCE AND CONSULTATION: FIRE INSPECTION: The operator had a scheduled fire inspection to obtain a fire inspection. However, the fire inspector has conducted two visits and is requiring compliance with certain requirements. CRIMINIAL BACKGROUND APPLICATIONS: A New feature links criminal background applications to facilities. North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. This feature should be implemented to avoid violations. The training has been completed. However, all staff should be linked to the facility. A copy of the instructions was given to the provider during the visit. T. Baker criminal background check expired January 20, 2025. P. Williams criminal background check expired January 17, 2025. This was corrected during the visit because both staff members have current and valid qualification letters available for review. To ensure compliance with criminal background checks, printing the roster and posting it will help as a reminder to administration and staff when CBC are coming due. CLEAN WATER FOR US KIDS: The Clean Water for Carolina Kids status for lead in water is “analysis complete”. For the lead base paint and asbestos, the status is “survey review by RTI” and no results. This information was verified in the clean water for US kids facility data. You should log into your account to follow-up with the results for lead-based paint and asbestos. A checklist was used to note the requirements I monitored today. The last sanitation inspection was completed on March 24, 2024, earning a “Superior” classification and “13” demerits. The last fire inspection was completed on March 8, 2024, and was approved for day time and night care. T. Baker, Administrator and C. Slade, Owner/Operator, were present and available to assist. One hundred and six (106) children, age ranging from zero (0) to five (5) years old were present. Children were observed in free choice play, teacher directed activities, bottle feeding and preparing for lunch. All staff interacted with children in a positive manner. CONSULTATION/TECHNICAL ASSISTANCE CHILDREN AND STAFF RECORDS: Ten percent (10%) or more of children’s records were monitored. Children records reviewed, were in compliance. Other children’s files not reviewed should be monitored to ensure compliance with the same information. Ten percent (10%) or more of existing staff files were monitored. Staff records reviewed were not in compliance. One staff record reviewed did not complete the recognizing and responding to suspicion of child maltreatment training within five years of the last training date of February 12, 20219. However, the training was completed on October 22, 2024. This was corrected during the visit. Other staff files not reviewed should be monitored to ensure compliance with the same information. ACTIVITY PLAN: A current activity plan was not available for review in space two. This was corrected during the visit when the staff completed and posted the activity plan. 2024 ANNUAL LICENSE FEE: Keep an eye on your email for the 2024 annual license fee invoice. Invoices will be emailed to the facility addresses on file by November 30, 2024. Online payments will be due by December 31, 2024. CRIMNIAL BACKGROUND APPLICATIONS: A New feature links criminal background applications to facilities. North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. Provider Access to ABCMS allows administrators to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. To get started, complete the ABCMS Child Care Provider Portal Training in Moodle—a video tutorial followed by a few questions. CLEAN WATER FOR US KIDS: The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Currently operating child care facilities are required to test lead in paint and asbestos once only unless the consultant notes a concern during a visit and refers to the program to environmental health. All existing licensed programs should complete the application summary for their program regarding lead paint and asbestos. The application deadline has been extended. The application is on the https://www.cleanwaterforuskids.org/en/carolina/ website. In the facility data, status for lead water, lead based paint and asbestos shows the enrollment has started. STAR RATED LICENSE (SRL): Hold harmless has been extended until the new QRIS is implemented (SB 425). This means that no child care facilities in cohort one will have a rated license assessment unless they voluntarily want to. Also, cohort two do not need to start a preparation year. D. Wheeler, Group Leader assisted upon arrival. You (C. Slade) arrived shortly after the visit started. Forty-two (42) children ranging from ages zero to ten years old were observed in care today. Children were engaged in free-choice play with developmentally appropriate materials/toys, interacting with caregivers, completing toileting, diapering, and hand washing routines, and eating breakfast. Breakfast consisted of bagels, bananas, and milk. I completed a general walk-through of the center and conducted observation in spaces used for child care. Limited monitoring of the child care requirements was completed today. The following child care requirements were monitored: Adequate approved space, permit restrictions, staff/child ratio and supervision. The following violation was documented today: Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. Enhanced staff/child ratios requirements were not met when one caregiver was observed providing care for eight children ranging from ages one to two years old in Space #1. 