The entire home is licensed for child care and must meet the applicable child care requirements. Although you designated space to be used for child care in a specified section of your home, the license is issued for the entire location. Any monitoring visit conducted may include spaces other than those you designated for child care. You, M. Harrison and P. Arrington, Operators were both present and available to assist with today’s visit. Your program currently operates with a three-star license issued May 29, 2019, earning 5 points in the education component, 2 points in the program standards component and 1 quality for restricting the infant capacity to no more than 2 children under one year of age. The last annual compliance visit was conducted on January 13, 2024. The compliance history was reviewed with the operator. The program’s compliance history was 98% as of November 17, 2025. One (1) child aged two years old was present during the visit. The child was observed playing with toys and interacting with the caregivers. The developmental needs were being met and the caregivers were interacting in a nurturing and positive manner. TRANSPORTATION: You stated you do not provide transportation. HOME OCCUPANCY PERMIT: Your home occupancy permit is current. The permit was issued on June 10, 2025, and will need to be renewed on or prior to June 11, 2026. FIRE INSPECTION: The last fire inspection was conducted on January 31, 2025, and will need to be completed on or before January 31, 2026. Please remember once the fire inspection is completed the inspection report must be sent to your child care consultant within a week of the inspection date. Including the Adjustments to the Appropriations Act (SB 357), which includes the $67.5 million for reduced compensation payments for the first two quarters of FY 24-25 and the DHHS Agency Bill (Senate Bill 425) which includes the QRIS provisions: • QRIS modernization (3 new pathways to stars) is now approved (SB 425). • Hold harmless has been extended until the new QRIS is implemented (SB 425). Providers in Cohort 1 are not required to go any further with a rated license assessment unless they want to, and providers in Cohort 2 do not need to start their preparation year unless they want to. • SB 425 does mean that programs currently accredited through the approved accrediting bodies can move to a 3- or 5-star license if they are not already at this star level. Providers will need to submit a request to their consultant and provide a copy of their accreditation certificate for processing. • SB 425 also requires that we move to the ERS-3 versions for ECCERS. ITERS and FCCERS, effective February 1, 2025. Self-paced online training modules regarding the new assessments will be available in September 2024. • SB 357 provides $67.5 million dollars to continue a reduced level of funding for the compensation portion only of the Child Care Stabilization Grant. You stated, “Mom brings in the bottle in the morning.” I reminded you that all food, cups, and bottles brought must be labeled with the child’s name and date. We also discussed if the parent forgets to label the bottles for you to label them with the child’s name and date. You labeled the three (3) bottles with the child’s name and date and put them back in refrigerator. This was corrected during the visit. SAFETY AND SUPERVISION (ELECTRICAL OUTLETS): There was an electrical outlet located to the right of the rocking chair in the caregiving space not covered by a protective outlet cover. You stated one of the household members forgot to cover the outlet after using it. You immediately cover the electrical outlet with a protective outlet cover. Before the children arrive for the day walk through the caregiving space and make sure all electrical outlets are covered. This was corrected during the visit. WRITTEN PLAN OF CARE: There was one child enrolled with an enrollment date of June 28, 2023, that did not have a written plan of care on file. The entire home is licensed for child care and must meet the applicable child care requirements. Although you designated space to be used for child care in a specified section of your home, the license is issued for the entire location. Any monitoring visit conducted may include spaces other than those you designated for child care. Upon arrival, you, M. Harrison, additional caregiver, was present and available to assist with today’s visit. You stated Peggy Arrington was at an appointment and would return shortly. Your program currently operates with a three-star license issued May 29, 2019, earning 5 points in the education component, 2 points in the program standards component and 1 quality for restricting the infant capacity to no more than 2 children under one year of age. The last annual compliance visit was conducted on February 13, 2023. The compliance history was reviewed with the operator. The program’s compliance history was 100% percent as of January 24, 2023. One (1) child aged ten (10) months old was present during the visit. The infant child was observed playing with toys and interacting with the caregiver and engaging in personal care routines. The developmental needs were being met and the caregivers were interacting in a nurturing and positive manner. HOME OCCUPANCY PERMIT: Your home occupancy permit was renewed on June 5, 2024, and will need to be renewed prior to June 5, 2025. You stated you do not provide transportation. The following violation(s) were documented: Any violations of the child care requirements observed today were discussed with you and documented in the visit summary report and reviewed with you at the conclusion of the visit. The following violations of the child care requirements were documented today: Violation Number Comment Rule 716 Electrical outlets not in use were not covered. There was an electrical outlet located to the right of the rocking chair in the caregiving space not covered by a protective outlet cover. 10A NCAC .1719(a)(27) 811 Baby food, snack items, and meal items sent from home, for each individual child, were not dated and labeled with the child's name. There were three (3) bottles in the refrigerator that were not labeled with the child’s name and date. .1706(o) 1544 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. The last fire inspection was conducted on January 11, 2023, and needed to be completed on or before January 11, 2024. GS 110-91 1718 The written plan of care was not given and explained to parents of children in care on or before the first day the child attended the home. Parents did not sign a statement acknowledging the receipt and explanation of the plan. Parents did not give written permission for their child to be transported by the operator for specific routine tasks that are included on the written schedule. There was one child enrolled with an enrollment date of June 28, 2023, that did not have a signed written plan of care on file. .1712(e )(6) 1948 Sleeping infants, ages 12 months and younger were not visually checked every 15 minutes. You have one (1) ten (10) month old enrolled in your program. The last sleep check available for review was conducted on September 22, 2023. There were no sleep checks conducted from September 23, 2023 through January 24, 2024. .1724(a)(7) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before February 8, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware that any written information submitted by you regarding correction of violations documented during today's visit is legal documentation. This child is required to have sleep checks completed every 15 minutes. Sleeping infants, ages 12 months and younger are required to be visually checked every 15 minutes. The last sleep check was conducted on September 22, 2023. Sleep checks were not available for review from September 23, 2023 through January 24, 2024. We discussed putting the sleep charts on the wall near the child’s sleeping area to help remind you to compete the sleep checks. NUTRITION AND FEEDING (INFANTS AND TODDLERS): There were three (3) bottles in the refrigerator that were not labeled with the child’s name and date. You stated, “Mom brings in the bottle in the morning.” I reminded you that all food, cups, and bottles brought must be labeled with the child’s name and date. We also discussed if the parent forgets to label the bottles for you to label them with the child’s name and date. You labeled the three (3) bottles with the child’s name and date and put them back in refrigerator. This was corrected during the visit. SAFETY AND SUPERVISION (ELECTRICAL OUTLETS): There was an electrical outlet located to the right of the rocking chair in the caregiving space not covered by a protective outlet cover. You stated one of the household members forgot to cover the outlet after using it. You immediately cover the electrical outlet with a protective outlet cover. Before the children arrive for the day walk through the caregiving space and make sure all electrical outlets are covered. This was corrected during the visit. WRITTEN PLAN OF CARE: There was one child enrolled with an enrollment date of June 28, 2023, that did not have a written plan of care on file. Please remember the written plan of care must be given and explained to parents of children in care on or before the first day of care.
