This facility operates with a Four-Star License issued June 26, 2019. The last annual compliance visit was completed April 2, 2025. The last sanitation inspection was completed on December 19, 2025, earning a "Superior" classification and "0" demerits. The last satisfactory fire inspection was completed on February 16, 2025, and was approved for day and night care. L. Hines, Teacher, were present and available to assist. Five (5) children were present during the visit. The children were observed completing teacher directed activities, during free choice play, toileting, handwashing, during lunch and preparing for nap/quiet time. Staff interacted with children in a positive manner. This facility operates as a center located in a residence and is licensed to serve twelve (12) children. Fire drills, shelter in place/lockdown drills, and playground inspections were all current. You stated you do not provide transportation. This program is owned and operated by Tiffany Dickens and Helena Dickens, Sole Proprietors. The following violations of child care requirements were documented. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A can of mosquito repellent and room freshener was observed under two different unlocked cabinets. .2820(b) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Prior to the expiration date October 15, 2025, the provider did not receive a current criminal qualification letter. Compliance History: Prior to today's visit, this facility maintained an 18-month compliance score of 95%. CONSULTATION/TECHNICAL ASSISTANCE CHILDREN AND STAFF RECORDS: Ten percent (10%) of children’s records were monitored. Children records reviewed, were in compliance. Other children’s files not reviewed should be monitored to ensure compliance for the same information. All records should always be available for review. Two (2) out of the nine (9) children enrolled did not have records available for review. Ten percent of existing staff files were monitored. Staff records reviewed were currently in compliance. However, prior to the expiration date October 15, 2025, a staff member criminal background qualification letter expired. The current letter is dated October 16, 2025. This was corrected during the visit. Other staff files not reviewed should be monitored to ensure compliance for the same information. HAZARDOUS ITEMS: A can of mosquito repellent and room freshener was observed under the handwashing sink in the classroom and bathroom. All aerosol cans under pressure should be kept in a locked cabinet or behind a locked door. To ensure compliance, I encourage you to print the hazardous items list off the DCDEE website under provider documents and forms and use as a reference. This was corrected during the visit when the teacher removed the cans and locked them in a cabinet. CRIMINAL BACKGROUND APPLICATIONS: A New feature links criminal background applications to facilities. The training has been completed; however, all staff should be linked to the facility. Please remember, this information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. CLEAN WATER FOR US KIDS: The Clean Water for Carolina Kids status for lead in water is “in progress”. The status for lead base paint and asbestos is “Section Complete”. STAR RATED LICENSE (SRL/QRIS): An overview of section .3200 of the child care rules was provided to the facility in October 2025. Upon arrival, children were on a walk through the neighborhood. Ten (10) children were observed when arriving back to the center. The children were observed toileting, handwashing, during lunch and preparing for and during nap/quiet time. Staff interacted with children in a positive manner. This center in residence is licensed to serve twelve (12) children. Fire drills, shelter in place/lockdown drills, and playground inspections were all current. A checklist was used to note the Center in Residence requirements monitored today. T. Richardson, T. Patterson and L. Hines, Teachers were present and available to assist. Upon arrival, fourteen (14) children were observed with two teachers. The children were aged zero (0) to five (5) years of age. The children were observed during teacher directed activities, outdoor play and during lunch. Staff interacted with children in a positive manner. Two staff members arrived later during the visit. This center in residence is licensed to serve twelve (12) children. Fire drills, shelter in place/lockdown drills, and playground inspections were all current. CAPACITY: Fourteen (14) children ranging from 0-5 years old were present with two staff members when I arrived. When asked, T. Patterson stated three of the children were from their other center, Westside Community Learning Center. The three children were removed from the classroom by T. Dickens and transitioned back to the other location. To ensure compliance, the number of children present should always be within permit capacity. STAFF CHILD RATIO: Ten (10) children ranging from 1-3 years of age were on the playground with one staff member for about 10-15 minutes. To ensure compliance with staff child ratio, staff should refer to the staff child ratio chart for centers located in a residence. CRIMINIAL BACKGROUND APPLICATIONS/CRIMINAL BACKGROUND RENEWAL: A New feature links criminal background applications to facilities. North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. This feature should be implemented to avoid violations. To get started, complete the ABCMS provider portal training in MOODLE. This is a reminder that three staff members criminal backgrounds will expire this year in the months of June, September and October. I encourage you to be proactive and ensure steps are completed and the process is completed prior to expiration of current qualification letter.