10A NCAC 09 .2818 The violation documented must be corrected immediately. Please send a compliance letter describing how the violation was corrected. The compliance letter must be received on or before July 26, 2023. The corporation status for your program must always remain Current and Active. Failure to comply may affect your child care license. FACILITY PROFILE INFORMATION: You stated the mailing address, phone number and email address listed for your family child care home are correct. If changes in your facility information occurs, please contact your child care consultant April Fleming at telephone number 252-450-9796 or email april.fleming@dhhs.nc.gov to discuss the changes and ensure the information is updated in the system. COMPLIANCE HISTORY SCORE: The center’s compliance history score was reviewed with the operator. The center’s compliance history score was ninety (90%) percent as of July 11, 2023. STAFF/CHILD RATIO REQUIREMENTS: Based on my observation during the walk-through to obtain the center attendance, it was determined your program was out of compliance with enhanced staff/child ratio requirements. One caregiver was observed caring for eight (8) children ages one to two years old from 9:03am to 9:15am in Space #1. You stated you and other staff were late this morning due to handling personal crisis. You added staff would have been in place if they were able to arrive to work on time. We discussed the importance of always maintaining compliance with staff/child ratio requirements. It was suggested encouraging staff to transition children as needed based on ages and availability in each classroom to ensure staff/child ratio is maintained. It was also recommended having staff to use the two-way communication system so they can notify others when they have reached the staff/child ratio capacity for their classroom. I explained the goal is to ensure staff/child ratio is always maintained. I shared if two of the one-year-old children were transitioned to Space # 3, compliance would have been met. You stated you are the individual who usually transition children as needed; however, you were running late this morning. I encouraged you to continue implementing strategies that will help you and staff maintain awareness of staff/child ratios requirements for your program. The staff/child ratio chart posted can be used to help with determining when additional staff is required based on the number and ages of children in care. Be reminded you are responsible for always maintaining compliance with child care requirements. UNANNOUNCED FOLLOW-UP VISIT: An unannounced follow-up visit will be conducted in the future to monitor staff/child ratio requirements. ADMINISTRATIVE ACTION: I informed you based on documentation of violations for staff/child ratio requirements on two consecutive visits, an administrative action will be recommended for your program. If an action is issued, you will be notified by the Divisions of Child Development and Early Education. I recommended you review Child Care Rule 10A NCAC 09 .2201. D. Wheeler, Group Leader assisted upon arrival. You (C. Slade) arrived shortly after the visit started. Forty-two (42) children ranging from ages zero to ten years old were observed in care today. Children were engaged in free-choice play with developmentally appropriate materials/toys, interacting with caregivers, completing toileting, diapering, and hand washing routines, and eating breakfast. Breakfast consisted of bagels, bananas, and milk. I completed a general walk-through of the center and conducted observation in spaces used for child care. Limited monitoring of the child care requirements was completed today. The following child care requirements were monitored: Adequate approved space, permit restrictions, staff/child ratio and supervision. The following violation was documented today: Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. Enhanced staff/child ratios requirements were not met when one caregiver was observed providing care for eight children ranging from ages one to two years old in Space #1. 10A NCAC 09 .2818 The violation documented must be corrected immediately. Please send a compliance letter describing how the violation was corrected. The compliance letter must be received on or before July 26, 2023. FACILITY PROFILE INFORMATION: You stated the mailing address, phone number and email address listed for your family child care home are correct. COMPLIANCE HISTORY SCORE: The center’s compliance history score was reviewed with the operator. The center’s compliance history score was ninety (90%) percent as of July 11, 2023. STAFF/CHILD RATIO REQUIREMENTS: Based on my observation during the walk-through to obtain the center attendance, it was determined your program was out of compliance with enhanced staff/child ratio requirements. One caregiver was observed caring for eight (8) children ages one to two years old from 9:03am to 9:15am in Space #1. You stated you and other staff were late this morning due to handling personal crisis. You added staff would have been in place if they were able to arrive to work on time. We discussed the importance of always maintaining compliance with staff/child ratio requirements. It was suggested encouraging staff to transition children as needed based on ages and availability in each classroom to ensure staff/child ratio is maintained. It was also recommended having staff to use the two-way communication system so they can notify others when they have reached the staff/child ratio capacity for their classroom. I explained the goal is to ensure staff/child ratio is always maintained. I shared if two of the one-year-old children were transitioned to Space # 3, compliance would have been met. You stated you are the individual who usually transition children as needed; however, you were running late this morning. I encouraged you to continue implementing strategies that will help you and staff maintain awareness of staff/child ratios requirements for your program. The staff/child ratio chart posted can be used to help with determining when additional staff is required based on the number and ages of children in care. Be reminded you are responsible for always maintaining compliance with child care requirements. UNANNOUNCED FOLLOW-UP VISIT: An unannounced follow-up visit will be conducted in the future to monitor staff/child ratio requirements. ADMINISTRATIVE ACTION: I informed you based on documentation of violations for staff/child ratio requirements on two consecutive visits, an administrative action will be recommended for your program. If an action is issued, you will be notified by the Divisions of Child Development and Early Education. I recommended you review Child Care Rule 10A NCAC 09 .2201. VISIT SUMMARY DOCUMENTATION CORRECTION: I informed you the space documented for the violation in the visit summary report reviewed, signed, and left with you on June 28, 2023, was incorrect. The Space should have been documented as Space #1 instead of Space #2. You stated you were aware of the error and did not have any questions. The correction was made in the system and a revised summary report was printed and left with you today.
